Obstructive jaundice: an unusual initial manifestation of intra-abdominal non-Hodgkin lymphoma in a child.

Obstructive jaundice is an unusual manifestation of non-Hodgkin lymphomas in children. Although surgical drainage is one of the initial treatment choices in some cases, usually lymphomatous masses rapidly response to chemotherapy and jaundice decreases due to regression of the mass, without any surgical procedure. The authors report the case of a 16-year-old girl who presented with biliary obstruction due to a neoplasm involving the duodenum. Histological examination of the specimen, which was taken from the mass by endoscopic biopsy, revealed Burkitt lymphoma infiltrating the duodenum. Chemot

Evaluation of Pringle maneuver during liver resection in a rat model of surgical obstructive jaundice.

Temporary portal triad clamping (Pringle maneuver) during liver resection reduces intraoperative blood loss. A normal liver can safely tolerate normothermic ischemia for up to 60 min. However, its safety in patients with surgical obstructive jaundice (SOJ) is not known. Therefore, we investigated the effect of hepatic ischemia in an experimental rat model of SOJ created by ligating the bile duct. Four groups of rats were created: Group I (sham operation, 10 days later, liver resection); Group II (sham operation, 10 days later, liver resection with 5 min of hepatic ischemia); Group III (bile du

Common bile duct papillary adenoma causing obstructive jaundice: case report and review of the literature.

Common bile duct (CBD) adenomas are rare tumors that often present in a fashion similar to their malignant counterparts. We report a case of a papillary CBD adenoma causing obstructive jaundice. We also present a review of the CBD adenomas found in the English literature through a Medline search (1970-2003). A 74-year-old female presented with a 10-day history of pruritus, progressive painless jaundice, acholic stools, bilirubinuria, and 10-lb weight loss. Abdominal exam showed no masses. Abdominal ultrasound (U/S) showed cholelithiasis without choledocholithiasis. Endoscopic retrograde cholan

The effect of granulocyte colony stimulating factor on the immune parameters in experimental obstructive jaundice.

BACKGROUND/AIMS: Obstructive jaundice is an important clinical problem. It may cause complications such as renal insufficiency, cardiovascular sequels, coagulation defects, gastrointestinal bleeding, delayed wound healing, secondary biliary cirrhosis and sepsis. We investigated the effect of GM-CSF on immunological parameters in the experimental obstructive jaundice. METHODOLOGY: In our experimental study we studied four groups that consisted of 28 rats. The 1st group consisted of sham rats, the 2nd group consisted of sham and GM-CSF applied rats and the 3rd and 4th groups consisted of rats t

Stones in a long pancreaticobiliary common channel: a rare cause of obstructive jaundice and pancreatitis.

A case of obstructive jaundice associated with acute pancreatitis is reported. The underlying cause was a dilated long pancreaticobiliary common channel impacted with stones and bile debris. There was no stone in the rest of the biliary system except for one in the lowest part of the common bile duct and the overall appearance of the stones suggested that they originated within the common channel itself. A long common channel may be an underlying cause of various pathological conditions in the pancreaticobiliary system. Early operative intervention is recommended when such a diagnosis is made.

Splenic hematoma following ERCP for obstructive jaundice requiring splenectomy: a case report and literature review.

INTRODUCTION AND IMPORTANCE: Splenic injury following endoscopic retrograde cholangiopancreatography (ERCP) is an exceptionally rare but potentially life-threatening complication. CASE PRESENTATION: We report the case of a female patient admitted with obstructive jaundice and suspected periampullary malignancy associated with markedly elevated CA 19-9 levels. Following ERCP, abdominal computed tomography demonstrated a large subcapsular splenic hematoma. The patient underwent splenectomy, cholecystectomy, hepaticojejunostomy, and Roux-en-Y reconstruction. Histopathological examination confirm

Prevalence of sarcopenia in patients with surgical obstructive jaundice and its impact on clinical outcomes.

BACKGROUND: Sarcopenia is a clinical syndrome presented with progressive and generalized skeletal muscle loss and function dysfunction. Usually, it is considered an age-related process influenced by genetic, lifestyle factors, and diseases. Obstructive jaundice is one of the most common pathophysiological changes in patients needing hepatobiliary or pancreatic operations that can adversely affect the tissue and organ function throughout the human body. However, the effects of obstructive jaundice on the occurrence of sarcopenia remain unclear. AIM: To investigate the incidence of sarcopenia i

Obstructive Jaundice as a Presenting Symptom of Acute Lymphocytic Leukemia in Adults: A Case Report.

We present a unique case of acute lymphoblastic leukemia in an adult. A 58-year-old male presented complaining of epigastric pain. He was initially diagnosed with obstructive jaundice, as indicated by an elevated ratio of direct bilirubin to total bilirubin levels. Bone marrow aspirate revealed hypercellular marrow with heavy infiltration of blast cells. The target cells were positive for CD20 by immunostaining. This finding is consistent with acute lymphoblastic leukemia B cell type. This case emphasizes that acute lymphoblastic leukemia can exhibit a wide range of symptoms and should be cons

Rare case report: Liver metastasis from cervical adenocarcinoma presenting with cystic mass and obstructive jaundice.

