Guilt and Shame in Patients with Obsessive-Compulsive Disorders.

INTRODUCTION: Obsessive-compulsive disorder (OCD) is a tremendous psychiatric illness with a variety of severe symptoms. Feelings of shame and guilt are universal social emotions that fundamentally shape the way people interact with each other. Mental illness is therefore often related to pronounced feelings of shame and guilt in a maladaptive form. METHODS: A total of 62 participants (38 women and 24 men) were clinically and psychometrically investigated. RESULTS: The OCD patients (n = 31) showed a maladaptive guilt and shame profile, characterized by increased interpersonal feelings of gui

What should be done with antisocial personality disorder in the new edition of the Diagnostic and Statistical Manual of Mental Disorders (DSM-V)?

Antisocial personality disorder, psychopathy, dissocial personality disorder and sociopathy are constructs that have generally been used to predict recidivism and dangerousness, alongside being used to exclude patients from treatment services. However, 'antisocial personality disorder' has recently begun to emerge as a treatment diagnosis, a development reflected within cognitive behaviour therapy and mentalisation-based psychotherapy. Many of the behaviour characteristics of antisocial personality disorder are, at the same time, being targeted by interventions at criminal justice settings. A

Training Communication Skills and Mentalization in Group Schema Therapy for Patients with Borderline Personality Disorder.

BACKGROUND: This article introduces a structured method for communication skills training within group schema therapy (GST) specifically tailored for individuals with borderline personality disorder (BPD). Patients with BPD often face relational difficulties due to intense emotional responses and maladaptive schema modes, thus creating a need for targeted communication training to promote healthier interpersonal relationships. METHODS: This training approach was developed based on interviews with six experienced schema therapists specialising in GST for BPD. The therapists shared practical in

A psychogeriatric nursing home resocialization program.

The institutional atmosphere and programming of a nursing home can often engender a downward spiral of increasing dependence and learned helplessness in its elderly residents. Elderly patients with chronic medical complaints combined with psychiatric and management problems are especially susceptible. An intermediate-and skilled-care nursing home began a special program in 1974 to deal with these patients. The program features problem-oriented rather than diagnosis-oriented recordkeeping and the use of a multidisciplinary therapeutic team to treat problem residents. Over the first four years o

Exploring the Relationship Between Prolonged Grief and Borderline Personality Symptoms.

BACKGROUND: Prolonged grief disorder (PGD) is a new diagnosis that is often mistaken for other psychopathologies. We report a case of PGD in a patient who screened positive for personality disorder. Both PGD and personality disorder symptoms were much improved after a course of prolonged grief disorder therapy. CASE PRESENTATION: A bereaved woman in her mid-20s sought treatment for depression 9 months after the death of her father. Her depressive symptoms responded well to antidepressant medication and a brief course of interpersonal psychotherapy. However, prolonged grief symptoms persisted …

Relational and Growth Outcomes Following Couples Therapy With MDMA for PTSD.

Healing from trauma occurs in a relational context, and the impacts of traumatic experiences that result in post-traumatic stress disorder (PTSD) go beyond the diagnosis itself. To fully understand a treatment for PTSD, understanding its impact on interpersonal, relational, and growth outcomes yields a more fulsome picture of the effects of the treatment. The current paper examines these secondary outcomes of a pilot trial of Cognitive Behavioral Conjoint Therapy (CBCT) for PTSD with MDMA. Six romantic dyads, where one partner had PTSD, undertook a course of treatment combining CBCT for PTSD w

Effects of coping-oriented couples therapy on depression: a randomized clinical trial.

The aim of this study was to evaluate the effectiveness of treating depression with coping-oriented couples therapy (COCT) as compared with cognitive-behavioral therapy (CBT; A. T. Beck, C. Ward, & M. Mendelson, 1961) and interpersonal psychotherapy (IPT; M. M. Weissman, J. C. Markowitz, & G. L. Klerman, 2000). Sixty couples, including 1 clinically depressed partner, completed pre- and posttest questionnaires as well as follow-up assessments at 6-month intervals over the subsequent 1.5 years. Effects of the 3 treatments on depressive symptomatology assessed by the Beck Depression Inventory (A.…

Psychological treatment of bulimia nervosa and binge eating disorder.

