[Rheumatoid arthritis: surgery of the metacarpophalangeal joints of the long fingers].

The author reviews the pathophysiological mechanisms of rheumatoid deformations of the long fingers' MP joint. He considers the different possibilities of surgical treatments; then focusing on arthroplasty, he compares and comments on technical points (incision, resection of the MP head, section of intrinsic muscles, size of the implant, joint capsule and ligament reconstruction, dressing, postoperative care) and results (mobility, implant fracture, ulnar deviation and palmar dislocation) achieved by various authors. He concludes that the Swanson implant remains at present the best therapeutic

Primary flexible implant arthroplasty of the metacarpophalangeal joints during digital replantation.

Primary silicone rubber arthroplasty in a total of four metacarpophalangeal joints during digital replantation or revascularisation in two patients is reported. The results appear preferable to present alternatives and allow good function without prejudicing further management.

Histological changes in temporomandibular joints from elderly individuals. An autopsy study.

Our aim was to study microscopically the disc position and structural tissue changes such as disc perforation of the temporomandibular joint (TMJ). An autopsy material comprising the right and left TMJ from 21 individuals with a mean age of 75 years was studied. Anterior disc position was found in 11 joints from 8 individuals. Discs in anterior position were most often of even thickness or convex, whereas most discs in normal position were concave. Perforation was found in the central dense part of the disc in one joint and in the posterior disc attachment of five joints. The latter five discs…

Current concepts in the management of infections in bones and joints.

Significant changes have taken place in the epidemiology, microbiology and antibiotic therapy of bone and joint infections. Gram-negative bacilli have become an increasingly common cause, particularly in immunocompromised patients; anaerobes have been implicated in osteomyelitis associated with metallic foreign bodies; and there is increasing use of oral antibiotic regimens following an initial period of parenteral treatment. Gram-negative bacilli and anaerobes are found in polymicrobial non-haematogenous osteomyelitis (e.g. post-traumatic, post-surgical), but Staphylococcus aureus remains the

Lisfranc dislocations: fractures and/or dislocations through the tarso-metatarsal joints.

Because of the anatomical configuration of the tarso-metatarsal joints with their strong ligamentous connections, pure dislocations in this region are rare. We describe our experience of 24 cases of Lisfranc dislocation. Classification is according to Wilson. In the long term, functional and radiological results appear to depend on the accuracy of reduction. For good anatomical results, immediate closed or, if necessary, open reduction and stabilization by percutaneous Kirschner wires is required. Because of the severity of soft tissue damage, decompression with dorsal skin incisions should al…

Pigmented villonodular synovitis of synovial joints: clinical, pathologic, and radiologic features.

The clinical, pathologic, and radiologic features of pigmented villonodular synovitis of large synovial joints are presented. The typical plain-film presentation of this unusual entity is a noncalcified capsular soft-tissue mass of the knee, without bony abnormalities. When pigmented villonodular synovitis occurs in other joints, it is often accompanied by cystic bone erosions. This important radiographic finding might mislead one to an improper diagnosis of neoplasia or infection. This review reveals insufficient emphasis in the radiologic literature on the high incidence of bone lesions in j…

Treatment of fungal infections in the bones and joints with ketoconazole.

Seven patients with fungal infections in the bones and joints were treated with ketoconazole. One patient had aspergillosis, two had sporotrichosis, and four had coccidioidomycosis. All of the organisms that were tested demonstrated in vitro sensitivity to ketoconazole. Patients were treated with 200-600 mg of oral ketoconazole per day for a minimum of six months. Initially four of seven patients responded to therapy, but only one was cured. The other three patients did not respond. Treatment failure may be due to poor penetration of ketoconazole into bone. Ketoconazole will not cure fungal os…

Early, asymptomatic stage of degenerative joint disease in canine hip joints.

The early stages of degenerative joint disease were investigated in coxofemoral joints from dogs with a hereditary predisposition to hip dysplasia. Alterations observed included mild nonsuppurative synovitis, increased volume of both synovial fluid and the ligamentum teres, and focal degenerative articular cartilage lesions. On radiologic examination, subluxation of the femoral head was seen, but only in the most severely affected joints. Synovial inflammation with increased synovial fluid and ligament volumes were indicators of early degenerative joint disease in dogs. These changes seemed to…

Active muscle torques about long-bone axes of major human joints.

Incorporaton of the activee muscle torques into the multisegmented total-human-body models can be considered the next major improvement of These models. The first step toward achievement of this goal is to obtain active muscle torque data about long-bone axes of major human joints. This paper presents results of research conducted to collect such data at the shoulder, hip, elbow, knee, and ankle joints. Active muscle torques were determined about the long-bone axes of the upper arm, upper leg, lower arm, and lower leg, which are the body segments associated with the major joints mentioned. Num…

Postoperative Cryotherapy in Joints Other Than the Knee: A Systematic Review of Pain, Edema, Analgesic Use, and Blood Loss in the Shoulder, Hand, Hip, and Ankle Joints.

BACKGROUND: Cryotherapy has proven effective for pain reduction in the knee joint. However, its efficacy in other joints is less clear. PURPOSE: To explore the benefits of cryotherapy in joints excluding the knee. STUDY DESIGN: Systematic review; Level of evidence, 4. METHODS: In June 2023, a review of EMBASE, PubMed MEDLINE, PROSPERO, ClinicalTrials.org, and the Cochrane Library was performed following PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) 2020 guidelines. Search terms were "cryotherapy,""cold application,""postoperative," and "postoperative period." S…

Dexamethasone palmitate-loaded nanodiscs target inflamed joints and suppress rheumatoid arthritis.

Rheumatoid arthritis (RA) is driven by persistent joint inflammation and dysregulated adaptive immune responses, highlighting the need for drug delivery strategies that can concentrate immunomodulatory therapy at inflamed joints and disease-associated lymphatic tissues. Here, we formulated dexamethasone palmitate-loaded synthetic high-density lipoprotein nanodiscs (DXP-NDs) as a biomimetic nanodelivery platform for subcutaneous administration and inflammation-directed delivery. DXP-NDs exhibited excellent encapsulation efficiency (95.8% ± 2.4%) and maintained their ~10 nm size for 60 days duri…

Design features in human joints with recommendations for humanoid robot design.

There are many types of moveable joint in the human body, with each characterised by compactness and high functionality. In contrast, humano … designers. The paper highlights key future goals for humanoid robotics such as replicating the MTP joints of the foot, replicating the push-retract motion in fingers and replicating the compact linkage pronation mechanism of the wrist. Human joints exhibit design principles that are quite different to those of traditional engineering such as high degree …

From Joints to the Heart: An Integrated Perspective on Systemic Inflammation.

BACKGROUND: Rheumatoid arthritis (RA) is an autoimmune inflammatory disease which predominantly affects joints, but it can also lead to significant extra-articular complications, particularly cardiovascular disease (CVD). Chronic systemic inflammation promotes endothelial dysfunction and accelerates atherosclerosis, increasing cardiovascular risk. METHODS: Current data were analyzed to explore the mechanisms between RA and CVD, focusing on systemic inflammation, pro-inflammatory cytokine patways (IL-1, IL-6, TNF, and JAK-STAT), and their interactions with traditional cardiovascular risk facto…
