OBJECTIVE: To report the result of a severe articular defect secondary to a complex open fracture of the distal humerus, using the ipsilateral radial head as a bone autograft for the trochclear region as an alternative to total arthroplasty.
CASE REPORT: We describe a patient who suffered an open fracture of the elbow with bone loss in the distal humeral region and, after the initial stabilisation surgery, needed a reconstruction with an ipsilateral radial head autograft.
DISCUSSION: Open fractures of elbow are rare. When there is a severe trochlear defect, surgical options are total elbow a

BACKGROUND: The surgical management of distal humeral fractures remains challenging. Posterior approaches involving the triceps mechanism are crucial for achieving stable fixation and functional recovery but are associated with potential approach-related complications. Traditional approaches often involve olecranon osteotomy to achieve adequate exposure, but this technique is associated with specific complications. The purpose of the present study is to describe the clinical outcomes and surgical rationale of the anconeus-triceps lateral flap (ATLF) approach applied to open reduction and inter

INTRODUCTION: Conventional open reduction and internal fixation of the humeral shaft often has a high chance of non-union and delayed union, because of primary healing. Furthermore, a shaft humerus fracture can be managed by conservative means. However, the problem of conservative management is stiffness of joints and the possibility of delayed union of fractures. Minimally invasive percutaneous plate osteosynthesis (MIPPO) utilizes the principles of both conservative and surgical management. Healing is through secondary healing, as in conservative management, while maintaining the principles

INTRODUCTION: Lateral humeral condyle fractures typically require a longer period of internal fixation than other distal humeral fractures due to the increased risk of non-union. K-wires can be buried and left in situ until union or they can be left unburied and require removal after four weeks, with plaster immobilisation until union. There is no consensus as to whether wire burial is preferable or not. The aim of this study was to determine whether K-wire burial is associated with more complications than non-buried wires in treating lateral condyle fractures of the elbow.

OBJECTIVE: To investigate the surgical approach, fixation, and clinical effect of comminuted coronal shear fracture of distal humerus.
METHODS: From March 2017 to February 2019, we had used open reduction and internal fixation to treat 19 cases of comminuted distal humeral coronal shear fracture. There were 8 males and 11 females, with an average age of 44.6 years (19-72 years). There were 10 cases on left side and 9 cases on right side. All cases were closed fractures. According to Dubberley's classification, there were six cases of type 1, six cases of type 2, and seven cases of type 3. The

BACKGROUND: Proximal humeral shaft fractures are surgically challenging and plate osteosynthesis with a long straight plate is one operative treatment option in these patients although endangering the radial nerve distally. Helical plates potentially avoid the radial nerve by twisting around the humeral shaft. Aim of the study was to investigate in a human cadaveric model the biomechanical competence of helical plates versus straight lateral plates used for fixation of proximal third comminuted humeral shaft fractures.
METHODS: Eight pairs of humeral cadaveric humeri were instrumented using e

Distal third comminuted humerus shaft fractures are challenging injuries to treat because of complex anatomy and fracture patterns. Functional bracing, operative treatment with intramedullary nails or conventional plates also has limitation of inadequate fixation in the distal third comminuted humerus shaft fractures. Locking compression plate (LCP) has been introduced to overcome this problem. Our aim of this study was to assess the effectiveness of osteosynthesis of distal third comminuted humerus shaft fractures by LCP using posterior approach. This prospective observational study was condu

OBJECTIVES: Total elbow arthroplasty (TEA) is a viable treatment for elderly patients with distal humerus fracture who frequently present with low-grade open fractures. This purpose of this study was to evaluate the results of a protocol of serial irrigations and debridements (I&Ds) followed by primary TEA for the treatment of open intra-articular distal humerus fractures.
METHODS: Seven patients (mean 74 years; range 56-86 years) with open (two Grade I and five Grade 2) distal humerus fractures (OTA 13C) who were treated between 2001 and 2007 with a standard staged protocol that included TEA

BACKGROUND: Distal humeral fractures accounted for ~1% of all fractures; however, they were prone to complications, including nonunion if left untreated or inadequately managed. Nonunion, which predominantly occurred at the supracondylar level, resulted in mechanical instability, functional impairment, and persistent discomfort. The most commonly employed surgical options included open reduction and internal fixation (ORIF), total elbow arthroplasty (TEA), and external fixation. This article provides a comprehensive assessment of these surgical procedures and shared clinical experiences relate

