Long-term stability of keratometric astigmatism after limbal relaxing incisions.

PURPOSE: To study the keratometric stability of limbal relaxing incisions (LRIs) performed during cataract surgery. SETTING: Princess Royal University Hospital, Kent, United Kingdom. DESIGN: Retrospective case series. METHODS: Medical notes of patients who had small-incision cataract surgery combined with LRIs between November 2006 and December 2010 were reviewed. Keratometric astigmatism was measured using Scheimpflug pachymetry (Pentacam). Surgically induced astigmatism (SIA) was calculated using the Astig Plot application. RESULTS: Twenty eyes of 20 patients were evaluated. The median k

One donor cornea for 3 recipients: a new concept for corneal transplantation surgery.

OBJECTIVE: To describe the use of a single donor corneal tissue in 3 patients with corneal pathologic conditions. METHODS: A donor corneal tissue was divided into 3 parts using a microkeratome and a trephine. The anterior lamellar disc was transplanted into a patient with macular corneal dystrophy using the automated lamellar therapeutic keratoplasty technique. The posterior lamellar disc was transplanted into a patient with pseudophakic bullous keratopathy using the Descemet stripping automated endothelial keratoplasty technique. The peripheral corneoscleral rim was used for limbal stem cell

Implementing stem-cell therapies for limbal stem cell deficiency.

Limbal stem cell deficiency (LSCD) is a severe ocular surface disorder that remains a major therapeutic challenge, particularly in bilateral disease where autologous limbal tissue is unavailable. Over the past three decades, stem cell-based interventions have transformed LSCD management, evolving from large limbal grafts to refined ex vivo expansion techniques and alternative stem cell sources. Despite these advances, long-term clinical outcomes remain variable and are strongly influenced by disease severity, immune rejection, stem cell quality, and optimization of the ocular surface microenvi

Ocular Surface Inflammation as a Driver of Cornea Limbal Stem Cell Deficiency: Mechanisms and Implications.

Ocular surface inflammation is a major disruptor of corneal epithelial homeostasis and a key driver of limbal stem cell deficiency (LSCD). Limbal stem cells (LSCs), residing within the specialized limbal niche, maintain corneal transparency through continuous epithelial renewal and by preventing conjunctival encroachment onto the corneal surface. Chronic or severe inflammatory insults-stemming from systemic autoimmune disorders, ocular surface diseases, infections, trauma, or environmental stressors-can damage both LSCs and their microenvironment, ultimately leading to limbal insufficiency. Modified Simple Limbal Epithelial Transplant for the Treatment of Symblepharon in Cats: A Pilot Study.

PURPOSE: To describe a surgical technique to treat keratoconjunctival symblepharon with a modified simple limbal epithelial transplant (SLET). METHODS: Four cats underwent an autologous modified SLET from a normal contralateral eye to the recipient eye. Patients were scored preoperatively and postoperatively based on the degree of cornea affected and the corneal clarity. RESULTS: Three of the four cases had significant improvements in both their corneal clarity scores and the amount of cornea occupied by conjunctiva. One case developed a corneal infection in the recipient eye 1 month postope

Biology and medicine on ocular surface restoration: Advancements and limits of limbal stem cell deficiency treatments.

Ocular vision can be hampered by corneal damages, sensibly reducing patients' quality of life and having important social and economic consequences. Ocular surface diseases, which often lead to corneal opacities with visual impairment are the most severe forms of the Limbal Stem Cell Deficiency (LSCD). The present review provides an updated perspective on the available treatments for LSCD, focusing on clinical and biological features, as well as critical points to monitor during clinical translation. Recently developed surgical treatments for LSCD are described, along with their benefits and l

Medical management and visual rehabilitation of limbal niche dysfunction.

Limbal niche dysfunction (LND) has been described as a medically reversible form of limbal stem cell deficiency. The current literature is sparse regarding therapeutic options to improve visual function after stabilization of the disease. A 61-year-old monocular woman with an extensive medical and ocular history presented with long-standing recalcitrant epitheliopathy in both eyes. History and examination findings on presentation led to a diagnosis of LND, and medical therapy was initiated. After 8 months, her ocular surface had improved and corrected distance visual acuity (CDVA) was 20/40. S

Treatment of superior limbic keratoconjunctivitis with topical tacrolimus 0.03% ointment.

PURPOSE: To report 2 cases of superior limbic keratoconjunctivitis (SLK) treated with topical tacrolimus 0.03% ointment (Protopic; Fujisawa Healthcare). METHODS: A female patient aged 52 years and a male patient aged 21 years presented with SLK in both eyes. Both patients had previously received several treatments [such as topical steroids and lubrication, contact lenses, silicone punctal plugs, and vitamin A eye drops], with no improvement. Because of poor response to initial management, the authors started treatment with topical tacrolimus 0.03% twice a day. Objective tarsal conjunctiva inf

[Resistance of limbo-keratoplasty to blunt trauma].

