Phosphate binders and the gut microbiota in chronic kidney disease: mechanisms, mixed evidence, and clinical considerations.

Chronic kidney disease (CKD) disrupts the gut microbiome through dietary restrictions, uraemia, and polypharmacy, including phosphate binders. This dysbiosis contributes to systemic inflammation, accumulation of uremic toxins, and reduced short-chain fatty acid (SCFA)-producing bacteria. Hyperphosphatemia, a key CKD complication, typically emerges in advanced stages. This review examines the impact of phosphate binders on gut microbiota and explores emerging biological therapies. Phosphate binders are standard treatment for hyperphosphatemia but may influence gut microbiota by altering luminal…

PA21, a novel phosphate binder, improves renal osteodystrophy in rats with chronic renal failure.

The effects of PA21, a novel iron-based and non-calcium-based phosphate binder, on hyperphosphatemia and its accompanying bone abnormality in chronic kidney disease-mineral and bone disorder (CKD-MBD) were evaluated. Rats with adenine-induced chronic renal failure (CRF) were prepared by feeding them an adenine-containing diet for four weeks. They were also freely fed a diet that contained PA21 (0.5, 1.5, and 5%), sevelamer hydrochloride (0.6 and 2%) or lanthanum carbonate hydrate (0.6 and 2%) for four weeks. Blood biochemical parameters were measured and bone histomorphometry was performed for…

Congestive heart failure: an unusual presentation of pseudohypoparathyroidism.

We report on an infant presenting with acute cardiogenic shock. She was eventually diagnosed with pseudohypoparathyroidism, which is a heterogeneous group of disorders characterized by severe hypocalcemia, hyperphosphatemia, and increased parathyroid hormone. The patient responded dramatically to calcium and vitamin D supplementation; left ventricular systolic function was normalized within days of treatment. Although the diagnosis of pseudohypoparathyroidism is rare, this case is a reminder of the importance of obtaining calcium levels in patients presenting in acute cardiogenic shock.

New clinical trials with vitamin D and analogs in renal disease.

Two new clinical trials highlight refinements in the use of vitamin D and its analogs in the treatment of secondary hyperparathyroidism in end-stage renal disease (ESRD), and the treatment of proteinuria in diabetics. In patients with ESRD, alfacalcidol is as effective as paricalcitol in suppressing parathyroid hormone; the occurrence of hypercalcemia and hyperphosphatemia is infrequent and similar with the two analogs. Oral cholecalciferol reduces albuminuria and urinary transforming growth factor-β1 in patients with type 2 diabetes mellitus and proteinuria.

Secondary hyperparathyroidism: pathophysiology and treatment.

Secondary hyperparathyroidism is a frequently encountered problem in the management of patients with chronic kidney disease (CKD). Its pathophysiology is mainly due to hyperphosphatemia and vitamin D deficiency and resistance. This condition has a high impact on the mortality and morbidity of dialysis patients. Early diagnosis of secondary hyperparathyroidism is crucial in the management of patients with CKD. The treatment remains a challenge for patients and their clinicians. It should include a combination of dietary phosphorus restriction, phosphate binders, vitamin D analogues, and calcimi…

The surgical management of renal hyperparathyroidism.

Secondary and tertiary hyperparathyroidism (HPT) develop in patients with renal failure due to a variety of mechanisms including increased phosphorus and fibroblast growth factor 23 (FGF23), and decreased calcium and 1,25-dihydroxy vitamin D levels. Patients present with various bone disorders, cardiovascular disease, and typical laboratory abnormalities. Medical treatment consists of controlling hyperphosphatemia, vitamin D/analog and calcium administration, and calcimimetic agents. Improved medical therapies have led to a decrease in the use of parathyroidectomy (PTX). The surgical indicatio…

Suspected familial renal disease in chow chows.

Renal disease was diagnosed in 6 young Chow Chows. Clinical abnormalities included vomiting, polyuria, polydipsia, and weight loss. Common abnormal laboratory findings were azotemia, hyperphosphatemia, hypocalcemia, nonregenerative anemia, and low urine specific gravity. All 6 dogs had similar microscopic renal lesions. characterized by interstitial fibrosis, a population of small glomeruli, and lack of inflammatory cells. A familial basis for the renal disease is suggested because of its development in 4 closely related dogs.

Intraarticular heterotopic ossification as the initial manifestation in a child with pseudohypoparathyroidism 1a.

