Allograft ligament transplantation in anterior cruciate ligament reconstruction patients with meniscal tears.

BACKGROUND: It currently remains unclear whether the meniscal repair clinical results were affected by the graft used in anterior cruciate ligament (ACL) reconstruction. This retrospective study designed to evaluate the difference in clinical outcomes of meniscal repair using autograft and allograft for the ACL reconstruction. METHODS: The injury of the ACL and meniscus was evaluated with MRI and treated simultaneously. One hundred and eighty-nine cases were initially fulfilled the study criteria, and had the surgery in the period June 2007 and July 2010. Thirty-four patients were lost to fol

Anterior cruciate ligament reconstruction with the ligament augmentation and reconstruction system: a systematic review.

PURPOSE: The Ligament Augmentation and Reconstruction System (LARS) is a third generation of synthetic ligament, designed to overcome the issues of graft failure and synovitis which led previous generations of synthetic ligaments to fall out of favour. The theoretical benefits of LARS are appealing but this has not led to widespread uptake of the system in preference to autograft. The aim of this systematic review is to assess whether the evidence exists to support the use of LARS with respect to outcomes and complications. METHODS: A systematic search process was undertaken from January 1990…

Effects of ligament repair on laxity and creep behavior of an early healing ligament scar.

BACKGROUND: Previous clinical studies have reported that conservatively managed medial collateral ligament (MCL) injuries remained unstable … the failure load, stiffness, and tensile strength between gap scars and Z-plasty scars. All healing ligaments had significantly lower failure load, stiffness, and tensile strength than normal controls. CONCLUSIONS: The present study demonstrates that ligament repair is effective in decreasing short-term laxity of ligament scars. These 6-week scars would be …

Roles of the anterior cruciate ligament and the medial collateral ligament in preventing valgus instability.

Both the medial collateral ligament (MCL) and the anterior cruciate ligament (ACL) are reported to prevent valgus instability of the knee. In this study, the anatomical mechanisms by which these ligaments prevent valgus instability were experimentally investigated. The valgus rotation angle and the magnitude of the medial joint space opening were measured in six cadaveric knees, using biplanar photography before and after the MCL and/or the ACL were severed. A significant increase in the valgus rotation angle and a large medial joint space opening were observed when the MCL was severed. An inc…

Snow skiing combined anterior cruciate ligament/medial collateral ligament disruptions.

Recent reports indicate that combined anterior cruciate ligament/medial collateral ligament (ACL/MCL) knee injuries are usually associated with a lateral meniscus tear. In our center, snow skiing is the athletic activity most frequently associated with this double-ligament injury complex. A sports-specific analysis was undertaken to evaluate the hypothesis that the snow skiing ligament injury is different from similar injuries caused by other athletic activities. Of a total of 64 acute arthroscopically confirmed tears of both the MCL and ACL, 23 were caused by snow skiing and 41 by nonskiing a

Quadriceps or patellar ligament reconstruction with artificial ligament after total knee replacement.

BACKGROUND AND AIM OF THE WORK: Ruptures of the quadriceps or patellar tendon after TKA implantation are a fearful and disabling complication. Direct reconstructions highlighted various problems, such as the decrease in extensor force, failure, need for prolonged post-operative immobilization. So augmentation techniques have been proposed with autologous tendons, allografts, artificial ligaments. Among these, the LARS seems the most modern and promising. For this reason, we have retrospectively reviewed a case series patients operated with this artificial ligament, to evaluate the results and …

Anatomic Measurement and Variability Analysis of the Anterior Talofibular Ligament and Calcaneofibular Ligament of the Ankle.

BACKGROUND: The anterior talofibular ligament (ATFL) and calcaneofibular ligament (CFL) contribute greatly to the overall stability of the ankle joint; however, ATFL and combined ATFL-CFL sprains are common. Anatomic reconstruction of the lateral collateral ligament with grafts has been proposed for patients with poor tissue quality or inadequate local tissue. Anatomic reconstruction of the lateral ankle ligaments requires a good understanding of their anatomic location. PURPOSE: To describe the anatomy of the ATFL and CFL ligaments quantitatively and qualitatively and explore the relationshi…

Scapholunate Ligament: Comparing the Native Strength of the Ligament to an All-suture Anchor Construct.

Our aim was to compare the tensile strength of the native scapholunate ligament (SLL) with that of an all-suture anchor construct in a cadaveric model. The scaphoid and lunate were isolated, preserving all segments of the SLL. Using a servohydraulic testing machine, we increased the load until peak load-to-failure of the native SLL was reached in nine specimens (mean ± standard deviation, 273 ± 132 N). Using the same specimens, two JuggerKnot 1.4-mm suture anchors (Zimmer Biomet) were placed into the lunate and tensioned through transosseous tunnels in the scaphoid. Sutures were tied over the

Ossification of the Yellow Ligament Combined with Ossification of the Posterior Longitudinal Ligament at the Cervicothoracic Junction.

