Primary Mesenteric Carcinoid Tumor Presenting with Carcinoid Syndrome.

Neuroendocrine neoplasms (NENs) are a diverse group of tumors arising throughout the body with a common origin from neuroendocrine cells. Well-differentiated NENs, also known as neuroendocrine tumors (NETs), are generally indolent and are often found incidentally, while poorly differentiated tumors are more aggressive. Carcinoid tumors are NETs arising from the gastrointestinal tract and less commonly from the lungs, thymus, and kidneys. NETs in the mesentery arise from metastasis from primary tumor, and carcinoid syndrome in this setting results from concomitant metastasis to the liver. Prima

Hepatic tumor and tumor-like lesions in childhood.

Tumors of the liver are rare in infancy and childhood. Some are peculiar to the pediatric age, e.g., hepatoblastoma, infantile hemangioendothelioma, mesenchymal hamartoma, embryonal rhabdomyosarcoma. This review is based upon personal experience with a series of 32 cases. On the basis of the histological features it is proposed that focal modular hyperplasia (FNH) and mesenchymal hamartoma be considered as tumor-like lesions rather than true neoplasms. A few benign epithelial lesions (FNH, adenoma) were associated with inborn error of metabolism. In half of the patients, hepatocellular carcino

Metastatic Oncocytic Renal Neoplasm Presenting as Gastrointestinal Bleeding.

Renal cancer (RC) is known for its diverse clinical presentations and unpredictable behavior. While it often metastasizes to common sites such as the lymph nodes, lungs, bones, and liver, its potential to metastasize to the gastrointestinal tract (GIT) is relatively rare. We report an unusual case of an 87-year-old male patient with a history of metastatic oncocytic renal neoplasm who presented with intermittent rectal bleeding. Colonoscopy revealed a bleeding mass in the sigmoid colon. Biopsy confirmed metastatic renal neoplasm, consistent with prior pathology. Immunohistochemistry was positi

Imaging and Histopathologic Features of Incidentally Detected Hepatic Perivascular Epithelioid Cell Tumors: A Case Report.

Hepatic perivascular epithelioid cell tumor (PEComa) are rare mesenchymal neoplasms. The limited number of reported cases, along with nonspecific clinical symptoms and laboratory findings, often complicates accurate preoperative diagnosis. Here, we present a case of hepatic PEComa incidentally detected in a 64-year-old woman without underlying liver disease. We describe the clinical presentation, laboratory findings, CT and MRI features, and histopathological characteristics confirmed after surgical resection that may facilitate the diagnosis of hepatic PEComa.

Fascioliasis: A challenging differential diagnosis for radiologists.

We report a case of a 39 year old male who presented with nausea and right upper quadrant pain. Marked eosinophilia and a hypoechoic liver lesion on ultrasound were identified. The differential diagnosis included neoplasms, infectious diseases and hepatic abscess. Indirect hemagglutination test using purified adult Fasciola hepatica f1Ag confirmed serologic diagnosis of fascioliasis. Radiologists should keep in mind the importance of correlating imaging, clinical and laboratory findings in order to reach the correct diagnosis.

Late-developing tongue adenoid cystic carcinoma after pulmonary metastasectomy: a case report.

Adenoid cystic carcinoma (ACC) is a relatively rare epithelial tumor of the salivary glands that accounts for approximately 5 to 10% of all salivary gland neoplasms. The typical clinical and pathological findings of this tumor include slow indolent growth, common local recurrence, and late distant metastasis to lung, brain, bone, liver, thyroid, and spleen. We report a 52-year-old female patient who presented a tongue ACC, 27 months after successful pulmonary ACC resection.

Fulminant course of a microinvasive vulvar carcinoma in an immunosuppressed woman.

Iatrogenic immune suppression following renal transplantation is frequently associated with certain neoplasms, including vulvar carcinoma. We describe a patient with a vulvar carcinoma less than 1 mm depth of invasion and less than 3 mm superficial spread 12 years after renal transplantation. A simple vulvectomy was performed but 4 months later disease recurred in the inguinal nodes. The patient died 20 months later with progressive disease in the retroperitoneum and liver metastases.

