Expansive open-door laminoplasty versus laminectomy and instrumented fusion for cases with cervical ossification of the posterior longitudinal ligament and straight lordosis.

PURPOSE: To identify whether expansive open-door laminoplasty (Lam) is more appropriate than laminectomy and instrumented fusion (LIF) for cases with ossification of the posterior longitudinal ligament (OPLL) and straight cervical lordosis. METHODS: A total of 67 cases were included and divided into Group Lam (n = 32) and Group LIF (n = 35), and the mean follow-up periods were 38 and 42 months, respectively. The cervical lordosis was elevated by C2-7 Cobb angle and cervical sagittal balance by C2-C7 sagittal vertical axis (SVA). Japanese Orthopedic Association (JOA), neurological recovery rat

Knee mechanics during landing in anterior cruciate ligament patients: A longitudinal study from pre- to 12 months post-reconstruction.

BACKGROUND: Patients with a history of anterior cruciate ligament rupture are at elevated risk of developing knee osteoarthritis. Altered knee kinematics and kinetics during functional activities have been viewed as risk factors for cartilage breakdown and, therefore, one of the primary goals of anterior cruciate ligament reconstruction is to restore knee joint function. METHODS: Patients' (n=18) knee mechanics while performing a single leg hop for distance were calculated for both legs using a soft-tissue artifact optimized rigid lower-body model at the pre-reconstruction state and six and t

Association between BH3 interacting domain death agonist (BID) gene polymorphism and ossification of the posterior longitudinal ligament in Korean population.

The purpose of this study was to investigate single nucleotide polymorphisms (SNPs) of the BH3 interacting domain death agonist (BID) gene as a risk factor in Korean patients with ossification of the posterior longitudinal ligament (OPLL). To investigate the genetic association, two coding SNPs (rs8190315, Ser10Gly; rs2072392, Asp60Asp) of BID were genotyped in 157 OPLL patients and 209 control subjects. SNPStats, SNPAnalyzer Pro, Helixtree, and Haploview 4.2 programs were used for association analysis. Multiple logistic regression models (codominant, dominant, and recessive) were calculated f

Influence of spinal cord compression and traumatic force on the severity of cervical spinal cord injury associated with ossification of the posterior longitudinal ligament.

STUDY DESIGN: Retrospective review. OBJECTIVE: To evaluate the influence of static compression factors and dynamic factors based on the various degrees of traumatic force on the cervical spinal cord injury (SCI) in patients with ossification of the posterior longitudinal ligament. SUMMARY OF BACKGROUND DATA: Spinal cord disorder occurs as a result of various factors, including static factors and traumatic force. Discussions about the severity of paralysis resulting from SCI must therefore focus on dynamic factors based on the traumatic force as well as on static compression factors. However,

Association of transforming growth factor-beta 1 gene polymorphism with genetic susceptibility to ossification of the posterior longitudinal ligament in Korean patients.

Ossification of the posterior longitudinal ligaments (OPLL) has been considered to be associated with abnormalities of bone metabolism, and transforming growth factor-β1 (TGF-β1) has been demonstrated to affect the bone remodeling process. We investigated two SNPs of the TGF-β1 promoter (-509C>T; rs1800469) and exon 1 (869T>C; rs1982073) in 298 Koreans (98 patients with OPLL and 200 control subjects). The promoter SNP -509C>T was determined by PCR and RFLP, and the TaqMan probe assay was used to determine 869T>C polymorphism genotypes. The subjects were divided into OPLL continuous group (cont…

Imaging longitudinal changes in articular cartilage and bone following doxycycline treatment in a rabbit anterior cruciate ligament transection model of osteoarthritis.

OBJECTIVE: The development of osteoarthritis following traumatic anterior cruciate ligament (ACL) injury is well established. However, few reliable indicators of early osteoarthritic changes have been established, which has limited the development of effective therapies. T(1ρ) and T(2) mapping techniques have the ability to provide highly accurate and quantitative measurements of articular cartilage degeneration in vivo. Relating these cartilaginous changes to high-resolution bone-densitometric evaluations of the late-stage osteoarthritic bone is crucial in elucidating the mechanisms of develo

A new surgical technique: open-window corpectomy in the treatment of ossification of the posterior longitudinal ligament and advanced cervical spondylosis: technical note.

OBJECTIVE: To achieve satisfactory cervical spinal cord decompression with minimal removal of bone. METHODS: The open-window corpectomy technique is designed to remove a minimal amount of bone and achieve satisfactory decompression. With the use of a high-speed drill under a surgical microscope, only the dorsal surface of the corpus is removed after appropriate microdiscectomies. This leaves the anterior and the lateral portions of the vertebral corpus intact. RESULTS: In a 15-month period, a total of 11 patients were treated with this technique. Five patients improved, and the remaining six

O-arm-navigated open-door laminoplasty to reduce technical complications in multilevel cervical ossification of the posterior longitudinal ligament: operative video.

Open-door laminoplasty is an established posterior decompression technique for multilevel cervical myelopathy. However, the procedure is technically demanding and may result in complications such as hinge fracture, facet joint violation, and improper gutter placement. The authors present an operative video demonstrating O-arm-based intraoperative navigation during open-door laminoplasty in a 75-year-old man with multilevel ossification of the posterior longitudinal ligament (OPLL) and severe cervical canal stenosis. Navigation allowed precise localization of the medial facet, controlled hinge

Comparative efficacy of contralateral versus ipsilateral open-door laminoplasty for cervical ossification of the posterior longitudinal ligament: a systematic review and meta-analysis.

BACKGROUND: Unilateral open-door laminoplasty (UODL) is an established posterior surgical intervention for cervical ossification of the posterior longitudinal ligament (OPLL). However, a conclusive consensus is lacking regarding the comparative efficacy and safety of performing the open-door procedure on the contralateral versus the ipsilateral side relative to the predominant compression or symptoms. METHODS: A systematic literature search adhering to PRISMA guidelines was conducted across multiple databases (PubMed, Web of Science, Cochrane Library, Embase, CNKI, Wanfang) from inception to

Performance Evaluation of YOLO-Based Models for Automated Detection of Osteophytes and Ossification of the Posterior Longitudinal Ligament (OPLL) in Sagittal Cervical CT Images.

Osteophytes and ossification of the posterior longitudinal ligament (OPLL) are major contributors to degenerative cervical myelopathy (DCM), a leading cause of spinal cord dysfunction in adults. Accurate assessment of these lesions is essential for surgical planning, particularly in patients considered for artificial disc replacement (ADR), where the extent of ossification critically influences surgical eligibility. This study evaluated the performance of YOLO (You Only Look Once)-based deep learning models for automated detection of osteophytes and OPLL in cervical computed tomography (CT) im