BACKGROUND: Traumatic wound dehiscence is a rare complication after penetrating keratoplasty. Limbo-keratoplasty seems to alter the pattern of rupture after blunt trauma. CASE REPORT: We report on one patient with dehiscence of a penetrating limbo-keratoplasty after blunt trauma. RESULTS: The dehiscence occurred over the entire extent of the clear corneal scar, whereas the sclero-corneal interface remained adapted. CONCLUSIONS: This case suggests a different tensile strength of these two scar types, possibly associated with their different grades of vascularization or epithelialization.

Perilimbal aneurysms of conjunctival vessels in glaucoma patients.

Aneurysms of the conjunctival vessels occur frequently. Such aneurysms have even been described in glaucomatous patients but their significance in this condition is not clear. Because they may be of clinical value in some conditions, the extent of perilimbal aneurysms of the conjunctiva was evaluated in 20 normal-tension-glaucoma patients, 20 primary open-angle-glaucoma patients, and 60 controls. The extent of these aneurysms was graded as follows: none (0), faint (1-2), moderate (3-5), and copious (> or = 6). The group frequencies of these categories were 5%, 25%, 10%, and 60% in normal-tensi

[Goldenhar's oculoauricular dysplasia, limbic dermoid and conjunctival dermolipoma].

The paper presents two clinical observations on Goldenhar's syndrome which puts in associations multi-lesions of the first branchial arc. The first observation presents a dermoid of the limb, the second associates a conjunctival dermolipoma at one eye with dermoid of the limb at the other eye. Morphologically, these tumoral formations constitute dysgenetic heterotopias which present at the surface a weak keratinized epidermal, under which it may be identified a dense conjunctival tissue which contains pilosebaceous and sudoripous glands and also a well-represented adipose tissues at the dermol

An Unusual Presentation of Corneal Intraepithelial Neoplasia: A Case Report.

INTRODUCTION: Corneal squamous neoplasia is less common than that of conjunctiva and can cause diagnostic confusion. CASE: A 40-year-old male presented with gradual onset blurring of vision in left eye for 8 weeks. He had received treatment for dry eyes, then for herpetic dendritic keratitis, but without improvement. On slit-lamp examination with diffuse light, apparently the cornea looked clear with some dilated conjunctival vessels nasally. But in the retro-illumination, on the corneal surface, there was a translucent inverted "V" shaped lesion with irregular fimbricated margin. He underwen

The Changing Face of the Cornea in a Case of Juvenile Glaucoma and Subclinical Keratoconus.

We report on a 23-year-old man who presented with bilateral subclinical keratoconus and juvenile glaucoma (JG). With intraocular pressures (IOPs) of 30 and 28 mmHg, both eyes were consecutively operated by adjusted trabeculotomy, leading to a remarkable decrease in IOP to well below the mean for this surgery in JG. Meanwhile, most keratoconus indices clearly progressed in the first 5 months postoperatively, with increases in corneal hysteresis, associated with a remarkable drop in the corneal resistance factor. During the following years, IOP remained low, and all changes (except the increase

Isolated keratinising corneal ocular surface squamous neoplasia with multifocal recurrence.

Corneal ocular surface squamous neoplasia usually begins at the limbus and presents as an avascular translucent sheet over the corneal surface. This case report describes a 67-year-old man with an unusual isolated, keratinous nodular corneal lesion. Slit-lamp examination showed an elevated, avascular, whitish plaque-like lesion at the central cornea surrounded by a normal corneal epithelium, with reduced visual acuity of 20/200 in the right eye. Anterior segment optical coherence tomography displayed a hyper-reflective, thickened epithelium with back shadowing. Histopathology showed keratinisi

Current perspectives in Bietti crystalline dystrophy.

Bietti crystalline dystrophy (BCD) is a rare-inherited disease caused by mutations in the CYP4V2 gene and characterized by the presence of multiple shimmering yellow-white deposits in the posterior pole of the retina in association with atrophy of the retinal pigment epithelium (RPE) and chorioretinal atrophy. The additional presence of glittering dots located at the corneal limbus is also a frequent finding. The CYP4V2 protein belongs to the cytochrome P450 subfamily 4 and is mainly expressed in the retina and the RPE and less expressed in the cornea. The disease has its metabolic origin in t

Stem cell and tissue engineering therapies for ocular regeneration.

The eye is a relatively small but very complex organ. It is responsible for vision. Most of its cells are terminally differentiated, and several pathologies affecting those cells lead to vision loss and eventual blindness. Several years ago, a group of cells, located in the limbus, was identified as having the capacity of self-renewal and later on found to feed the renewal of the corneal epithelial layer. Since then, this niche of stem cells has been studied in order to provide clues that can be valuable for the regeneration of ocular structures. The worldwide shortage of donors, increased ris