[Current diagnostic-therapeutic trends in carcinoids of the papilla of Vater].

Diagnosis and treatment of carcinoid tumor of the ampulla of Vater: case report. The carcinoid tumour of the ampulla of Vater is extremely rare. The authors report one case. Therefore, they dwell upon the problem connected with classifications, symptomatology and diagnosis of these neoplasms which present difficulty to diagnose preoperatively. At last, they reaffirm the primary role of pancreaticoduodenectomy with lymphadenectomy and, if it is possible, the necessity to extirpate the liver metastases.

Use of radio-frequency ablation for the palliative treatment of sacral chordoma.

Chordomas are the most common sacrococcygeal tumors in adults. They have a high recurrence rate, are locally aggressive, and are resistant to radiation therapy. Radio frequency is a relatively new therapeutic technique, used primarily in the treatment of liver tumors; however, its application has widened to include other neoplasms. We report its use in the palliative treatment of chordomas in two patients for debulking and control of pain, with good results and low morbidity.

Pancreatic metastases from renal cell carcinoma in von Hippel-Lindau disease.

von Hippel-Lindau (VHL) disease is an unusual inherited chromosomal dominant disease characterized by CNS hemangioblastomas and visceral neoplasms. Multiple abnormalities may involve the abdomen including pathology in the liver, pancreas, kidneys, and adrenal glands. The present case report describes a patient with renal cell carcinoma in von Hippel-Lindau disease. What was most unusual about this case was that the patient also developed pancreatic masses that were metastatic disease from renal cell carcinoma.

Radiofrequency ablation for cholangiocarcinoma - Safety and effectiveness in clinical practice.

Cholangiocarcinoma comprises a heterogeneous group of biliary tract neoplasms, and represents the second most common neoplasm of the liver (15 %) after hepatocellular carcinoma. Diagnosis is achieved through various imaging techniques and is generally made at an advanced stage, often rendering the tumor unresectable. Bipolar biliary radiofrequency ablation is a palliative treatment that, when applied endoscopically, is a safe and effective technique. It is indicated for inoperable cholangiocarcinomas presenting with jaundice, to recanalize the biliary tract and restore patency of obstructed me

[The prevalence of diffuse and focal lesions of the hepatic parenchyma in a nonselected, resident population. A prospective echographic study].

A group of 2978 randomly chosen residents from a rural area of Veneto (Italian region), aged 18-69 years, was assumed as a study population. They underwent US examination of the liver, routine liver function tests, and--when necessary--other imaging techniques and liver biopsy. Liver US exam detected diffuse lesions in 26% of cases (mostly bright liver pattern), and focal lesions in 3%; US findings were normal in 66% of patients and questionable or inconclusive in 5%. As far as focal lesions were concerned, the final diagnosis was angioma (24 cases), cyst (31), neoplasm (5). When bright liver

Pathological, molecular, and clinical characteristics of cholangiocarcinoma: A comprehensive review.

Cholangiocarcinomas are a heterogeneous group of highly aggressive cancers that may arise anywhere within the biliary tree. There is a wide geographical variation with regards to its incidence, and risk-factor associations which may include liver fluke infection, primary sclerosing cholangitis, and hepatolithiasis amongst others. These tumours are classified into intrahepatic, perihilar and distal based on their anatomical location. Morphologically, intrahepatic cholangiocarcinomas are further sub-classified into small and large duct variants. Perihilar and distal cholangiocarcinomas are usual

Pigmented well-differentiated hepatocellular neoplasm with beta-catenin mutation.

According to the most recent WHO classification of hepatocellular adenomas, a small percentage of inflammatory hepatocellular adenomas presents with mutation in the beta-catenin gene and are at higher risk of malignant transformation. It has been recognized that adenoma-like hepatocellular neoplasms with focal atypia, or in unusual clinical context present with similar cytogenetic and immunohistochemistry characteristics to well-differentiated hepatocellular carcinomas. We report a case of a well-differentiated hepatocellular neoplasm with Dubin-Johnson-like pigment displaying histological fea

Clear cell papillary carcinoma of the liver: an unusual variant of peripheral cholangiocarcinoma.