The authors describe a 6 year old girl who initially presented with limping due to intraarticular ossification of left knee joint but later developed carpopedal spasms. She had Albright's hereditary osteodystrophy phenotype with hypocalcemia and hyperphosphatemia with elevated parathyroid hormone. She was diagnosed to have pseudohypoparathyroidism 1a (PHP1a) with heterotopic intraarticular ossification. This case emphasizes the need for evaluation of PHP in patients with intraarticular ossification to facilitate early diagnosis of PHP.

Importance of plasma phosphate determination.

Phosphate is an important component of all tissues and disorders of phosphate homeostasis are common in hospital populations. Hypophosphatemia, which is much more frequent than hyperphosphatemia, is commonly caused by infusion of carbohydrate or respiratory alkalosis. If hypophosphatemia is prolonged, severe consequences such as hemolysis, myopathy, and respiratory dysfunction may occur. In order to prevent these complications it is important to measure plasma phosphate concentration in a number of clinical situations. If severe hypophosphatemia is detected, phosphate supplements should be giv…

Pseudohyperphosphatemia related to a monoclonal immunoglobulin in a patient with plasma cell leukemia: an analytical interference to be aware of.

We report a case of pseudohyperphosphatemia in a patient with multiple myeloma that progressed to plasma cell leukaemia. This pseudohyperphosphatemia was caused by an analytical interference between monoclonal immunoglobulin and ammonium molybdate, a reagent used for measuring inorganic phosphate. Accurate measurement of blood inorganic phosphate levels requires prior protein precipitation to eliminate assay interference. Extrarenal purification treatments helped reduce the monoclonal immunoglobulin concentration and eliminated the interference during phosphate measurement. Pseudohyperphosphat…

Phosphate in Cardiovascular Disease: From New Insights Into Molecular Mechanisms to Clinical Implications.

Hyperphosphatemia is a common feature in patients with impaired kidney function and is associated with increased risk of cardiovascular disease. This phenomenon extends to the general population, whereby elevations of serum phosphate within the normal range increase risk; however, the mechanism by which this occurs is multifaceted, and many aspects are poorly understood. Less than 1% of total body phosphate is found in the circulation and extracellular space, and its regulation involves multiple organ cross talk and hormones to coordinate absorption from the small intestine and excretion by th…

Pseudohypoparathyroidism: a diagnosis to consider once a PTH elevation is detected.

BACKGROUND AND AIM: Pseudohypoparathyroidism (PHP) is a rare disease, which can occur in the youth, characterized by hypocalcemia and hyperphosphatemia due to resistance to parathyroid hormone (PTH) in target organs. This condition encompasses different conditions which differ between one another by different clinical, biochemically, and genetic features. METHODS: Herein we report the clinical history of a boy with PHP1B with an interesting clinical presentation. He came in fact to the attention of the Emergency Department because of a spontaneously resolving epileptic attack, lasting about 1…

Primary hyperphosphatemic tumoral calcinosis: a case report.

Tumoral calcinosis (TC) is a rare disease characterized by periarticular soft tissue calcification. Some cases were reported in Africa and the Middle East. We report an 11-year-old Chinese girl presenting with recurrent multiple subcutaneous masses around the right elbow and hip regions. Although we found abnormalities in FGF23, a protein associated with phosphate metabolism, no positive results were observed in gene sequencing and analysis. The imaging features, laboratory examination, and pathology results confirmed our diagnosis. By using oral phosphorus-lowering drugs (acetazolamide) combi

Therapeutic success of sodium thiosulfate in treating cutaneous calciphylaxis in a patient with hyperphosphataemic familial tumoral calcinosis.

Calciphylaxis is a potencially disorder in patients with hyperphosphatemic familial tumoral calcinosis (HFTC). Patients commonly present livedo racemosa and retiform purpura, which may progress to necrosis and very painful ulcers. Treatment with sodium thiosulfate provides good results; however, intralesional and intravenous treatment can be limited by its adverse effects. Topical sodium thiosulfate has been successfully reported for cutaneous calcification associated with connective tissue diseases and calciphylaxis in patients with chronic kidney disease. We provide a case report of a patien

[Pediatric reference values for alkaline phosphatase and pathophysiological variations].

Alkaline phosphatase activity is a parameter included in biochemical liver test. These isoenzymes are of various cellular origin inducing physiological variations on age and sex. The alkaline phosphatase activity standardization as well as numerous international studies have made it possible to standardize the pediatric reference values. The hyperphosphatasemia etiologies are very well know but the hypophosphatasemia are hardly explored and can allow the diagnosis of pathologies including hypophosphatasia, a rare treatable disease.
