Peutz-Jeghers syndrome: case report.

A case of Peutz-Jeghers syndrome (PJS) in an African girl is presented to document the clinical presentation, management and follow up of this condition. The patient who presented with black buccal mucosal hyperpigmentation and clinical features of jejuno-jejunal intussusception was successfully managed by operative reduction of the intussusception and polypectomy. She is being followed up for evidence of malignant transformation in associated intestinal polyps and development of extra-intestinal malignancies at other sites. The management of PJS in light of recent trends is discussed, especia…

Familial pediatric Peutz-Jeghers syndrome with recurrent intussusception: case report and literature review.

BACKGROUND: Peutz-Jeghers syndrome (PJS) is a rare autosomal dominant disorder characterized by mucocutaneous pigmentation and gastrointestinal hamartomatous polyposis, predisposing affected individuals to recurrent small bowel intussusception and increased cancer risk. CASE PRESENTATION: We report two siblings with genetically confirmed PJS who presented with intestinal obstruction due to small bowel intussusception. The first, a 13-year-old girl, had necrotic ileal intussusception caused by a large polyp requiring segmental resection. Her younger brother had jejuno-jejunal intussusception s

Colonic polyps: a rare clinical manifestation of schistosomiasis (a case report).

Schistosomiasis is caused by parasitic blood flukes. It is one of the neglected tropical diseases (NTDs) and has variable manifestations depending on the species involved. Eggs and not adult worms are mostly responsible for the resultant pathologies commonly involving the small intestines. Colon polyps resulting from schistosomiasis represent a rare entity despite the endemicity of Schistosoma mansoni in the tropical regions. Praziquantel is the mainstay of treatment. Case presentation is of a 28 year old male with complaints of bloating, mild abdominal pain and loose stools over two weeks. On

Beyond diagnosis: the emerging role of fecal calprotectin in longitudinal management of pediatric juvenile polyps.

Fecal calprotectin (FCP) is an established noninvasive marker of intestinal inflammation, but its clinical relevance in children with juvenile polyps extends beyond the initial diagnostic evaluation. Available studies consistently show that FCP is elevated in most affected children and decreases markedly after complete polypectomy. Some data also suggest an association with polyp size, although this finding has not been reproduced in all cohorts. These observations suggest that FCP reflects lesion-associated inflammatory activity and the response to its removal. More limited longitudinal evide

Helicobacter pylori infection and the development of gastric cancer.

BACKGROUND: Although many studies have found an association between Helicobacter pylori infection and the development of gastric cancer, many aspects of this relation remain uncertain. METHODS: We prospectively studied 1526 Japanese patients who had duodenal ulcers, gastric ulcers, gastric hyperplasia, or nonulcer dyspepsia at the time of enrollment; 1246 had H. pylori infection and 280 did not. The mean follow-up was 7.8 years (range, 1.0 to 10.6). Patients underwent endoscopy with biopsy at enrollment and then between one and three years after enrollment. H. pylori infection was assessed by

Management of Peutz-Jeghers Syndrome in Children and Adolescents: A Position Paper From the ESPGHAN Polyposis Working Group.

Peutz-Jeghers syndrome (PJS) is a well-described inherited syndrome, characterized by the development of gastrointestinal polyps, and characteristic mucocutaneous freckling. Development of small bowel intestinal polyps may lead to intussusception in children may require emergency laparotomy with potential loss of bowel. Gastrointestinal polyps may lead to bleeding and anemia. This European Society for Paediatric Gastroenterology Hepatology and Nutrition position paper provides a guide for diagnosis, assessment, and management of PJS in children and adolescents and guidance on avoiding complica…

Clinicopathological features of duodenal bulb biopsies and their relationship with upper gastrointestinal diseases.

AIM: To assess the prevalence of the lesions in duodenal bulb mucosa and the relationship between duodenal lesions and upper gastrointestinal diseases, including helicobacter pylori infection. METHODS: Clinical, endoscopic and pathological data of the cases with duodenal bulb and gastric mucosal biopsy from January 2005 to May 2017 were analyzed retrospectively. RESULTS: A total of 3540 patients were enrolled. The biopsy from protuberant lesions with endoscopic morphology are mostly duodenal gastric heterotopia or adenoma. The biopsy from duodenal ulcers are often observed in inflammatory ch

Concordance Between Pan-Intestinal Capsule Endoscopy and Colonoscopy for the Detection and Characterization of Colonic Lesions: Post Hoc Analysis on the Impact of Colonic Transit Time.

