Differences in voice-hearing experiences of people with psychosis in the U.S.A., India and Ghana: interview-based study.

BACKGROUND: We still know little about whether and how the auditory hallucinations associated with serious psychotic disorder shift across cultural boundaries. AIMS: To compare auditory hallucinations across three different cultures, by means of an interview-based study. METHOD: An anthropologist and several psychiatrists interviewed participants from the USA, India and Ghana, each sample comprising 20 persons who heard voices and met the inclusion criteria of schizophrenia, about their experience of voices. RESULTS: Participants in the U.S.A. were more likely to use diagnostic labels and t

The tripartite model of fear in children with specific phobias: assessing concordance and discordance using the behavioral approach test.

Lang's tripartite model posits that three main components characterize a fear response: physiological arousal, cognitive (subjective) distress, and behavioral avoidance. These components may occur in tandem with one another (concordance) or they may vary independently (discordance). The behavioral approach test (BAT) has been used to simultaneously examine the three components of the fear response. In the present study, 73 clinic-referred children and adolescents with a specific phobia participated in a phobia-specific BAT. Results revealed an overall pattern of concordance: correlation analys

Affective lability in bipolar disorder and borderline personality disorder.

BACKGROUND: The boundaries between the affective instability in bipolar disorder and borderline personality disorder have not been clearly defined. Using self-report measures, previous research has suggested that the affective lability of patients with bipolar disorder and borderline personality disorder may have different characteristics. METHODS: We assessed the mood states of 29 subjects meeting Revised Diagnostic Interview for Borderlines and Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition (DSM-IV) criteria for BPD and 25 subjects meeting DSM-IV criteria for bipolar

Assessing Axis I symptomatology on the SADS-C in two correctional samples: the validation of subscales and a screen for malingered presentations.

The Schedule of Affective Disorders and Schizophrenia-Change Version (SADS-C; Spitzer & Endicott, 1978b) is a brief, highly reliable structured interview with clinical applications to diverse populations. This investigation involved reanalyses of data from 2 earlier studies (Rogers, Grandjean, Tillbrook, Vitacco, & Sewell, 2001; Ustad, Rogers, & Salekin, 1998). Focusing on 2 clinical samples from a metropolitan jail, we investigated its subscales via exploratory and confirmatory factor analysis. A good model fit was found (comparative fit index =.92; robust comparative fit index =.94) for 4 su

Psychological features in panic disorder: a comparison with major depression.

OBJECTIVE: We aim to evaluate the psychodynamic model for panic disorder (PD) formulated by Shear et al. (1993), comparing PD patients and major depression (MD) patients. METHOD: We evaluated these parameters in open interviews in 10 PD patients and 10 patients with MD (DSM-IV). The data were recorded on videotape and were examined by 5 diagnostic blind appraisers. RESULTS: The data allowed a comparative analysis that underscores the existence of a psychological model for PD vs MD: 1) the protracted symbiotic phase of development and the existence of problems with separation in PD patients;

Influence of patient caregivers on course of patient illness: "expressed emotion" and alternative measures.

The influence of the quality of care that patients receive during the course of their illness has been the subject of special scrutiny in the area of mental health. The concept of "expressed emotion" (EE) evolved in an effort to understand the impact of family and social environment on the vulnerability to relapse of schizophrenic patients. A semistructured interview, the Camberwell Family Interview, was developed to assess expressed emotion. This article examines the historical context, generalizability, methodological strengths and limitations of the construct of EE and the Camberwell Family

The measurement of adolescent adaptive processes in the clinical interview.

A study is described measuring the clinical usefulness of Adolescent Adaptive Process Scales. These processes are measured in three groups of adolescents in which psychopathology is also measured using a standardized psychiatric interview. Inter-rater reliability is calculated between two raters and mean score comparisons are made between the groups using the t-test method. In the study a higher inter-rater reliability and some preliminary evidence of construct validity are found for these scales. The findings indicate that these scales could constitute a useful clinical tool in psychotherapy,

Diagnosing borderline patients with a semistructured interview.

As a part of an ongoing effort to provide operational criteria for a diagnosis of borderline personality disorder, the Diagnostic Interview for Borderlines (DIB) has been developed. The DIB is an hour-long, semistructured interview that evaluates five areas of borderline pathologic features--social adaptation, impulse/action patterns. affects, psychotic symptoms, and interpersonal relations. The DIBs were administered to 70 hospitalized patients and compared with clinical diagnoses made at the time of discharge. The DIB discriminates borderline patients from others, especially from schizophren

[Psychological aspects of infertile couples in China].

From March 1991 to February 1992, a psychological interview survey was performed among 652 women and 425 of their husbands who came to the Female Infertility Clinic in PUMC Hospital to seek consultation and treatment for their infertility. The results showed that more than 80 percent of the couples had been tolerating various psychological stresses caused by infertility. Disapproval was the most common feeling. The couples' emotion was influenced by each other. Emotional scores of husbands and wives were significantly positive correlated (P < 0.001). However, men are more capable to cope with

Cognitive and behavioral treatments for anxiety and depression in a patient with an implantable cardioverter defibrillator (ICD): a case report and clinical discussion.

Implantable cardioverter-defibrillators (ICDs) are the treatment of choice for patients at risk for potentially life threatening arrhythmias. The associated stress of living with an implanted device and receiving ICD shock therapy has been noted to exert a psychological toll on the individual patient and family. Anxious and depressive symptomatology is frequently reported by these patients, thus creating a demand for tailored psychological interventions for this population. The current case report describes the components, delivery, and effectiveness of a targeted treatment approach for anxiet

The psychological and social aspects of orthognathic treatment.

Cooperative management of dentofacial abnormalities by orthodontists and oral and maxillofacial surgeons has experienced tremendous growth in recent years. Frequently these patients have suffered considerable emotional trauma from ridicule of their deformity. This history of emotional abuse may influence the patient's attitude and expectations regarding corrective treatment and even lead to dissatisfaction in spite of technical success. Review of the literature and interviews with patients previously treated for dentofacial abnormalities revealed a very high rate of satisfaction. This high deg
