[Anoxic spell in an adult case for intracardiac repair].

We report a 51-year-old male patient with tetralogy of Fallot who had an anoxic spell during pre-bypass period before intracardiac repair. Immediately after starting sternotomy, SpO2 decreased suddenly from 83% to 30% with PaO2 of 26 mmHg. Heart rate showed an increase to 120-140 bpm with little change in blood pressure. We considered that the causes of this spell were 1) light level of anesthesia, 2) hypoxemia and 3) lack of intravascular blood volume. This case warns us that we should take care to avoid an anoxic spell in an adult case for intracardiac repair.

Mitigating and addressing complications in equine laparoscopy.

The purpose of this article was to report a condensed overview of current strategies for mitigating and managing complications in equine laparoscopic surgery. Minimally invasive surgical techniques have become standard in equine surgery for a variety of diagnostic and therapeutic procedures, with purported advantages including reduced postoperative pain, expedited recovery, fewer incisional complications, and enhanced visual imagery for the surgeon compared to traditional open approaches. Despite these benefits, complications can arise, and the literature demonstrates variability in definition

Prevalence of complications of buccal fat removal: A systematic review and meta-analysis.

Removal of the buccal fat pad can be considered safe as long as there is a detailed analysis of anatomical landmarks. The objective of this study was to estimate the prevalence of intra- and postoperative complications resulting from buccal fat pad removal through a systematic review. The search strategy involved observational and/or interventional studies in humans that included at least one case of buccal fat pad removal with a description of the surgery, postoperative progress, and complications. Methodological quality assessment was performed using the JBI checklist. Study heterogeneity wa

Intraoperative hypotension and postoperative acute kidney injury: A systematic review.

BACKGROUND: There is no consensus regarding safe intraoperative blood pressure thresholds that protect against postoperative acute kidney injury (AKI). This review aims to examine the existing literature to delineate safe intraoperative hypotension (IOH) parameters to prevent postoperative AKI. METHODS: PubMed, Cochrane Central, and Web of Science were systematically searched for articles published between 2015 and 2022 relating the effects of IOH on postoperative AKI. RESULTS: Our search yielded 19 articles. IOH risk thresholds ranged from <50 to <75 mmHg for mean arterial pressure (MAP) a

A new 6 o'clock traction suture technique for glaucoma filtration surgery.

PURPOSE: Recent studies show that using a traction suture through the superior rectus muscle in filtering glaucoma surgery is associated with a lower success rate compared with a corneal traction suture. A 12 o'clock corneal traction suture combined with a fornix-based approach may distort the architecture of the scleral flap. METHODS: In a new technique, a corneal 6 o'clock traction suture is placed near the limbus, thus avoiding any disturbance at the site of filtering surgery. The suture is passed underneath a notched solid-bladed lid speculum that pushes the suture deep into the inferior

[Possible complications of transpleural neurotomy].

The occurrence of complications in course of thoracoscopic vegetative denervation has been found low in the author's analysis of his 2679 cases. There arose several small lung lesions with consecutive pneumothorax or tension pneumothorax, furthermore some events of emphysema of the skin, one bleeding of an intercostal artery, which was stopped under thoracotomy, one division of the thoracic duct, one thrombosis of the anterior spinal artery, one cardiac failure. On the other hand there could be avoided air embolism, Horner's syndrome, lesions of recurrent and phrenic nerve, damage on trachea,

Diagnosis and treatment of gallstone ileus. Report of 83 cases.

83 patients with gallstone ileus treated at 6 hospitals in Sweden during 1960-1979 are presented. Overall mortality was 26.5%. The high age (mean 75 years) and poor general condition of many patients seem to be the main reason for the high mortality rate. 76 patients (92%) were laparotomized and in 63 (76%) the operative procedure was limited to enterolihotomy. Three of these patients had later a second attack of gallstone ileus; other late complications of the gallbladder disease were rare and less serious. In three patients a cholecystectomy was added to enterolithotomy. The postoperative pe

Perioperative Quality Initiative consensus statement on intraoperative blood pressure, risk and outcomes for elective surgery.

BACKGROUND: Intraoperative mortality is now rare, but death within 30 days of surgery remains surprisingly common. Perioperative myocardial infarction is associated with a remarkably high mortality. There are strong associations between hypotension and myocardial injury, myocardial infarction, renal injury, and death. Perioperative arterial blood pressure management was thus the basis of a Perioperative Quality Initiative consensus-building conference held in London in July 2017. METHODS: The meeting featured a modified Delphi process in which groups addressed various aspects of perioperative

[Results of laryngeal nerve monitoring during thyroid operations--Studies and value for clinical practice].

Injuries of the laryngeal nerve still remain a source of morbidity for patients undergoing surgery for the thyroid gland. The incidence of permanent damage of the nerve varies from 0.5 % to 2.7 % depending on the series, but for special thyroid diseases like thyroid carcinoma or goiter recurrence the damage of the recurrent laryngeal nerve could be up to 25 %. For that reason several methods of monitoring the movement of the nerve while stimulation have been proposed. Such as inserting a needle into the vocal muscles or working with a surface laryngeal electrode that is attached above the cuff

Effect of antithrombic therapy on bleeding complications in patients receiving emergency cholecystectomy for acute cholecystitis.

BACKGROUND: The risk of developing hemorrhagic complications during or after emergency cholecystectomy in patients with antithrombic therapy (ATT) remains uncertain. In this study, we evaluate outcomes in patients with ATT undergoing emergency cholecystectomy and assess the relevance between ATT and perioperative complications including bleeding complications. METHODS: We retrospectively evaluated 296 patients who were diagnosed as acute cholecystitis and underwent emergency cholecystectomy between 2005 and 2017. One hundred and thirty-three of them (45%) were on ATT. The primary outcome meas

The Association Between Fluid Management and Intraoperative Blood Pressure and Patients' Outcome After Complex Spine Surgeries.

BACKGROUND: Both intraoperative hypotension and excessive fluid administration can lead to detrimental perioperative complications. However, how much fluid is considered excessive and how is intraoperative hypotension related to major postoperative complications? METHODS: We conducted a single-center retrospective cohort study in 6243 patients undergoing complex spine surgery at the Cleveland Clinic Foundation between 2012 and 2022 and studied the relationship between intraoperative net fluid administration and intraoperative hypotension with major postoperative complications. The primary out

Prevention and management of complications in sphenoidotomy.

Endoscopic sphenoidotomy is a common surgical procedure that often accompanies routine sinus surgery. Safe completion of a sphenoidotomy depends on a thorough understanding of the surrounding anatomy, reviewing preoperative imaging, and maintaining intraoperative orientation. Intraoperative complications include local hemorrhage, catastrophic hemorrhage caused by internal carotid injury, optic nerve injury, and CSF leak. Postoperative complications tend to be less severe and include postoperative stenosis and mucocele formation. Regarding surgery of the sphenoid sinuses, the best management of…
