INTRODUCTION: Lichen planus is a chronic inflammatory disease of the skin, rarely associated with a thymoma.
CASE REPORT: A 72-year-old woman with erosive buccal lichen, hypertrophic lichen planus of the lower limbs, severe myasthenia and acquired hypogammaglobulinaemia associated with thymoma. CD8 lymphocyte count was increased. In the months following surgical ablation of the thymoma, the clinical examination and laboratory findings progressively returned to normal. Two years later, the patient was in good health.
DISCUSSION: To our knowledge, this is the first report of favourable outcome

We describe the second case of annular atrophic lichen planus. The annular configuration of the lesions resulted from peripheral enlargement with simultaneous central clearing. Histology showed features of lichen planus in the active border of the lesion and a pattern of resolved lichen planus in its center. Elastic fibers had been destroyed in the papillary dermis both in the border and in the center of the lesion, resulting in an atrophic appearance of the lesion. Annular atrophic lichen planus is an uncommon variant of lichen planus that results from elastolytic activity of inflammatory cel

Lichen planus is a somewhat frequent oral lesion. The most prevalent type is reticular, which is asymptomatic. The erosive and atropic forms, although not as common, cause discomfort or pain, and the patient usually demands attention. Treatment modalities include the use of topical steroids, although retinoic acid, griseofulvin and cyclosporine have been used. The occurrence of squamous cell carcinoma in areas of lichen planus make it necessary to follow up on patients suffering from chronic lesions.

Two patients with lichen sclerosus (et atrophicus) are described in whom there was conspicuous localization of lesions to sites subjected to friction from tight clothing. This was highly suggestive of the Köbner phenomenon. Both patients experienced symptomatic improvement following avoidance of pressure on the skin from tight clothing.

BACKGROUND: Lichen planopilaris is believed to be a variant of lichen planus because both diseases have similar histologic features. However, as the clinical features of the two diseases differ, we conducted immunofluorescence studies to examine the relation between the two conditions more closely.
OBSERVATIONS: Direct immunofluorescence was performed on scalp lesions of seven patients with lichen planopilaris. All had abnormal linear deposits of Ig restricted to the basement membrane zone of hair follicles. The deposits consisted of only IgG or IgA in five patients (70%) and of IgG in combin

Skin biopsies from three patients with lichen nitidus were studied using a battery of monoclonal antibodies against cells in the dermal infiltrate. In each case the findings were identical with a marked excess of CD4+ cells over CD8+ cells and the presence of large numbers of CD1+ cells. These results are very similar to those seen in lichen planus reinforcing the association between these two conditions.

Oral dapsone 200 mg daily for 16 weeks was tried on 52 adult patients, of whom 33 patients (males 17, femals 16) completed the study. No other topical or systemic medication was permitted. All the patients tolerated the drug very well. In 22 (66.5%) patients there was complete healing with hyperpigmentaiion after 16 weeks of therapy. In 7 (21.5%) patients partial respone was seen. There were 4 (12%) treatment failures. In patients with hiucosal lesions, the response started early and was seen more often (80.0%) No relapse was seen in any of the patients followed-up for a preiod of one year.

Twenty-three patients diagnosed as lichen sclerosus with colposcopic directed biopsy were given topical testosterone as a first line therapy. The overall remission rate was found to be 87.7%; 8.7% of the patients experienced undesirable androgenic side effects. Of the three patients who did not respond satisfactorily to testosterone treatment, two had alcohol injection to the vulva and one had vulvar denervation.

Lichen nitidus (LN) and lichen planus (LP) are considered by some investigators to be two variants of the same disease, and by others to be two distinct dermatoses. In order to obtain further information about the relationship between LN and LP we examined the ultrastructure of lesions from two LN patients. In the central part of the lesion, the basement membrane was absent, or was interrupted by migrating phagocytes or lymphocytes. The basal cells and the lower cells of the stratum spinosum exhibited karyolysis and appeared to be compressed and often necrotic. In the upper dermis irregular ce
