The current issues and challenges in the management of floating knee injury: a retrospective study.

PURPOSE: The management of floating knee injuries is still controversial and challenging for trauma specialists. This study aims to evaluate the incidence of the floating knee in lower limb trauma, analyzing the challenges in its management, and factors affecting clinical outcomes. METHODS: In this mono-center retrospective study, 36 consecutive patients were included. All individuals were diagnosed with an ipsilateral fracture of the femur and tibia, managed surgically according to their fracture pattern (Fraser classification), and the severity of the injury. The timing for each operation w…

Direct thermal injury to the popliteal artery after total knee arthroplasty.

Vascular damage after total knee arthroplasty is rare. However, delayed diagnosis and management may cause adverse outcomes for patients. In particular, direct thermal injury to the popliteal artery after total knee arthroplasty is extremely rare. A 74-year-old woman presented to another institution with a left popliteal artery injury after left total knee arthroplasty. Arteriography revealed total occlusion of the popliteal artery, and emergency surgery was performed. Because of the total occlusion of the popliteal artery due to severe direct thermal injury, anastomosis was performed in an en

Influence of a posteromedial meniscocapsular injury on the knee anterior laxity. A cadaveric study.

PURPOSE: The number of anterior cruciate ligament (ACL) injuries has considerably increased in the recent years, especially in young adults and adolescents. Associated meniscal tears increase anterior and rotary laxity. Posterior peripheral meniscocapsular tear of medial meniscus is also called ramp lesion. Prevalence of 9% to 17% in adults and up to 23% in pediatric population has been reported. The aim of this study was to determine anterior laxity of cadaveric ACL-deficient knees with several size of ramp lesions. METHODS: Fresh cadaveric knees were explored. Major osteoarthritis and/or AC

Lower-leg injury and knee arthroscopy have distinct effects on coagulation.

It is unknown how lower-leg injury and knee arthroscopy, both associated with venous thromboembolism (VTE), affect coagulation. To study the effect of (1) lower-leg trauma and (2) knee arthroscopy on coagulation, plasma samples of the Prevention of Thrombosis following CAST immobilization (POT-CAST, #NCT01542762) and Prevention of Thrombosis following Knee Arthroscopy (POT-KAST, #NCT01542723) trials were used, which were collected shortly after lower-leg trauma and before/after (<4 hours) knee arthroscopy. For aim 1, 1204 lower-leg injury patients were compared with preoperative samples of 100

Acute Management of Combined Floating Hip and Knee Injury: A Case Report.

INTRODUCTION: Surgical management of isolated acetabular fracture dislocations, femoral shaft fractures, and tibial shaft fractures are well-documented, but there is limited literature on the acute management of these injuries when they occur concomitantly in the same patient on the ipsilateral extremity. We present a case of the acute treatment of a 33-year-old patient with a right ipsilateral floating hip and knee injury secondary to a motor vehicle accident. We offer a unique surgical technique for the urgent management of this rare combined injury. CASE PRESENTATION: A 33-year-old female

Environmental Risk Factors for Osteoarthritis: The Impact on Individuals with Knee Joint Injury.

Osteoarthritis is a debilitating chronic condition involving joint degeneration, impacting over 300 million people worldwide. This places a high social and economic burden on society. The knee is the most common joint impacted by osteoarthritis. A common cause of osteoarthritis is traumatic joint injury, specifically injury to the anterior cruciate ligament. The purpose of this review is to detail the non-modifiable and modifiable risk factors for osteoarthritis with particular focus on individuals after anterior cruciate ligament injury. After reading this, health care providers will better c

Health-related Outcomes after a Youth Sport-related Knee Injury.

PURPOSE: Active youth are vulnerable to knee injury and subsequent osteoarthritis. Improved understanding of the association between health-related outcomes and history of joint injury could inform osteoarthritis prevention strategies. The purpose of this historical cohort study is to examine the association between youth sport-related knee injury and various clinical, physiological, behavioral, and functional health-related outcomes, 3-10 yr postinjury. METHODS: Participants included 100 individuals who experienced a youth sport-related knee injury 3-10 yr earlier and 100 age-, sex-, and spo

[CLINICAL AND ARTHROSCOPIC FINDINGS IN MENISCAL INJURY AND KNEE INSTABILITY: A PROSPECTIVE STUDY].