We studied 28 displaced (Jacob types II and III) fractures of the lateral humeral condyle in 28 children. There were only two Milch type I fractures. Twenty-one fractures were treated by open reduction and internal fixation with K-wires, Palmer nails, or sutures. Seven patients were treated with a plaster cast, five following closed reduction, and the primary displaced position being accepted in two. Anatomic reduction was obtained in 18 patients. All but one fracture were united at review 2 years to 16 years later. The distal humerus was wider on the injured side in all patients. Six patients

BACKGROUND: Shear humeral capitellum fracture (CF) treatment can be complicated by comminution of the distal lateral humeral column (LHC). Although treatment with a 3.5 mm posterolateral distal humerus plate with support (PDHPWS) has been proposed, its indications have not yet been outlined. The purpose of this study was to describe a classification system for this fracture pattern and provide a therapeutic algorithm to avoid complications associated with PDHPWS fixation.
METHODS: Thirty-four patients who underwent surgical treatment for CF with LHC comminution were enrolled. The humeral capi

BACKGROUND: Open reduction internal fixation (ORIF) is the gold standard management of fractures of the distal humerus. Stable fixation to allow early mobilization is not always possible in cases with comminuted fracture patterns and bone loss, with a high failure rate. We propose augmentation of internal fixation in these unstable situations with a spanning plate across the elbow to protect the fixation construct temporarily until bone union.
METHODS: Eighteen patients with complex distal humeral fractures were managed with standard ORIF technique augmented with a temporary plate spanning ac

BACKGROUND: Overuse injuries are a frequent occurrence among competitive athletes. When analysing the incidence of overuse injuries in tennis players, it has been determined that a significant number of these injures occur in the upper limb area. In this study, we describe five cases of a stress-induced injury to the middle and distal humerus occurring mainly due to repetitive serving.
METHODS: Athletes studied were competitive tennis players and elite junior players, two of whom played at international level. Four of the five were male. In all cases, diagnosis was confirmed by magnetic reson

PURPOSE: There is still no consensus about treatment distal humerus fractures (DHF) in elderly patients. This retrospective study compare the mid-term outcomes of intraarticular DHF in patients over 60 years of age treated with open reduction and internal fixation (ORIF) and total elbow arthroplasty (TEA).
METHODS: We have evaluated 64 patients who have undergone treatment for intra-articular DHF (AO types C, 31 TEA vs. 33 ORIF). Our assessment included: range of motion, Mayo Elbow Performance Score (MEPS), Quick-DASH score and numeric pain score (NRS). Rate of complications and reoperations

Adolescent distal humerus fractures usually result from high-energy trauma; however, open fractures with associated bone loss are quite rare. Although the Masquelet technique has demonstrated proven success in the reconstruction of lower extremity bone defects, its use in upper extremity periarticular defects, especially in adolescents, has been very rarely reported. This study aims to present the clinical and radiological outcomes of the Masquelet technique in adolescent distal humerus fractures with bone loss. A retrospective review was conducted of patients aged 12-16 years who underwent th

Introduction Fractures of the distal humerus in the adult comprise approximately one-third of all humeral fractures. Over the past 20 years, nonoperative treatment for these fractures has been substituted by anatomic reduction and internal fixation based on the Association for Osteosynthesis (AO)/Association for the Study of Internal Fixation (ASIF) philosophy of plate fixation which resulted in early mobilization and superior performance. Pre-contoured, anatomically designed locking plates are anticipated to offer sufficient stability, permit early elbow range of motion, and safeguard the sof

AIMS: To compare the accuracy of radiography and CT to diagnose anatomical surgical reduction of lateral humeral condylar fractures using an ex vivo canine model.
METHODS: Ten right and left pairs of thoracic limbs were obtained from fresh canine cadavers. Lateral humeral condylar fractures were created and reduced, with or without an articular step-off (0-, 1-, 2-, 3-mm in a caudal or distal direction), and stabilised using a transcondylar screw and a supracondylar pin. Mediolateral and craniocaudal radiographs of each limb were taken, followed by CT imaging in a sagittal, dorsal, and transv

BACKGROUND: Snapping triceps is a dynamic condition in which a portion of the medial head of the triceps dislocates over the medial epicondyle during flexion or extension. Pushed by the triceps, the ulnar nerve typically also dislocates over the medial epicondyle, causing neuropathy. Posttraumatic cubitus varus deformities resulting from pediatric supracondylar fractures have been associated with snapping triceps. This is the first case of snapping triceps associated with cubitus varus due to distal humeral malunion, which occurred in an adult.
OBSERVATIONS: A 23-year-old man sustained a left
