Redefining hyperuricemia in chronic kidney disease: a focus on tubulo-glomerular urate handling.

Scheme of a proposed classification of hyperuricemia. A proposed scheme of hyperuricemia integrating renal uric acid handling. Renal underexcretion type can occur due to reduced glomerular filtration rate (glomerular under-filtration type) or increased reabsorption of urate in the renal tubules (tubular over-reabsorption type).

Hyperuricemia Exacerbates Psoriatic Inflammation by Inducing M1 Macrophage Activation and Th1 Cell Differentiation.

A higher prevalence of hyperuricemia is observed in psoriasis, yet the precise involvement of hyperuricemia in psoriasis remains unclear. Therefore, we investigated the relationship between hyperuricemia and psoriasis, as well as the potential mechanisms through which hyperuricemia may promote psoriatic inflammation. Firstly, a literature review on psoriasis and serum uric acid (SUA) levels and a retrospective analysis on PASI scores and SUA of 147 psoriasis patients at the Dermatology Hospital of Southern Medical University were performed. Then mouse models of hyperuricemia and psoriasis were…

Association between gout, hyperuricemia, and Parkinson's disease: a systematic review and meta-analysis.

BACKGROUND: The association between gout, hyperuricemia, and Parkinson's disease (PD) has been widely debated with conflicting findings across studies. Although the antioxidant properties of urate suggest a potential neuroprotective role, chronic inflammation in gout may negate this benefit. OBJECTIVES: To systematically evaluate the relationship between gout, hyperuricemia, and the risk of developing PD through a comprehensive review and meta-analysis of observational studies. METHODS: Following PRISMA guidelines, we systematically searched PubMed, Web of Science, Scopus, and Embase (incept…

Hyperuricemia in Chinese Patients with Mood Disorders: Prevalence, Related Factors, and Predictive Model.

PURPOSE: Previous studies have shown that serum uric acid (UA) levels are significantly higher in patients with bipolar disorder (BD) than in patients with depressive disorder (DD), schizophrenia, and healthy controls. Currently, studies generally report that there is a complex bidirectional interaction between mood disorders (MD) and hyperuricemia (HUA). We investigated the prevalence and related factors of hyperuricemia in Chinese patients with mood disorders to find out potential mechanisms and build a predictive model. PATIENTS AND METHODS: A total of 771 patients with mood disorders who …

Anti-hyperuricemia effect of Clerodendranthus spicatus: a molecular biology study combined with metabolomics.

Hyperuricemia (HUA), a metabolic disease caused by excessive production or decreased excretion of uric acid (UA), has been reported to be closely associated with a variety of UA transporters. Clerodendranthus spicatus (C. spicatus) is an herbal widely used in China for the treatment of HUA. However, the mechanism has not been clarified. Here, the rat model of HUA was induced via 10% fructose. The levels of biochemical indicators, including UA, xanthine oxidase (XOD), adenosine deaminase (ADA), blood urea nitrogen (BUN), and creatinine (Cre), were measured. Western blotting was applied to explo…

β-Carboline alkaloid harmaline alleviates hyperuricemia-mediated renal inflammation by suppressing oxidative stress.

Hyperuricemia is a metabolic disorder marked by elevated serum uric acid levels and is closely linked to the development of gout and progressive renal dysfunction. It is increasingly recognized as a major public health concern due to its association with chronic kidney disease and cardiovascular complications. Hyperuricemia induces kidney impairment through a complex interplay of oxidative and inflammatory stress. Harmaline, a β-carboline alkaloid primarily found in Peganum harmala, exhibits antioxidant and anti-inflammatory activities. Considering the roles of oxidative and inflammatory stres…

An herbal formula alleviates hyperuricemia: Evidence from network pharmacology and in vivo experiments.

Qianhua Tongfeng Capsule (QHTFC), an herbal formula, has demonstrated favorable therapeutic efficacy in the clinical management of hyperuricemia (HUA) and gout. Phytochemical analysis revealed that QHTFC is primarily composed of coumarins, flavonoids, and alkaloids. Nevertheless, the anti-hyperuricemic mechanisms of QHTFC remain to be further elucidated. This study aimed to elucidate the molecular targets and potential mechanisms of QHTFC in the treatment of hyperuricemia (HUA) by combining network pharmacology with in vivo experimental validation. Network pharmacology was employed to predict …

Triphala ameliorates hyperuricemia-associated nephropathy by modulating the gut microbiota-glycerophospholipid-TLR4 axis.

