Surgery and Postoperative Care of Patients Undergoing Percutaneous Nephrolithotomy under the Guidance of B-Ultrasound Based on Smart Internet of Things.

Источник пометил публикацию как отозванную

At present, percutaneous nephrolithotomy has become an option for hospitals increasingly to treat calculus. However, due to the certain risk of percutaneous nephrolithotomy, it may lead to postoperative complications. Analyzing the preoperative factors of septic shock after percutaneous nephrolithotomy can provide guidance for reducing the incidence of septic shock after PCNL. This article establishes a logistic regression model based on preoperative factors, uses statistical methods, and uses the professional statistical software SPSS to create a database and analyze the data. The method of c

Pilot study of pancreatoscopic lithotripsy using a 5-fr instrument: selected patients may benefit.

BACKGROUND AND STUDY AIMS: Despite advances in endoscopic procedures and extracorporeal shock wave lithotripsy (ESWL), the removal of impacted pancreatic duct stones demands a certain amount of time. The aim of this study was to determine which patient group might suitable for our single-session treatment. PATIENTS AND METHODS: The study included 17 patients who were symptomatic due to relapsing pancreatitis with pancreatic duct stones. A 5-Fr miniscope was inserted via the papilla to the impacted pancreatic duct stone and pancreatoscopic laser lithotripsy (PSLL) was performed, under direct v

Endoscopic treatment of bile duct stones.

Approximately fifteen per cent of patients who undergo gallstones also have concomitant common bile duct stones. Endoscopic papillosphincterotomy (EPS) is the treatment of choice for common bile duct stones in cholecystectomized patients as well as in patients with gallbladder in situ but at high operative risk. Endoscopic papillosphincterotomy is a highly effective technique and able to remove stones from choledochus in 85-90% of cases. The failure of EPS is mainly related to the size of stones (greater than 15 mm). However, recently introduced sophisticated techniques, such as extraluminal (

Preconceived Impressions Regarding Holmium:YAG Laser Safety in the Urology Operating Theatre.

BACKGROUND: Since its introduction over two decades ago, the surgical laser has served in the lithotripsy of urinary calculi, resection of bladder tumours, bladder neck incisions, and prostate enucleation. Concerns regarding the safe use of holmium lasers have resulted in potentially excessive and overly precautious theatre regulations. We aimed to evaluate the preconceived impressions and practice patterns at a single site surrounding laser use in endourology. METHODS: We designed a three-part online questionnaire that could be accessed using a smart device or computer. This survey was distr

Experts' recommendations in laser use for the treatment of upper tract urothelial carcinoma: a comprehensive guide by the European Section of Uro-Technology (ESUT) and Training Research in Urological Surgery and Technology (T.R.U.S.T.) group.

PURPOSE: To highlight and compare experts' laser settings during endoscopic laser treatment of upper tract urothelial carcinoma (UTUC), to identify measures to reduce complications, and to propose guidance for endourologists. METHODS: Following a focused literature search to identify relevant questions, a survey was sent to laser experts. We asked participants for typical settings during specific scenarios (ureteroscopy (URS), retrograde intrarenal surgery (RIRS), and percutaneous treatment). These settings were compared among the reported laser types to find common settings and limits. Addit

Retrograde ureteropyeloscopic treatment of 2 cm. or greater upper urinary tract and minor Staghorn calculi.

PURPOSE: Upper urinary tract calculi that are too large to treat with extracorporeal shock wave lithotripsy are most commonly cleared with percutaneous endoscopic techniques. In a select group of patients who were poor candidates for percutaneous nephrostolithotomy we used retrograde endoscopic lithotripsy, and define the safety and efficacy of this modality in treating large, noninfectious stone burdens (2 cm. or greater). MATERIALS AND METHODS: A total of 51 patients with 66 large (2 cm. or greater) upper urinary tract stones were chosen for retrograde ureteroscopic surgery. Many of these p

Dystrophic calcification of the prostate after cryotherapy.

We present a previously undocumented complication of dystrophic calcification of the prostate after cryotherapy. An 87-year-old male presented with recurrent lower urinary tract infections and was found to have an obstructing large calcified mass in the right lobe of the prostate. Subsequently, he underwent transurethral resection of the prostate (TURP) and bladder neck with laser lithotripsy to remove the calculus. We propose that chronic inflammation and necrosis of the prostate from cryotherapy resulted in dystrophic calcification of the prostate. As the use of cryotherapy for the treatment

A Usability Comparison of Laser Suction Handpieces for Percutaneous Nephrolithotomy.

INTRODUCTION: The holmium laser has revolutionized the practice of minimally invasive endoscopy for kidney stones. Recently, a novel, rigid handpiece for use in percutaneous nephrolithotomy (PCNL) that couples the holmium laser with suction has been developed. To date, limited data exist regarding the usability and ergonomics of such treatment systems. We thus sought to compare surgeon-rated usability with three different suction laser handpieces in a porcine model. MATERIALS AND METHODS: We performed bilateral reverse PCNL on four female domestic farm pigs. After induction of general anesthe

Laparoscopic lithotripsy and lithotomy of impacted stone at the terminal end of the common bile duct using a laparotomy biliary lithotomy spoon: A case report.

