BACKGROUND AND AIMS: Reactive hemophagocytic syndrome (RHS) is a rare disease in which inappropriately activated macrophages consume bone ma … erapy, with nearly half taking >1 agent. All patients presented with fever, leukopenia, anemia, and hyperferritinemia. Infection by a member of the herpesvirus family or an intracellular pathogen precipitated RHS in 6 of 7 patients. The mortality rate was 29%. The diagnosis of RHS should be considered in patients with IBD taking immunosuppressive therapy who presen …

Haemophagocytic syndrome (HS), also referred to as haemophagocytic lymphohistiocytosis or macrophage activation syndrome, comprises a heterogeneous group of disorders featuring sepsislike characteristics typically combined with haemophagocytosis, hyperferritinemia, hypercytokinemia and variable cytopenias, often resulting in fatal multiple organ failure. The availability of widely accepted diagnostic and therapeutic guidelines for the hereditary, paediatric forms of HS has improved outcome and lead to a better pathophysiological understanding. Although similar, reactive (secondary) HS in adult…

Bone marrow biopsy demonstrated hemophagocytosis but was negative for fungal organisms on Grocott methenamine silver staining. Macrophage activation syndrome (MAS) is part of a spectrum of cytokine storms. This case report describes a presentation of an 18-year-old female with systemic lupus erythematosus (SLE) complicated by MAS. The patient, previously healthy, presented with a 3-week history of high-grade intermittent fever, abdominal pain, nausea, and loss of appetite. Clinical examination revealed fever, tachycardia, and normal blood pressure without signs of lymphadenopathy or organomegaly. Laboratory tests indicated anemia, thrombocytopenia, leukopenia, neutropenia, elevated liver enzymes, hyperferritinemia, hy…

This study evaluated the clinical characteristics, treatment outcomes, and complications of patients with adult onset Still's disease (AOSD) … nts, with elevated erythrocyte sedimentation rates (77.05%) and C-reactive protein levels (84.06%). Hyperferritinemia was present in 74.14% of cases, and serum ferritin (SF) levels declined after treatment in most cases. Liver abnormalities were usually transient, but were more severe in 5 patients. Most patients (92%) required corticosteroid therapy; of these, 33. …

The classification of hereditary abnormalities of iron metabolism was recently expanded and diversified. Genetic hemochromatosis now corresp … patz syndrome is caused by a pantothenate kinase 2 gene (PANK-2) defect; neuroferritinopathies; and hyperferritinemia--cataract syndrome due to a mutation within the L-ferritin gene. In addition to this wide range of genetic abnormalities, two other features characterize these iron disorders: 1) most are transmitted by an autosomal recessive mechanism, but some, in …

The authors present the case of a 58-year-old man with the unique combination of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection and, later on, haemophagocytic lymphohistiocytosis admitted to the intensive care unit. During his ICU stay the patient developed a variety of complications including acute respiratory distress syndrome, pulmonary embolism, right heart failure and suspected HLH leading to multiorgan failure and death. Despite the proven diagnosis of haemophagocytic lymphohistiocytosis, the excessively high ferritin levels of the patient did not seem fully expla

Leukemic transformation of polycythemia vera (PV) to acute myeloid leukemia (AML) with a predominantly erythroid blast population is rare and carries a poor prognosis, particularly when driven by a TP53 mutation. We report a 55-year-old man with JAK2 V617F-positive PV, previously noted on surveillance biopsy to harbor a TP53 mutation and Grade 2 marrow fibrosis, who presented three years later with pancytopenia, schistocytosis, hyperferritinemia, hypofibrinogenemia, and altered mental status. The initial clinical presentation raised concern for thrombotic thrombocytopenic purpura (TTP), while …

BACKGROUND: We sought to screen for clinical and laboratory features of hemophagocytic lymphohistiocytosis among pediatric patients with severe sepsis.
METHODS: We conducted a retrospective study that analyzed the clinical and laboratory data of 70 pediatric patients who died of severe sepsis. Medical records were revised for the presence of fever, splenomegaly, pancytopenia, hyperferritinemia, hypertriglyceridemia, and hypofibrinogenemia. Soluble CD25 was measured in stored samples.
RESULTS: Patients' ages ranged between 0.5 and 11 years with median (interquartile range) 2 (1-5). All patien…
