Post-hysteroscopic ablation reaction: a histopathologic study of the effects of electrosurgical ablation.

Ablation of the endometrium is now an established treatment modality for dysfunctional uterine bleeding (DUB). There have been few reports of the inflammatory and healing reaction within the endomyometrium subsequent to this therapy, and its identifying characteristics. The morphologic reaction after electrosurgical ablation in particular has received scant attention. In this retrospective study, 19 women were identified who had undergone both an endometrial ablation using electrosurgical rollerball ablation and a subsequent endometrial sampling or hysterectomy. The average age of the patients

Hysteroscopic resection of submucous myoma: a result of 50 procedures at Ramathibodi Hospital.

We reported an initial result of the safety and efficacy of myomectomies performed between September 1994 and June 1997 by the hysteroscopic resectoscope in 50 patients at Ramathibodi Hospital. The indications for hysteroscopy and/or hysteroscopic myomectomy were menorrhagia in 23, metrorrhagia in 3, menometrorrhagia in 2, infertility with abnormal uterine bleeding in 12, abnormal uterine bleeding during hormonal replacement therapy (HRT) in 4, and suspected submucous myomas detected by ultrasonography and/or sonohysterography in 6 patients. The mean age of the 50 patients was 39.5 years with

Benefit of GnRH analogue pretreatment for hysteroscopic surgery in patients with bleeding disorders.

Hysteroscopic surgery is widely used for the treatment of patients suffering from menorrhagia. In different studies, pretreatment of the endometrium with GnRH analogues (GnRH-a) prior to endometrial ablation has been reported to increase the success rate, as well as to reduce the menstrual blood flow, accounting for a significantly higher postoperative amenorrhea rate (42% in pretreated patients vs. 24% in those receiving no pretreatment). The aim of pretreatment is not only to obtain a thin endometrium but also to reduce the size and vascularization of myomas being treated. In our study, GnRH

Beneficial effect of hysteroscopic metroplasty on the reproductive outcome in a 'T-shaped' uterus.

Eight women (aged 27-43) with reproductive dysfunction who were diagnosed by hysterosalpingogram and hysteroscopy as having a 'T-shaped' uterus were operated on using fiberoptic hysteroscopic guidance; the uterine side walls were incised until a normal uterine cavity was achieved. The women's gynecologic and obstetric records were compared before and after the operation. In all the 8 women the operation was without complications and resulted in a satisfactory uterine cavity. Before the operative procedure, the women had had 10 spontaneous abortions and 1 ectopic pregnancy. The postoperative pe

Hysteroscopic tubal catheterization and hydrotubation for treatment of infertile women with tubal obstruction.

We reported the efficacy of interval hysteroscopic tubal catherization and hydrotubation for three months in 54 infertile women with tubal obstruction (102 fallopian tubes). By comparison of HSGs before and after treatment, 86 of 102 tubes (84%) showed the improvement of tubal patency (P < 0.01). 56 tubes were patent (54.9%). This method is effective for partially patent tubes (87.5%), intramural tubal obstruction (62.5%), and the obstruction at the distal portion of the fallopian tube (13.3). One -12 month follow-up after treatment, 12 of 35 women whose tubes became completely patent achieved

Results of Hysteroscopic Method of Foreign Body Removal out of Uterus Cavity.

Recognition of foreign bodies in the uterine cavity is of great importance for successful treatment of secondary infertility: e.g. a piece of intrauterine contraceptive (after a removal failure), suture after cesarean section, or cylindrical and flat small bones after abortions. Sixty-nine foreign bodies were removed with a hysteroscope during 1992-93. Sixty-one of the foreign bodies were intrauterine devices. Twelve patients (19.7%) had had several unsuccessful removal attempts. The remaining eight women (11.9%) underwent fetal bone removals. Removal of ligatures in the area of scar on the ut

Value of intrauterine device insertion and estrogen administration after hysteroscopic metroplasty.

Hysteroscopic metroplasty seems the treatment of choice for septate uterus. Little information is available on the possible usefulness of postoperative intrauterine device (IUD) insertion and estrogen administration in preventing fusion of a freshly cut septum and intrauterine adhesion formation. A hysteroscopic incision in a uterine septum was made in 20 women. Postoperatively an IUD was inserted in ten of them and conjugated estrogens administered for 30 days with medroxyprogesterone acetate on days 26-30 (group I); the other ten were given no other therapeutic measures (group II). AAGL Practice Report: Practice Guidelines for the Management of Hysteroscopic Distending Media: (Replaces Hysteroscopic Fluid Monitoring Guidelines. J Am Assoc Gynecol Laparosc. 2000;7:167-168.).

The objective of this guideline is to provide clinicians with evidence-based information about commonly used and available hysteroscopic distending media to guide them in their performance of both diagnostic and operative hysteroscopy. While necessary for the performance of hysteroscopy and hysteroscopically-directed procedures, distending media, if absorbed systemically in sufficient amounts, can have associated adverse events, including life-threatening complications. Consequently, understanding the physical properties and the potential risks associated with the use of the various distending

Hysteroscopic fundal fixation of LNG-IUS in adenomyosis: a prospective feasibility and safety study.

BACKGROUND: Expulsion of the levonorgestrel-releasing intrauterine system (LNG-IUS) limits its effectiveness in adenomyosis management, particularly in patients with anatomical risk factors. This study aimed to assess the feasibility and short-term safety of a novel hysteroscopic fixation technique designed to improve device retention. METHODS: In this single-center prospective feasibility study, 52 patients with adenomyosis underwent placement of a modified LNG-IUS (integrated with a GyneFix anchor) using a customized delivery cannula under hysteroscopic guidance. Feasibility outcomes include

Hysteroscopic metroplasty for a complete uterine septum: step-by-step technique and surgical pearls.

Septate uterus is the most common congenital uterine anomaly. It is associated with increased risks of adverse obstetric outcomes. Hysteroscopic metroplasty is the standard minimally invasive treatment in selected patients, although its effect on reproductive outcomes remains controversial. We present a case of hysteroscopic metroplasty in a 31-year-old woman with a complete uterine septum extending to the internal cervical os (class U2b5) and a history of preterm delivery at 26 weeks gestation. The procedure was performed using an operative hysteroscope with a bipolar Versapoint spring electr