The field of eating disorders has grown rapidly, amassing an impressive body of treatment research in the past 20 years. In particular, rese … nt for BED is effective in reducing binge eating, and it may have the added benefit of weight loss. Interpersonal psychotherapy (IPT), the combination of CBT and medication, and self-help manuals are promising treatment alternatives for both BN and BED. Future treatment trials should include longer-term followup periods and more consistent definitions of successful treatment …

Comparison of brief group therapies for depressed cancer patients receiving radiation treatment.

Although many studies have documented patterns of emotional distress in persons undergoing radiation treatment for cancer, there have been few controlled evaluations of counseling or psychotherapy outcomes with these persons. In this research, the effects of cognitive-behavioral and socially supportive group therapy were evaluated. A total of 72 depressed cancer patients were randomly assigned to one of three conditions--cognitive-behavioral treatment, social support, or a no-treatment control condition. Before and after intervention and at 6-month followup, study participants were individuall

Wired for Intensity: The Neuropsychological Dynamics of Borderline Personality Disorders-An Integrative Review.

Background: Borderline personality disorder (BPD) is a severe psychiatric condition characterised by emotional instability, impulsivity, interpersonal dysfunction, and self-injurious behaviours. Despite growing clinical interest, the neuropsychological mechanisms underlying these symptoms are still not fully understood. This review aims to summarise findings from neuroimaging, psychophysiological, and neurodevelopmental studies in order to clarify the neurobiological and physiological basis of BPD, with a particular focus on emotional dysregulation and implications for the treatment of adolesc

Is reflective functioning associated with clinical symptoms and long-term course in patients with personality disorders?

OBJECTIVE: Mentalization is the capacity to understand behavior as the expression of various mental states and is assumed to be important in a range of psychopathologies, especially personality disorders (PDs). The first aim of the present study was to investigate the relationship between mentalization capacity, operationalized as reflective functioning (RF), and clinical manifestations before entering study treatment. The second aim was to investigate the relationship between baseline RF and long-term clinical outcome both independent of treatment (predictor analyses) and dependent on treatme

Trauma-informed care for adult survivors of developmental trauma with psychotic and dissociative symptoms: a systematic review of intervention studies.

Developmental trauma is associated with an increased risk of psychosis and predicts poor prognosis. Despite this association, little is known about which treatments work best for survivors of developmental trauma with psychosis. We sought to do the first review, to our knowledge, to investigate treatments for people with psychotic and dissociative symptoms who have a history of developmental trauma. We searched MEDLINE, PsychINFO, and Google Scholar for studies reporting psychological and pharmacological treatments of psychotic or dissociative symptoms in adult survivors of developmental traum

Interpersonal behavior and therapeutic progress: therapists and clients rate themselves and each other.

To measure the extent to which progress in psychotherapy can be predicted from the interpersonal styles of therapists and clients, 42 outpatients and their 11 therapists rated themselves and each other on Benjamin's Structural Analysis of Social Behavior (SASB) scales. In addition, therapists rated amount of client change and therapy success-failure, based on the outcome section of the Rogers and Dymond (1954) scale, and clients used the Strupp, Fox, and Lessler (1969) questionnaire to evaluate progress-to-date. Clients also completed the Hopkins Symptom Checklist (HSCL) at the start of therap

Working with underrepresented groups: lessons from the SCHEMA trial.

There is a growing focus on ensuring research is accessible and inclusive to individuals traditionally not represented. To be truly inclusive, the broader context needs to be explored, as barriers might not only be linked to population characteristics but also to the complexity of their environment, and limited research opportunity within certain healthcare professions. The SCHEMA trial is a randomised controlled trial evaluating whether interpersonal art psychotherapy is effective at reducing aggressive behaviour in individuals with learning disability or borderline intellectual function in s

An Integrated Developmental Approach to Personality Disorders in Adolescence: Expanding Kernberg's Object Relations Theory.

This article is a tribute to Dr. Otto F. Kernberg and his contribution to the theoretical understanding of personality pathology in adolescence. In his object relations theory of the structure of personality, an integrated identity is considered central to healthy personality functioning and is contrasted with identity diffusion, which is posited to underlie the maladaptive interpersonal functioning of patients with severe personality disorders. His work provided an early theoretical foundation for the understanding and treatment of personality disorders in adults, but a need remained for a de