BACKGROUND: Hyperuricemia (HUA) is a metabolic disorder with an escalating global prevalence, and its progression to HUA-associated nephropathy represents an important contributor to chronic kidney injury. Emerging evidence highlights a bidirectional relationship between uric acid and lipid metabolism, with dyslipidemia contributing to the pathological progression of HUA-related complications. Triphala (TRP), a classic Tibetan herbal formula with broad metabolic regulatory properties, was selected as a potential intervention for the complex metabolic disturbances associated with HUA. However, …

Anti-hyperuricemia therapy alleviates diet-induced gut inflammation via AhR-IL-22-signalling.

High-fat diets (HFD) are implicated in metabolic disorders through mechanisms involving hyperuricemia, gut dysbiosis, and intestinal barrier dysfunction. This study investigated the effects of pharmacological uric acid reduction on gut homeostasis in HFD-fed mice. We assessed serum uric acid, gut microbial composition, pro- and anti-inflammatory cytokine expression, intestinal barrier dysfunction markers, and tryptophan metabolism in mice subjected to HFD with or without allopurinol treatment. AhR antagonist CH-223191 was used to provide functional evidence for the role of AhR activation in me…

Targeting amino acid metabolic pathways: a novel therapeutic strategy for hyperuricemia-associated complications.

Hyperuricemia (HUA) is a metabolic disorder that contributes to the pathogenesis of gout arthritis (GA), chronic kidney disease (CKD), and cardiovascular disease (CVD). While current urate-lowering therapies effectively reduce serum uric acid levels, they fail to address the underlying metabolic dysregulation driving HUA progression and associated tissue damage. Emerging evidence highlights that dysregulated amino acid (AA) metabolism in bone, kidney, and vascular tissues plays a pivotal role in HUA-related complications. This review synthesizes recent advances in understanding AA metabolic pa…

URAT1 inhibition in hyperuricemia and gout: from transporter biology to clinical precision therapy.

Impaired renal urate excretion is a major mechanism underlying hyperuricemia and gout, with urate transporter 1 (URAT1), encoded by SLC22A12, playing a central role in proximal tubular urate reabsorption. This review summarizes the biological relevance of URAT1, the pharmacological evolution of URAT1 inhibitors, and their clinical implications in urate-lowering therapy. Evidence from transporter biology, structural pharmacology, pharmacokinetic and pharmacodynamic studies, and clinical trials was narratively synthesized. URAT1 inhibitors lower serum urate by blocking renal tubular urate reabso…

Cyclodextrin-engineered bioinspired nanozyme microneedle for adaptive theranostic regulation of hyperuricemia and gout.

Engineering cyclodextrin-based materials enabling real-time diagnosis and on-demand therapy is highly desirable for hyperuricemia and gout management. Here, we design a novel cyclodextrin-engineered bioinspired nanozyme (T-CD/Pt), and integrate it into a bi-regional microneedle (TP-MNs). Coupled with a biowaste-derived coffee ground photothermal film (CPS-F), the resulting CPS-F/TP-MNs platform enables localized hyperthermia and significantly enhances nanozyme catalytic activity. In T-CD/Pt, the cyclodextrin cavities function as biomimetic substrate-binding pockets, enabling selective enrichme…

Hyperuricemia accelerates renal dysfunction due to hypertensive nephrosclerosis: the TAMA MED Project-CKD.

BACKGROUND: Nephrosclerosis has become an increasingly common underlying cause of renal replacement therapy worldwide. Hypertension is the primary cause of nephrosclerosis; however, other complex factors are believed to contribute to its onset and progression. In this study, we aimed to investigate whether hyperuricemia is a risk factor for the onset of chronic kidney disease and for progression of renal dysfunction. Additionally, we sought to determine whether the coexistence of hypertension and hyperuricemia exacerbates these risks. METHODS: A follow-up survey was conducted among 11,043 indi…