Cholangiocarcinomas may be extrahepatic or intrahepatic; the latter are further divided into hilar and peripheral types. Peripheral cholangiocarcinomas often resemble adenocarcinomas arising in other organs. Although clear cell changes may occur in hepatocellular carcinoma and extrahepatic cholangiocarcinoma, peripheral cholangiocarcinomas with clear cell change are rare. In such cases, an extrahepatic primary carcinoma must be excluded. We present a patient with a large, clear cell papillary carcinoma in the liver. Extensive workup of the patient for other possible primary sites including kid

Transcription factor GATA-4 is abundantly expressed in childhood but not in adult liver tumors.

OBJECTIVES: Transcription factor GATA-4 is expressed in early fetal liver and essential for organogenesis. It is also implicated in carcinogenesis in several endoderm-derived organs. Hepatoblastoma (HB), the most common malignant pediatric liver tumor, has features of fetal liver including extramedullary hematopoiesis. We investigated the expression of GATA-4 and its purported target gene erythropoietin (Epo) in liver tumors and the role of GATA-4 in HB pathogenesis. PATIENTS AND METHODS: Immunohistochemistry, Western blotting, and reverse transcription-polymerase chain reaction were used for

Breast metastases of eccrine porocarcinoma.

Eccrine porocarcinoma is a rare skin adnexal malignant neoplasm that may arise from a pre-existing benign eccrine poroma or without a predisposing factor. It is a highly invasive neoplasm and has a strong metastatic potential. The most frequently affected organs are the lymph nodes and rarely solid organs such as the liver, lungs and breast. We report a case of a woman with a history of surgically treated eccrine porocarcinoma that a year later presented with multiple lesions in both breasts and axillary lymphadenopathies. After a detailed imaging investigation, the diagnosis of metastatic les

Single-nucleus multiomic analysis of Beckwith-Wiedemann syndrome liver reveals PPARA signaling enrichment and metabolic dysfunction.

Beckwith-Wiedemann Syndrome (BWS) is an epigenetic overgrowth syndrome caused by methylation changes in the human 11p15 chromosomal locus. Patients with BWS exhibit tissue overgrowth, as well as an increased risk of childhood neoplasms in the liver and kidney. To understand the impact of these 11p15 changes, specifically in the liver, we performed single-nucleus RNA sequencing (snRNA-seq) and single-nucleus assay for transposase-accessible chromatin with sequencing (snATAC-seq) to generate paired, cell-type-specific transcriptional and chromatin accessibility profiles of both BWS-liver and non

Bile Duct Targeting or Preservation: Contrasting Liver Histology in Langerhans Cell Histiocytosis and Disseminated Juvenile Xanthogranuloma.

Liver involvement by histiocytic and dendritic cell neoplasms signals high-risk disease, often necessitating closer monitoring and aggressive management. Severe cases may progress to liver failure, requiring transplantation. Liver involvement occurs in about one-third of patients with systemic juvenile xanthogranuloma (JXG) and 20% to 60% of pediatric patients with Langerhans cell histiocytosis (LCH), particularly in multiorgan disease. Tyrosine kinase inhibitors show promise in LCH treatment, but optimal timing for treatment cessation remains uncertain. We present 2 pediatric cases, 1 with LC

Unresectable isolated hepatic metastases from solid pseudopapillary neoplasm of the pancreas: a case report of chemosaturation with high-dose melphalan.

BACKGROUND/OBJECTIVES: Solid pseudopapillary neoplasms of the pancreas (SPN) are rare tumors. For patients with unresectable liver metastases of SPN, no standard treatment has been defined so far. Here we report a case of a 40-year-old woman with SPN and metastases confirmed to the liver, and disease progression in the liver after primary tumor resection and chemotherapy with gemcitabine and cisplatin. METHODS: Chemosaturation with percutaneous hepatic perfusions is a minimally invasive, repeatable regional therapy which delivers chemotherapy directly to the liver while limiting systemic toxi
