Medical management of vestibular dysfunction.

An overview of the medical management of vestibular dysfunction is presented. The lack of firm etiologic and pathophysiologic data on which to base rational treatments is emphasised. Medical management of Meniere's disease is reviewed in depth while that of other vestibular disorders is touched on briefly.

[Multiple affections of the cranial nerves as a pathognomonic sign of periarteritis nodosa. Relationship with Cogna's syndrome (author's transl)].

A 43-year-old woman was found to have multiple cranial neuritis affections, present as an isolated disorder over a long period, and characterized by bilateral deafness and associated lesions of the VII and V cranial nerves. On investigation, visual disturbances were discovered which were of a transitory nature and made up a total clinical picture suggesting Cogan's syndrome. Neuropathological examination revealed the presence of typical periarteritis nodosa lesions in the cranial nerves. The authors suggest, therefore, that certain cases described as Cogan's syndrome are in fact particular for

[Surgical interventions for ameliorating of equilibrium disorders refractory to conservative therapy].

In cases of vertigo resistant to conservative treatment, surgery can be valuable for labyrinthine fistula, perilymphatic fistula, cupulolithiasis and Menière's disease. For instance, covering a labyrinthine fistula by a fascial graft relieves the vertigo in an amazingly short time. Perilymphatic fistula is verified and treated by tympanotomy and is used for patients with special auditory and/or vestibular features. Cupulolithiasis usually requires no surgical treatment: special postural training will speed recovery, but a few cases need neurectomy of the inferior vestibular nerve. There are se

Delayed endolymphatic hydrops: study and review of clinical implications and surgical treatment.

Delayed endolymphatic hydrops (DEH) differs from Ménière's disease in that it occurs in pre-existing ear pathology in patients who have a profound unilateral or total deafness that was caused by infection, trauma, or unknown causes during childhood or adulthood. We performed a retrospective review of 160 patients with ipsilateral, contralateral, or bilateral DEH. Eighty-seven patients who did not respond to medical therapy underwent surgical treatment. Our findings indicate that the more conservative surgical procedures--endolymphatic sac surgery, cochleosacculotomy, and streptomycin perfusion

[Otosclerosis--diagnosis and therapy].

Otosclerosis (synonym: otospongiosis) is a focal or diffuse spongifying disease of the bony labyrinth. So far pathogenesis is unknown, some recent investigations assume a paramyxovirus infection. But there are no doubts about hereditary and genetic factors, females are twice often affected as males with a maximum incidence between 20 and 40 years. If the disease invades the oval window niche it causes fixation of the stapes with conducting hearing loss. In some cases otospongiosis is associated with and presumably causes cochlear degeneration alone with sensoneurale hearing impairment of varyi

[Ultrastructure of the vestibular apparatus in patients with Meniere's disease].

The vestibular labyrinth of eight patients with Menière's disease obtained by translabyrinthine vestibular neurectomy or transtympanal labyrinthectomy was studied with the light microscope and transmission electron microscope. Previous invasive therapy was transtympanal application of gentamicin in one patient and endolymphatic sac surgery in two patients. Some of the pathological findings were unspecific, e.g. mucoid degeneration of the subepithelial spaces, fibrous long spacing collagen and high amount of lipofuscin in different cell types. Cuticular laminated structures were seen in some of

Perilymph total protein levels associated with cerebellopontine angle lesions.

The total protein levels in the perilymph of patients with acoustic neuromas, meningiomas, and Meniere's disease are contrasted. All specimens were obtained from the lateral semicircular canal during removal of the bony labyrinth. Any specimens with significant contamination by hemoglobin or serum proteins, as judged by the presence of hemoglobin/haptoglobin complexes, were discarded from this study. The findings of greatly increased perilymph protein levels in the tumor groups confirmed the reports of other workers. The levels in the Meniere's disease group compared favorably with the normal

[Effect of the Tahyña virus on Langat virus persistence in the central nervous system of the mouse].

The features of development of a mixed infection in which one of the infectious agents (Tahyña virus) had a marked immunodepressive activity and the other (Langat virus) caused predominantly asymptomatic course of the disease were studied. Successive infection with these agents increased the severity of the disease (lethality in monoinfection 7-18%, in mixed infection 57%). At the same time the duration of persistence in the brain of mice of Langat virus but not of Tahyña virus increased up to 110 days. It was demonstrated by the method of linear labyrinth that long-term asymptomatic carrier s

Carbon monoxide poisoning: an unusual cause of dizziness.

OBJECTIVE: We report an unusual case of dizziness caused by carbon monoxide poisoning. CASE REPORT: A 55-year-old man was referred to an ENT surgeon with dizziness. The patient described vague, non-specific symptoms not consistent with a diagnosis of benign paroxysmal positional vertigo, labyrinthitis or Ménière's disease. It emerged later that the patient had been suffering from carbon monoxide poisoning from a leaky gas hot water boiler in his house. After having the boiler fixed, the patient's symptoms completely resolved. CONCLUSION: When the more common causes of dizziness cannot be fou

[Facial nerve and petrous bone cholesteatoma].

The diagnosis and treatment of petrous bone cholesteatoma is a challenge to aural surgeons. Seven patients with extensive petrous bone cholesteatomas which invaded the labyrinth and fallopian canal are presented. These cholesteatomas originated as secondary to acquired lesions. The cases were evaluated according to the clinical features, the intraoperative findings, the radiological findings, and the surgical approaches. In this series, six patients presented with facial paralysis associated with profound or total deafness. The transtemporal lateral approach was used in all the cases. Acute fa

The predictive value of high resolution computerized tomography in chronic suppurative ear disease.

High resolution computerized tomographic scanning was used in the preoperative evaluation of 50 patients with chronic suppurative ear disease. The radiological predictions were compared with the operative findings in each case. This form of imaging proved to be highly accurate in depicting the extent of soft tissue within the middle ear cleft and mastoid. With the exception of the long process of the incus and the stapes superstructure, the state of the ossicular chain was correctly predicted in over 90% of cases. Erosion of the labyrinth was clearly depicted in 4 of the 5 cases in which it oc

Evidence for a perilymphatic origin of the endolymph: application to the pathophysiology of Ménière's disease.

The origin of the endolymph was elucidated by kinetic studies of the entry of water and electrolytes into endolymph and perilymph after intravenous administration of radioactive tracers in rats. The compartmental analysis of the data and the comparison of this study with the results of Konishi and associates (Acta Otolaryngol (Stockh) 86, 22-34 and 176-184, 1978), using perilymphatic perfusion of tracers, indicate that perilymph rather than plasma may be considered the precursor of endolymph. Since the cochlear epithelium was found to be freely permeable to water, an alteration of electrolyte

Complications of otitis media.

Although intracranial otogenic complications occur less frequently than in the preantibiotic period, their mortality rate is still relatively high. On the basis of some personal clinical observations made during the last decade attention has been drawn to the most common intra- and extracranial otogenic complications (posterior fossa syndrome, labyrinthine fistula, labyrinthitis, facial palsy) mostly due to cholesteatoma, and a less frequent intracranial complication occurring mainly in children (otitic hydrocephalus) caused by chronic otitis media. It may therefore be concluded that otitis me
