Tolerance induced by low dose D-penicillamine in the brown Norway rat model of drug-induced autoimmunity is immune-mediated.

Most patients taking drugs associated with idiosyncratic drug reactions tolerate the drug and do not develop adverse reactions. Understandin … Both LPS and poly I:C reversed tolerance in a dose-dependent manner. These results demonstrate that immune tolerance to D-penicillamine autoimmunity can be induced by short-term low dose pretreatment.

Investigation of a mechanism for accelerated breakdown of immune tolerance to the primary biliary cirrhosis-associated autoantigen, pyruvate dehydrogenase complex.

Primary biliary cirrhosis (PBC) is an autoimmune liver disease characterized by autoreactive T- and B-cell responses to the highly conserved enzyme pyruvate dehydrogenase complex (PDC). In this study we have examined the breakdown of T-cell tolerance to self-PDC using a mouse model. Female SJL/J mice were sensitized intraperitoneally with foreign-PDC (bovine) and/or self-PDC (murine) in complete Freund's adjuvant, and serum, spleen, and liver tissue was taken 8 weeks later. Animals sensitized with foreign-PDC produced IgG antibodies that were reactive with both foreign and self-PDC, but spleni

The Balance Between Immune Activation and Tolerance in AQP4 Antibody-Positive Neuromyelitis Optica Spectrum Disorders and MOG Antibody-Associated Disease.

Aquaporin-4 (AQP4) antibody-positive neuromyelitis optica spectrum disorder (NMOSD) and myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD) are distinct autoimmune inflammatory conditions of the central nervous system. Defined autoantigens characterize NMOSD and MOGAD; however, the mechanisms driving immune activation, tissue injury, and breakdown of immune tolerance differ substantially between these conditions. Both conditions reflect a breakdown of immune tolerance mechanisms, permitting the persistence and activation of autoreactive T and B cells. However, NMOSD is char…

Isolated A1-domain F8 large deletions are associated with favorable immune tolerance induction outcomes in children with severe hemophilia A.

BACKGROUND: Large F8 deletions are generally associated with unfavorable immune tolerance induction (ITI) outcomes in children with severe hemophilia A. Whether intragenic deletion location further stratifies ITI outcomes remains unclear. OBJECTIVES: To evaluate the association between the location of F8 large deletion and ITI outcome in children with severe hemophilia A and to exploratorily assess residual F8 expression across patients with different deletion locations. METHODS: This single-center, retrospective cohort included 34 children with severe hemophilia A, F8 large deletions, inhib…

Internal peacekeepers and external mediators: A new model of peripheral immune tolerance involving regulatory T cells and mesenchymal stem cells.

The 2025 Nobel Prize in Physiology or Medicine honored the seminal discovery that regulatory T cells (Tregs) restrain immune responses and prevent autoimmunity through peripheral immune tolerance. However, to obtain a holistic view of peripheral immune tolerance, it is also necessary to consider the role of mesenchymal stem/stromal cells (MSCs) in this process. Therefore, I propose a two-tier model that incorporates both Tregs and MSCs, with Tregs acting within the immune system as an "internal checkpoint" to temper effector cell activity, and tissue-resident MSCs - or "master signaling cells"…

Tetrahedral Framework Nucleic Acids Re-establish Immune Tolerance and Restore Thyroid Function in Hashimoto's Thyroiditis via NOTCH1 Signaling Pathway Inhibition.

Hashimoto's thyroiditis (HT) is a chronic autoimmune disorder in which persistent immune dysregulation gradually impairs thyroid structure and endocrine function. Although levothyroxine replacement can correct hypothyroidism, it has little effect on the underlying autoimmune activity that drives the progression of the disease. Tetrahedral framework nucleic acids (tFNAs) are self-assembled DNA nanostructures that have demonstrated immunomodulatory capabilities in several inflammatory conditions. In this study, we investigated whether tFNAs could exert therapeutic effects in experimental models

The extracts of Astragalus membranaceus overcome tumor immune tolerance by inhibition of tumor programmed cell death protein ligand-1 expression.

A polysaccharide isolated from the radix of Astragalus membranaceus, called PG2, used in traditional Chinese medicine, with potential hemato … and-1 (PD-L1), which binds with programmed cell death protein-1(PD-1) in immune cells, thus causing immune tolerance within the tumor microenvironment. With decreased expression of PD-L1, increased immune response can be observed, which might be helpful when developing tumor immunotherapy. The antitumor therapeutic effect mediated by PG2 may associate with an infl …

Old Dog New Tricks: PLGA Microparticles as an Adjuvant for Insulin Peptide Fragment-Induced Immune Tolerance against Type 1 Diabetes.

Poly[lactic-co-(glycolic acid)] (PLGA) is arguably one of the most versatile synthetic copolymers used for biomedical applications. In vivo … Subsequent mechanistic investigation further revealed a facilitative role of PLGA microparticles in immune tolerance induction. In summary, our data demonstrate an adjuvant potential of PLGA microparticles in tolerance induction and immune remodulation for effective prevention of autoimmune diseases such as T1D.

Treatment of Surgical Brain Injury by Immune Tolerance Induced by Peripheral Intravenous Injection of Biotargeting Nanoparticles Loaded With Brain Antigens.

