OBJECTIVE: A topic of current research is the development of a new approach to the diagnosis and treatment of severe brain injury taking into consideration its main pathophysiological mechanism-idiopathic intracranial hypertension syndrome. We present the case of a 38-year-old woman who was treated for a heterotopic interstitial (cornual) pregnancy diagnosed at the 7th week of gestation. The intervention was performed via transvaginal ultrasound-guided aspiration and instillation of a hypertonic solution of sodium chloride into the cornual sac. The heterotopic comrnual pregnancy was successfully aborted, and the intrauterine pregnancy was successfully maintained with delivery of a healthy newborn.

A case of central pontine myelinolysis (CPM) in an alcoholic patient with severe electrolyte changes is presented. Data in the literature suggest that it is safe to correct severe symptomatic hyponatremia to a value of 125-130 mEq/1 in 24 h. At the present time acute severe hyponatremia carries a bad prognosis if not treated with hypertonic NaCl solution. Electrolyte abnormalities are not the sole cause of CPM.

Kainic acid (KA) and muscimol were injected into the lateral preoptic area (LPO) of the rat to study their effects on drinking behavior. Primary muscular hydatidosis is an infrequent parasitic infection, caused by Echinococcus granulosus. We report 2 cases of female patients with diagnosis of primary muscular hydatidosis of the thigh. Both hail from an endemic area for hydatidosis, and had no pulmonary, hepatic, or systemic involvement. Cyst extraction was performed after the patients were given hypertonic solution, and antiparasitic treatment.

A smoking patient presented with altered mental status, left sided weakness, and an opacity on the chest roentgenogram. During hospitalization he developed hyponatremia, which was corrected with hypertonic saline solution, fluid restriction, and demethylchlortetracycline. The clinical presentation and the laboratory findings supported the diagnosis of the syndrome of inappropriate secretion of antidiuretic hormone. This was believed to be associated with the pulmonary carcinosarcoma documented at necropsy.
