Systemic and local predictors of medication-related osteonecrosis of the jaw in patients receiving antiresorptive therapy: The Shizuoka Kokuho Database study.

INTRODUCTION: Medication-related osteonecrosis of the jaw (MRONJ) is a rare but serious adverse effect of antiresorptive therapy. We aimed to identify dose-specific predictors for MRONJ risk stratification. METHODS: We conducted a retrospective cohort study using the Shizuoka Kokuho Database, an insurance claims database. Patients ≥40 years with osteoporosis or cancer who initiated bisphosphonate or denosumab between April 2015 and September 2022 were stratified into low- and high-dose groups. The outcome was MRONJ. Candidate predictors were defined using the information available before init

Association of Oral Health and Periodontal Disease With Oesophageal Cancer: A Nationwide Population-Based Study.

AIM: Oral dysbiosis and periodontal inflammation might play a role in oesophageal carcinogenesis via the translocation of periodontal pathogens. We aimed to assess whether poor oral health is associated with oesophageal cancer. METHODS: We conducted a population-based case-control study using the Korean National Health Insurance Service database to evaluate poor oral health as the exposure and oesophageal cancer as the outcome. Two study designs were employed: (1) oral health analysis using screening data, and (2) periodontal disease analysis using claims data. Oesophageal cancer cases were m

The Addition of Chinese Herbal Medicines Is Effective as a Prophylactic Treatment Against Dental Diseases for Sjögren's Syndrome Patients: Insight from Real-World Database.

Background and Objectives: Sjögren's syndrome (SS) is a chronic autoimmune disease that affects the salivary glands and increases the risk of developing dental diseases (DDs). Chinese herbal medicines (CHMs) represent a promising alternative strategy for SS treatment; however, the association between CHMs and DD risk has not been confirmed. In this retrospective, cohort-based, nested case-control study, we explored whether or not combining CHMs with routine treatments for SS can reduce the chance of DDs. Materials and Methods: In the beginning, we recruited subjects aged 20-80 years with newly

Intensive oral prophylaxis does not alter the tongue microbiome in young patients with chronic kidney disease: longitudinal, randomized, controlled study.

INTRODUCTION: Gingivitis is a common intraoral disease in patients with chronic kidney disease (CKD), which poses a particular interdisciplinary challenge. We aimed to determine the influence of an intensive oral prophylaxis program (OPP) compared to standard prevention measures on the tongue microbiome of young patients with CKD. METHODS: Thirty patients with CKD (mean age 14.2 ± 5.2 years) and generalized gingivitis were included. The effects of the intensive OPP were compared with standard prophylaxis according to statutory health insurance (treatment as usual, TAU) as a control. Tongue sw

Risk of dementia according to the severity of chronic periodontitis in Korea: a nationwide retrospective cohort study.

OBJECTIVES: We investigated the risk of dementia in older adults with chronic periodontitis according to the severity of periodontitis. METHODS: Data on patients with chronic periodontitis were extracted from the National Health Insurance Service-Senior cohort database from 2002 to 2014. Among 52,728 subjects eligible for inclusion, 11,953 subjects had newly diagnosed mild chronic periodontitis (MCP), and 40,775 subjects had newly diagnosed severe chronic periodontitis (SCP). Two 1:1 propensity score matched cohorts were created with 8,624 patients each in the MCP and SCP groups. To analyze t

Changes in prevalence of precancerous oral submucous fibrosis from 1996 to 2013 in Taiwan: A nationwide population-based retrospective study.

BACKGROUND/PURPOSE: Oral submucous fibrosis (OSF) has been regarded as a precancerous condition. Research examining the prevalence of OSF could be the first step in preventing or reducing malignant transformation. In this study, we probed a nationwide registered database to assess the prevalence, gender distribution, age, income, and urbanization status of OSF patients in Taiwan. METHODS: A retrospective study was conducted to analyze the registered database compiled by the National Health Insurance provided by the Ministry of Health and Welfare, Taiwan. A field survey of the partially edentate elderly: Investigation of factors related to the usage rate of removable partial dentures.

Although the shortened dental arch (SDA) concept has been known to all over the world, acceptance of the SDA concept as an oral health standard can be questionable from the patients' point of view, even if it is biologically reasonable. Furthermore, because the health insurance system covers removable partial dentures (RPDs) for all citizens in Japan, SDA patients seem to prefer to receive prosthetic treatment to replace the missing teeth. However, there were few field surveys to investigate the usage rate of RPDs in Japan. The purpose of this study was to determine the usage rate of RPDs in o

Self-assessed masticatory ability and hospitalisation costs among the elderly living independently.

The escalating medical costs are a social problem in many countries. Masticatory ability is thought to be related to the general health conditions. The purpose of this study was to show relationships between self-assessed masticatory ability and medical costs among the elderly living independently in community. Data on background factors and self-assessed masticatory ability were collected from 702 Japanese elderly persons by questionnaires. An intra-oral examination was performed to examine the number of remaining teeth. Self-assessed masticatory ability was classified into one of three categ

Exploring barriers and facilitators in primary oral health services: step towards Universal Health Coverage.

INTRODUCTION: Despite the Alma-Ata Declaration's emphasis on primary health care (PHC) to achieve universal health coverage (UHC), oral diseases remain prevalent and costly. In Iran, health reforms have expanded PHC, yet oral health services lag. This study explored barriers and facilitators of primary oral health service delivery in Tehran. METHODS: This qualitative study employed semi-structured, face-to-face interviews with dentists, heads of comprehensive health centers (CHCs), healthcare providers and visitors to the health centers in Tehran from November 2021 to January 2022.