INTRODUCTION AND IMPORTANCE: Endoscopic intervention is considered first-line therapy for common bile duct (CBD) stones, in recent times. However, surgically altered anatomy and consequent inaccessibility of the duodenal papilla necessitate surgery in some patients. CASE PRESENTATION: A 61-year-old woman presented with fever and right subcostal pain. She underwent total gastrectomy and Roux-en-Y reconstruction for gastric ulcer, 4 years prior to presentation. Based on the clinical findings, she was diagnosed with acute obstructive cholangitis secondary to a CBD stone. Endoscopic retrograde bi

Endoscopic removal of fungal balls using an access sheath and bendable suction device during URS: A novel approach.

We report the case of a high-risk patient with candidemia and obstructive uropathy caused by renal fungal balls. Initial management with systemic antifungals and stenting was insufficient. Given the patient's dual antiplatelet therapy, we opted for flexible ureterorenoscopy utilizing a bendable ureteral access sheath with an integrated suction device. This enabled in toto removal of fungal balls and efficient clearance of a concomitant stone using laser lithotripsy. To our knowledge, this is the first report demonstrating in toto removal of renal fungal balls using suction-assisted URS in a hi

Ureteric Stone Management in Crossed-Fused Renal Ectopia With Bilateral Duplex-Collecting System.

Crossed-fused renal ectopia (CFRE) is a rare congenital anomaly where both kidneys are fused on the same side. We report a case of a 52-year-old male patient who presented with central abdominal pain associated with hematuria and dysuria, with a history of left open ureterolithotomy. Abdominal computed tomography (CT) showed an 18-mm left distal ureteric stone and a CFRE with a bilateral duplex-collecting system. Left ureteroscopy and laser lithotripsy with magnetic double-J stent insertion were carried out successfully, and the patient was discharged on the same day in fair general condition.

The bladder-flap ureteral augmentation: An original solution in case of complex distal stricture.

An original surgical solution for complex stenosis of the distal ureter is presented. A young, single-kidney male patient developed a stricture of the pelvic ureter after ureteroscopy and laser lithotripsy. Surgical repair was planned after the failure of conservative management. The ureter was sectioned prevesically and spatulated; a bladder flap with the same dimensions of the ureteral plate was taken from the anterior wall, and used to augment the ureter; finally an omental flap was wrapped around the reconstructed tract. Further radiological and ureteroscopic controls showed a largely pate

Obstructing Nephrolithiasis in an 84-year-old Patient With a Diaphragmatic Herniated Ureter.

Diaphragmatic herniations of the ureter are a rare occurrence overall, with fewer than 10 cases reported. Obstructing nephrolithiasis in the setting of diaphragmatic ureteral herniation has sparsely been documented. Management options to consider include ureteroscopy and laser lithotripsy to alleviate obstruction in herniated ureters. The abnormal path of the ureter and surrounding structures pose unique risks that require careful attention and a high level of operator skill. We present imaging of an 84-year-old woman incidentally found to have right ureteral herniation through the diaphragm w

Troubleshooting for Ureteroscopy Complicated by Unexpected Guidewire Looping and Entrapment.

This report presents a bizarre and unusual case of looping and entrapment of a hydrophilic guidewire in the ureter of a 65-year-old woman with a stone in the lower ureter at the time of ureteroscopy and laser lithotripsy. We describe endourology maneuvers to remove the retained guidewire. Looping and entrapment of guidewire is a rare complication that urologists may face when they do not follow the basic principles of guidewire insertion. In that case, successful resolution is based both on surgeon's inspiration to apply the appropriate endourology maneuvers and availability of a variety of en

DIFFERENT APPROACHES TO MANAGING UROLITHIASIS IN KIDNEY TRANSPLANT PATIENTS - A CASE REPORT.

Urolithiasis is a rare urologic complication after kidney transplantation, and its diagnosis and treatment can be challenging for clinicians. In our 52-year-old male patient, graft hydronephrosis was found six months after transplantation. The patient had recurrent urinary tract infections followed by macrohematuria and an increase in creatinine levels. Computerized tomography revealed a 13-mm diameter stone in the ureter of the transplanted kidney as the cause of obstruction. Percutaneous nephrostomy was placed in the graft to solve the obstruction. Initial endoscopic treatment with a retrogr

Clearance of Lower Renal Calyceal Residual Fragments Following Lithotripsy in a Patient With Spinal Cord Injury: A Case Report.

The clearance of residual fragments after lithotripsy is a problem of clinical relevance. Residual stone fragments can serve as nucleation centers for the deposition of calcified material, leading to recurrent formation of calculi. This case report presents a 63-year-old male with paraplegia of lower limbs who, through an empirical observation, managed to develop a protocol that resulted in clearance of residual calcified fragments of a lithotripsy performed 267 days ago. Preoperative imaging revealed the presence of kidney stones localized in the lower left renal calyces. The patient underwen

Is flexible navigable suction ureteral access sheath (FANS) safer and more efficient than conventional sheaths? Italian multicentric experience.

BACKGROUND AND OBJECTIVE: The flexible and navigable suction ureteral access sheath (FANS) with continuous flow lithotripsy may improve stone-free rates (SFR) in ureterorenolithotripsy procedures, reducing operative time and the need for fragment extraction. This study aimed to compare the safety and efficacy of FANS versus conventional sheaths in patients undergoing retrograde intrarenal surgery (RIRS) for renal stones. METHODS: From January to September 2024, patients undergoing RIRS with high-power pulsed lasers (60 W Ho:YAG or 100 W pTm:YAG) were randomized to receive either a conventiona