Once excessive, neurological disorders associated with inflammatory conditions will inevitably cause secondary inflammatory damage to brain tissue. Immunosuppressive therapy can reduce the inflammatory state, but resulting infections can expose the patient to greater risk. Using specific immune tolerance organs or tissues from the body, brain antigen immune tolerance treatment can create a minimal immune response to the brain antigens that does not excessively affect the body's immunity. However, commonly used immune tolerance treatment approaches, such as those involving the nasal, gastrointe…

The Japanese Immune Tolerance Induction (J-ITI) study in haemophilia patients with inhibitor: Outcomes and successful predictors of ITI treatment.

INTRODUCTION: Immune tolerance induction (ITI) was the primary therapeutic approach to eradicate inhibitors in haemophilia patients. Several large ITI registries had been reported, but successful predictors of ITI outcome are still debated. No reports are available on large ITI studies in non-caucasian countries. AIM: We designed a retrospective cohort study of ITI in Japanese haemophilia patients with inhibitor. METHODS: Retrospective data were collected from 155 haemophilia (H)A (140 severe-type) and 7 HB (7 severe-type) patients treated at 45 institutions. ITI outcome was centrally review…

Successful immune tolerance induction with low-dose coagulation factor VIII in a patient with hemophilia A from a developing country.

Inhibitor development is the most frequent and serious complication of the treatment in patients with hemophilia. Immune tolerance induction (ITI) is the only option of treatment for the eradication of factor VIII (FVIII) inhibitor. We would like to present our case with hemophilia whose FVIII inhibitor eradication was done by a low-dose ITI regimen. Our patient has been applied on-demand therapy until 8 years of age. Secondary prophylaxis was began because of having hemophilic arthropathy. A low titer of FVIII inhibitor (4.2 BU/ml) was detected in the fifth month of the prophylaxis. The peak …

Mobilization without immune depletion fails to restore immunological tolerance or preserve beta cell function in recent onset type 1 diabetes.

Granulocyte colony-stimulating factor (G-CSF) has been used to restore immune competence following chemoablative cancer therapy and to promote immunological tolerance in certain settings of autoimmunity. Therefore, we tested the potential of G-CSF to impact type 1 diabetes (T1D) progression in patients with recent-onset disease [n = 14; n = 7 (placebo)] and assessed safety, efficacy and mechanistic effects on the immune system. We hypothesized that pegylated G-CSF (6 mg administered subcutaneously every 2 weeks for 12 weeks) would promote regulatory T cell (Treg) mobilization to a degree capab

Morphofunctional Features of the Immune System Response to Sublethal Hypoxic Load in Hypoxia-Tolerant and Hypoxia-Susceptible Animals.

Background/Objectives: The most commonly used method for determining tolerance to oxygen deficiency is applying a sublethal hypoxic load (SHL) in a decompression chamber at an altitude of 11,500 m. The aim of this study was to identify the morphofunctional characteristics of the immune system's response to SHL in animals with different tolerances to oxygen deficiency. Methods: This study was conducted on male Wistar rats. Resistance to SHL was determined at a critical altitude (11,500 m) once in a decompression chamber. To study the features of the reaction to SHL, morphological and morphometr

Distinguishing True Immune Tolerant Hepatitis B Patients: Insights From Long-Term Clinical Outcomes.

The long-term prognosis of immune tolerant (IT) phase patients remains unclear. This study aimed to identify true IT-phase patients with favourable outcomes and distinguish grey-zone IT patients. We retrospectively analysed 1064 chronic hepatitis B (CHB) patients (516 clinically inclusive IT, 548 antiviral-treated immune active [AVT-IA]). Clinically inclusive IT-phase was defined as HBeAg-positive, no cirrhosis, HBV DNA ≥ 10^6 IU/mL, and ALT < 80 U/L. Favourable prognosis was defined as the absence of HCC development and IA progression during follow-up. IT-phase patients had significantly lowe

[Strategies for hepatitis B virus-infected patients in the immune-tolerant phase: complete therapy at the last mile].

Hepatitis B is mostly a chronic, progressive disease that, if not treated promptly and effectively, can slowly progress to cirrhosis, liver failure, or hepatocellular carcinoma. Therefore, antiviral therapy, i.e., a "complete therapy" strategy, should be started as long as the virus is positive. Immediate antiviral treatment is not recommended for infected patients who are only in the immune-tolerant phase, mainly because of the milder conditions and poor antiviral therapy efficacy, according to antiviral indications in China's Guidelines for the Prevention and Treatment of Chronic Hepatitis B

Hair follicle stem cells and the collapse of self-tolerance in alopecia: the interplay of barrier function, the microbiome, and immunity.

The hair follicle is thought to be a site of ‘immune privilege,’ or relative protection from autoimmune and inflammatory responses. In particular, the long-lived, quiescent hair follicle stem cells (HFSCs) are strikingly resistant to cytotoxic immune effectors such as T cells and natural killer (NK) cells, even in systems where these cells are artificially directed to attack HFSCs. Nonetheless, many forms of alopecia are associated with immune-mediated damage of the hair follicle epithelium, suggesting a failure of immune privilege. What precipitates the collapse of HFSC immune privilege in al

Prediction of the chance of successful immune tolerance induction in persons with severe hemophilia A and inhibitors: a clinical prediction model.

BACKGROUND: Inhibitor eradication to restore factor (F)VIII efficacy is the treatment goal for persons with severe hemophilia A (HA) and inhibitors. Immune tolerance induction (ITI) is demanding and successful in about 70% of people. Until now, it has remained difficult to quantify the probability of ITI success or failure, complicating the decision to initiate or not initiate ITI. Estimating the individual chance of ITI success allows clinicians, patients, and their families to support shared decision-making. OBJECTIVES: We aimed to identify clinical predictors of ITI success and to develop …