Liver metastases can originate from primary tumors in various organs; however, metastasis from cervical cancer to the liver is rare. Cervical cancer patients with distant metastases have a poor prognosis and reduced survival rates. This report describes a case of a cystic liver mass with obstructive jaundice, observed four years after resection of cervical adenocarcinoma. The lesion lacked typical imaging characteristics of hepatic metastases and was initially suspected to be a biliary neoplasm. A contrast-enhanced, ultrasound-guided needle biopsy was performed to confirm the diagnosis. Histop

Hematogenous dissemination of malignant pleural mesothelioma: An autopsy case of gallbladder metastasis causing obstructive jaundice.

Malignant pleural mesothelioma (MPM) is an aggressive cancer with poor prognosis. Although hematogenous metastasis is rare, we report an autopsy case in which gallbladder metastasis had caused fatal obstructive jaundice. A 72-year-old man, who had experienced long-term exposure to asbestos, was diagnosed with epithelioid MPM. Despite immuno- and chemotherapy, the disease progressed. Nineteen months later, the patient was admitted with jaundice and renal failure. Imaging showed mild gallbladder wall thickening, but no overt obstruction. The patient died 10 days after admission. An autopsy revea

Successful Treatment of Concurrent Cholangiohydatidosis with Obstructive Jaundice and Hepatothoracic Transit in a Pediatric Patient.

Concurrent rupture of hepatic hydatid cysts into the biliary tree and into the pleural cavity is a very rare complication in echinococcosis and can pose diagnostic and treatment challenges. We present the case of a 15-year-old female patient with recurrent abdominal pain, chest pain, fever, vomiting, jaundice, and cholangitis. Ultrasound, X-rays, computed tomography of the abdomen and thorax and cholangio-magnetic resonance imaging revealed a hepatic hydatid cyst with rupture into the main biliary duct causing obstruction, gallbladder microlithiasis, rupture of the right hemidiaphragm, and ple

Efficacy and safety of percutaneous transhepatic biliary radiofrequency ablation in patients with malignant obstructive jaundice.

BACKGROUND: Percutaneous transhepatic cholangiodrainage (PTCD) and endoscopic retrograde cholangiopancreatography/endoscopic nasobiliary drainage are the most common clinical procedures for jaundice control in patients with unresectable malignant obstructive jaundice, yet the safety and effect of endobiliary radiofrequency ablation (EB-RFA) combined PTCD is rarely reported, in this article, we report our experience of EB-RFA combined PTCD in such patients. AIM: To retrospectively study the efficacy and safety of EB-RFA combined PTCD in patients with unresectable malignant obstructive jaundice

FXR contributes to obstructive jaundice-induced vascular hyporeactivity in mesenteric arteries by reconstituting BKCa channels.

OBJECTIVE: Vascular hyporeactivity increases with the incidence of obstructive jaundice (OJ). Evidence suggests that OJ activates the farnesoid X receptor (FXR) as well as the large-conductance Ca2+-activated K+ (BKCa or MaxiK) channel. This study was designed to explore the role of the FXR in vascular hyporesponsiveness induced by cholestasis. METHODS: The OJ model rats were constructed by bile duct ligation (BDL) and treated with an FXR agonist or antagonist. Vasoconstriction of the mesenteric arteries (MAs) was assessed in vitro. Whole-cell patch clamp recordings were used to investigate B

Embryonal Rhabdomyosarcoma of the Bile Ducts Causing Obstructive Jaundice in a Child: A Case Report.

Jaundice in older children can occur when any obstruction is found within the bile ducts, either from bile stones, parasites, choledochal cysts and rarely, secondary to tumors. We present a previously well, 10-year-old Filipino boy with three-week history of progressive jaundice and tea-colored urine, and was initially assessed to have biliary ascariasis. Ultrasound showed a heterogeneous focus spanning the gallbladder neck and confirmed on endoscopic retrograde cholangiopancreatography as an exophytic mass at the ampulla of Vater. Tumor biopsy and immunohistochemical staining confirmed the di

Solid pseudopapillary neoplasm of the pancreas causing obstructive jaundice: Case report of a rare entity.

INTRODUCTION: Solid pseudopapillary neoplasm (SPN) is an extremely rare, low-grade, malignant pancreatic tumour with an excellent prognosis. We describe a case of SPN causing obstructive jaundice in a young female, thus mimicking pancreatic adenocarcinoma clinically and radiologically. CASE PRESENTATION: A 32-year-old female presented with abdominal pain for 12 h, icterus, and an epigastric mass measuring 3 × 3 cm. Imaging studies revealed features of pancreatic neoplasm in the head of pancreas, possibly pancreatic adenocarcinoma. However, ultrasound-guided fine-needle aspiration cytology of

Time progression and regional expression of brain oxidative stress induced by obstructive jaundice in rats.

BACKGROUND: Obstructive jaundice induces oxidative changes in the brain parenchyma and plays significant role in clinical manifestations of hepatic encephalopathy. We aim to study the progression of the brain oxidative status over time and the differences of its pattern over the hemispheres, the brainstem and the cerebellum. We use an experimental model in rats and measuring the oxidative stress (OS) specific biomarkers protein malondialdehyde (PrMDA) and protein carbonyls (PrC = O). RESULTS: Hyperbilirubinemia has been confirmed in all study groups as the result of common bile duct obstructi