INTRODUCTION: Medical history and physical examination are expected to provide the basic knowledge allowing diagnosis of a disease and thus enabling to plan the course of treatment. OBJECTIVE: This study aimed to examine this hypothesis by comparing pre-operative diagnosis of meniscal and anterior cruciate ligament (ACL) injuries to final surgical findings. MATERIAL AND METHODS: We prospectively compared the pre-surgical diagnosis to the arthroscopic findings in 753 arthroscopic procedures. RESULTS: Clinical diagnosis of a medial meniscal (MM) tear was proven by arthroscopy in 65% of cases.

Total knee arthroplasty in patient with paraplegia after spinal cord injury.

The clinical management of paraplegic patients is more complex than in able-bodied subjects. Spinal cord injury (SCI) affects younger, active people more often than the elderly during high-energy fall or traffic accidents. In order to return to work after suffering an SCI, patients need to regain their functional independence, especially their ability to drive. The literature lacks strong evidence addressing the surgical solutions in severe knee arthrosis in paralyzed patients after SCI. We present a favourable outcome of total knee arthroplasty (TKA) of a stiff knee in extension in a man with

A rare knee extensor mechanism injury: Vastus intermedius tendon rupture.

INTRODUCTION: Quadriceps tendon injuries are rare. There is a limited number of studies in the literature, reporting partial quadriceps tendon ruptures. We did not find any study reporting an isolated vastus intermedius tendon injury in the literature. PRESENTATION OF CASE: A 22 years old professional rugby player with the complaints of pain in the right lower limb, decreased range of motion in right knee and a mass in the mid-anterior of the right thigh applied following an overloading on his hyperflexed knee during a rugby match. T2 sequence magnetic resonance images revealed discontinuity

The iliotibial band in acute knee trauma: patterns of injury on MR imaging.

OBJECTIVE: To delineate the spectrum of knee injuries associated with sprains and tears of the distal iliotibial band (ITB). MATERIALS AND METHODS: A retrospective review of 200 random MRI scans undertaken for acute knee trauma was performed. Scans were excluded if there was a history of injury over 4 weeks from the time of the scan, septic arthritis, inflammatory arthropathy, previous knee surgery, or significant artefact. In each scan, the ITB was scored as normal, minor sprain (grade 1), severe sprain (grade 2), and torn (grade 3). The menisci, ligaments, and tendons of each knee were also…

Rates of knee injury in the U.S. Active Duty Army, 2000-2005.

We sought to summarize knee injuries (KI) in the U.S. Active Duty Army (ADA) in terms of absolute numbers, examine current rate trends, and identify ADA who were at increased risk for experiencing a KI. We used the Total Army Injury and Health Outcomes Database (TAIHOD) to compute unadjusted and adjusted rates of KI, categorized by the Barell Matrix, within the ADA for the years 2000-2005. During this period, 21 to 25 per 1,000 ADA suffered from KI. The highest yearly rates were observed for knee dislocation and sprains/strains (31 per 1,000 ADA). In ADA with a history of a KI (within 2 years)…

Vascular injury after occult knee dislocation presenting as compartment syndrome.

BACKGROUND: Popliteal artery injury can occur in up to one-third of patients with knee dislocation. Delay in the diagnosis of popliteal artery injury is the leading cause of amputation in this limb-threatening injury. OBJECTIVES: To remind emergency physicians to consider popliteal artery injury in any pulseless lower extremity, even in cases of spontaneous reduction of the knee dislocation before evaluation by medical personnel. CASE REPORT: We present a case of popliteal artery injury and occult knee dislocation in which the diagnoses were delayed for hours by incorrectly attributing the a

Costs and effectiveness of a brief MRI examination of patients with acute knee injury.

The aim of this study was to assess the costs and effectiveness of selective short magnetic resonance imaging (MRI) in patients with acute knee injury. A model was developed to evaluate the selective use of MRI in patients with acute knee injury and no fracture on radiography based on the results of a trial in which 208 patients were randomized between radiography only and radiography plus MRI. We analyzed medical (diagnostic and therapeutic) costs, quality of life, duration of diagnostic workup, number of additional diagnostic examinations, time absent from work, and time to convalescence dur