BACKGROUND: Both ossification of the yellow ligament (OYL) and the ossification of the posterior longitudinal ligament are relatively rare clinical entities. We report a extremely rare case of the spinal canal stenosis because of OYL, OPLL and listhesis at the cervicothoracic junction. CASE DESCRIPTION: A 69-year-old man had progressive pain over his bilateral axillar portion and right lower extremity for 1 year. Radiology showed cervical canal stenosis with ossification of OYL, OPLL, and listhesis at the cervicothoracic junction. Posterior decompression therapy was performed, and he recovere

The lunotriquetral joint: kinematic effects of sequential ligament sectioning, ligament repair, and arthrodesis.

This experiment was conducted to study the effects of sequential sectioning of the ligaments of the lunotriquetral (LT) joint and the effects of simulated repair or arthodesis on kinematics of the wrist joint using an x-ray stereophotogrammetric technique. A 3-dimensional coordinate software program calculated relative motion between bodies as screw axis displacement and rotation about each axis. Sectioning of the proximal and dorsal component of the LT ligament had little effect on carpal kinematics, but sectioning of the proximal and palmar components of the ligament resulted in flexion of b…

The Dacron ligament prosthesis in anterior cruciate ligament reconstruction. A four-year review.

We studied 40 patients who underwent reconstruction for chronic anterior cruciate ligament deficiency with a Dacron ligament prosthesis using a modified MacIntosh over-the-top technique, augmented with iliotibial band. Thirty patients had undergone at least 1 prior surgical procedure on the affected knee, but only 7 patients had previous anterior cruciate ligament reconstruction. All patients were followed for a mean of 47.5 months. The results at final followup demonstrated an average side-to-side arthrometer difference of 1.0 mm. The Lysholm score improved from 65 preoperatively to 89 at the

Internal bracing with an artificial ligament for superficial medial collateral ligament injury impairs the mechanical property of repaired native ligament: A porcine study.

BACKGROUND: Superficial medial collateral ligament (sMCL) injury is common, but there are no in vivo studies describing the biomechanical efficacy of anatomic repair with internal bracing for grade 3 injuries. METHODS: We used a porcine model to evaluate the efficacy of internal bracing using an artificial ligament for the early repair of acute, grade 3 sMCL injuries. Sixteen male castrated pigs were randomly assigned to a repair group: (1) anatomic repair using two suture anchors (Group R; n = 9) or (2) anatomic repair with internal bracing augmentation (Group IB; n = 7). The left knees were

Effect of anterior cruciate ligament reconstruction with patellar tendon or prosthetic ligament on the morphology of the other ligaments of the knee joint. An experimental study in dogs.

The effect of anterior cruciate ligament replacement on the other ligaments of the knee, including the medial or lateral collateral and posterior cruciate ligaments, was studied histologically in the canine knee. The anterior cruciate ligament was replaced either with the medial third of the patellar tendon or with a prosthetic ligament. Histologic analysis was performed 3 months after prosthetic ligament implantation and 1 year after patellar ligament reconstruction. In all ligaments, numerous pathologic alterations were found. Degenerative changes such as glycosaminoglycan accumulation, meta…

Double-Bundle Posterior Cruciate Ligament and Concomitant Medial Collateral Ligament and Posterior Oblique Ligament Reconstruction After a Motor Vehicle Accident.

BACKGROUND: Motor vehicle accidents constitute the most prevalent high-energy mechanisms for knee dislocation, resulting in multiligament knee injuries (MLKIs). When concomitant posterior cruciate ligament (PCL) and posteromedial corner injury becomes chronic after failed conservative treatment, surgical intervention is indicated to restore the native knee biomechanics. INDICATIONS: Double-bundle PCL reconstruction is indicated for chronic instability, increased posterior tibial translation, decreased tibial slope, and failure of conservative treatment measures. posteromedial corner reconstru

A Simple and Atraumatic Loop-Passing Technique for Tibial Anterolateral Ligament Graft Passage in Sonnery-Cottet Anterior Cruciate Ligament + Anterolateral Ligament Reconstruction.

The purpose of this technical note is to describe a simple and atraumatic loop-passing technique that facilitates tibial anterolateral ligament (ALL) graft passage in Sonnery-Cottet combined anterior cruciate ligament and ALL reconstruction. In the Sonnery-Cottet technique, 2 converging tibial tunnels are created for the ALL, requiring a passing loop to shuttle the graft between tunnels. Conventionally, a needle-mounted braided suture is advanced blindly through subcutaneous tissue, which can be unreliable in patients with thick subcutaneous tissue and may increase the risk of cortical breach