INTRODUCTION: Pan-intestinal capsule endoscopy (PCE) is increasingly used as a minimally invasive alternative to colonoscopy in the evaluation of mid- and lower gastrointestinal bleeding (MLGIB). However, the impact of colonic transit time (CTT) on diagnostic performance remains insufficiently characterized. This study aims to assess the concordance between PCE and colonoscopy for colorectal lesions and to determine whether CTT influences diagnostic accuracy. METHODS: This post hoc analysis of a prospective single-center trial included patients with suspected MLGIB who underwent PCE followed

The pathogenesis of inflammatory polyps.

Eighty-six consecutive colonic resection specimens for inflammatory bowel disease were studied to determine the modes of inflammatory polyp formation. The two major groups of inflammatory polyps were polypoid mucosal tags due to undermining ulceration and mature inflammatory polyps composed of mucosa, muscularis mucosae, and a submucosal core. Mature inflammatory polyps were derived from polypoid mucosal tags after regeneration and the adjacent mucosa showed regenerative changes and submucosal scarring. The study confirms that ulceration which undermines the muscularis mucosae is the major pre

[Problems in the differential diagnosis of intestinal diseases in patients who have traveled in the tropics].

Taking into consideration the increasing intercontinental tourist traffic we are in an increasing degree confronted with diseases which above all or exclusively acquired in warm countries. Besides the diseases which occur in our country they must find differential diagnostic consideration in disturbances of the health. On account of their frequency gastrointestinal disturbances they are essential for these travellers. On the basis of the following instances of diseases problems of the differential diagnosis and the evaluation of findings are shortly demonstrated: colon amoeboma; colon carcinom

Successful treatment for Cronkhite-Canada syndrome with endoscopic mucosal resection and salazosulfapyridine.

A 79-year-old woman was referred to our hospital because numerous polyps were found in her stomach and large intestine at an ambulatory clinic. Although there were no characteristic symptoms or signs of Cronkhite-Canada syndrome (CCS), endoscopic and pathological findings indicated CCS. Moreover, colonoscopy showed two polypoid lesions (Is type), which appeared neoplastic by magnifying observation with image-enhanced endoscopy (IEE), in the ascending colon. Histologically, the resected specimens revealed tubular adenomas arising in the CCS inflammatory polyps. Remarkable remission of the polyp

A child with colo-colonic intussusception due to a large colonic polyp: Case report and literature review.

Colo-colonic intussusception (CI) due to a colonic polyp is a rarely reported cause of intestinal obstruction in school-aged children. Hydrostatic reduction (HR) and endoscopic polypectomy are minimally invasive and technically feasible for treating CI. We report a case of CI and review the literature, focusing on the diagnosis and treatment.

Mutations of PTEN in patients with Bannayan-Riley-Ruvalcaba phenotype.

We report three new mutations in PTEN, the gene responsible for Cowden disease in five patients with Bannayan-Riley-Ruvalcaba syndrome from three unrelated families. This finding confirms that Cowden disease, a dominant cancer predisposing syndrome, and Bannayan-Riley-Ruvalcaba syndrome, which includes macrocephaly, multiple lipomas, intestinal hamartomatous polyps, vascular malformations, and pigmented macules of the penis, are allelic disorders at the PTEN locus on chromosome 10q.

Needle knife-assisted endoscopic polypectomy for a large inflammatory fibroid colon polyp by making its stalk into an omega shape using an endoloop.

Colonic inflammatory fibroid polyp (IFP) is an uncommon benign polypoid lesion, which is composed of fibroblasts, numerous small vessels and edematous connective tissue with marked eosinophilic inflammatory cell infiltration. This condition is frequently detected in the stomach and small intestine, but uncommon in the colon. Although IFP is a benign lesion, surgical resections are performed in most colonic cases because the polyps are usually too large to resect endoscopically. Only three patients underwent endoscopic polypectomy in our literature reviews. Here, we present a case of IFP in the
