Neuroimmune Clearance and EEG Biomarkers: A Unified Model of ASD and Dyslexia.

Recent advances in neuroimmunology and cerebrovascular biology have highlighted the important roles of microglial synaptic pruning and the brain's meningeal lymphatic system in shaping neural circuits during development. Disruptions in one or both of these systems have been reported in neurodevelopmental conditions such as autism spectrum disorder (ASD) and developmental dyslexia. This review synthesizes existing evidence suggesting that impaired meningeal lymphatic clearance may be associated with sustained neuroinflammatory states, which in turn could alter microglial homeostasis and contrib

Case Report: Pembrolizumab unmasks a capillary leak syndrome with chylothorax in early TNBC.

Pembrolizumab has become standard of care in early-stage triple-negative breast cancer (TNBC), but its expanding use may induce rare adverse events involving vascular and lymphatic endothelium. Capillary leak syndrome (CLS) remains an exceptionally rare toxicity of PD-1 blockade, scarcely reported in breast cancer, especially in the adjuvant setting. We report the case of a 40-year-old female with early-stage TNBC who presented with progressive dyspnea, diffuse subcutaneous edema and bilateral pleural and peritoneal effusions after two cycles of adjuvant pembrolizumab. Imaging confirmed polyse

Immediate-delayed lymphatic reconstruction after axillary lymph nodes dissection for locally advanced breast cancer-related lymphedema prevention: Report of two cases.

Approximately 60%-70% of breast cancer patients in Indonesia are diagnosed in the locally advanced stage. The stage carries a higher risk of lymph node metastasis which increases susceptibility to lymph obstruction. Hence, breast cancer-related lymphedema (BCRL) could present before axillary lymph node dissection (ALND). The purpose of this case report is to describe immediate-delayed lymphatic reconstructions with lymphaticovenous anastomosis in two subclinical lymphedema cases that present before ALND. There were 51 and 58 years old breast cancer patients with stage IIIC and IIIB, respective

Lymphangioleiomyomatosis: what do we know and what are we looking for?

Lymphangioleiomyomatosis (LAM) is a rare disease characterised by proliferation of abnormal smooth muscle-like cells (LAM cells) leading to progressive cystic destruction of the lung, lymphatic abnormalities and abdominal tumours. It affects predominantly females and can occur sporadically or in patients with tuberous sclerosis complex. This review describes the recent progress in our understanding of the molecular pathogenesis of the disease and LAM cell biology. It also summarises current therapeutic approaches and the most promising areas of research for future therapeutic strategies.

Widespread Reactive Lymphatic and Splenic FDG Avidity After mRNA COVID Vaccination.

A 75-year-old woman with history of metastatic lung adenocarcinoma in remission develops new widespread FDG-avid lymphadenopathy in the neck, chest, abdomen, and pelvis on surveillance PET/CT, as well as intense FDG uptake in the spleen, without evidence of local recurrence. Short-term follow-up PET demonstrates near-complete resolution of FDG-avid lymphatic and splenic FDG avidity without interval change in management. Further history reveals that the patient received her fifth dose of COVID mRNA vaccine 6 days before the abnormal PET. Although unilateral axillary adenopathy after COVID vacci

Rare case of cystic hygroma in the epidural space resulting in multilevel spinal cord compression.

Lymphangioma, or cystic hygroma, involving the epidural space and spinal soft tissue, is a rare benign lesion consisting of an abnormal collection of lymphatic tissue isolated from the normal lymphatic system. This case report is the most extensive case of cystic hygroma involving the spine reported in the literature. A 23-year-old man with a history of cystic hygromas of the neck and thorax presented with bilateral upper and lower extremity weakness that progressively worsened over 3 months. A left hemilaminectomy from C4 to T5 with endoscopic exploration and cyst drainage was performed. At l

Capillary malformations in unusual territory: Klippel-Trénaunay syndrome with penoscrotal involvement.

Klippel-Trénaunay syndrome (KTS) is a rare congenital vascular disorder that typically presents with capillary and venous malformations, limb hypertrophy and, in some cases, lymphatic abnormalities. We report a 3-year-old boy with extensive vascular malformations affecting the left lower limb, predominantly involving the penoscrotal region - an uncommon presentation without systemic involvement. This case illustrates the diagnostic complexity of genitourinary KTS and emphasizes the need for vigilant monitoring, multidisciplinary input and tailored management to reduce complications and improve…

Increased NCAM expression and vascular development in trisomy 16 mouse embryos: relationship with nuchal translucency.

Increased nuchal translucency in the human fetus is associated with chromosomal abnormalities, enlarged jugular lymphatic sacs, cardiac defects and changed flow through the ductus venosus. The developmental background of this nuchal edema in relation to the associated anomalies remains elusive. We studied the morphologic correlation between neurogenesis and vasculogenesis in neck, heart, and ductus venosus region of wild type and trisomy 16 mice embryos (E10- E18), using an antibody against Neural Cell Adhesion Molecule (NCAM). Trisomy 16 mice are a model for the above described human phenotyp

Peripheral Lymphatic Shunts for Protein-Losing Enteropathy in a Child with THSD1 Mutation: A Case Report.

Protein-losing enteropathy (PLE) is a rare disorder characterized by abnormal protein loss through the gastrointestinal tract, often leading to hypoalbuminemia and malnutrition. This case report describes the successful surgical management of chronic lower limb lymphedema and PLE in a 7-year-old male with a thrombospondin type 1 domain-containing 1 (THSD1) mutation, unresponsive to conventional therapies. Despite aggressive nutritional support, including albumin infusions, the patient experienced persistent hypoalbuminemia and ongoing protein loss. Imaging revealed significant lymphatic dysfun

Case Report: Surgical management of primary lymphedema with a novel PROX1 mutation involving upper and lower limbs.

BACKGROUND: Primary lymphedema (PL) is a chronic condition characterized by abnormal swelling of tissues due to impaired lymphatic drainage, leading to increased deposition of adipose tissue and fibrosis. Although several pathogenic variants in genes associated with PL have been identified, a significant number of cases remain unexplained, suggesting the possibility of undiscovered genetic links. CASE PRESENTATION: This report describes a novel heterozygous mutation in the PROX1 gene (c.1019C>G, p.S340C) identified in a 59-year-old male patient with PL affecting both upper and lower extremiti

Unexplained Giant Genital Enlargement: Is It Due to Inverse Psoriasis?

A healthy 54-year-old man previously presented to vascular surgeons with a 4-year history of swelling of the penis and scrotum was scheduled for ultrasound evaluation in the angiology office in our department. At presentation, there was a giant enlargement of the penis and scrotum, without swelling of the legs. Ultrasound evaluation was negative for vascular abnormalities. A diagnosis of chronic lymphatic disease was suspected; thus, a lymphoscintigraphy was performed. This test was normal showing, a good visualization of major lymphatics. The patients had a history of psoriasis with a documen

Splenic lymphangioma.

Splenic lymphangioma is a rare malformation of the splenic lymphatic channels, mostly seen in children. It is characterized by the presence of cysts, resulting from increases in the size and number of thin-walled lymphatic vessels that are abnormally interconnected and dilated. The condition may be restricted to the spleen, but in most cases it involves multiple organs (systemic lymphangiomatosis). The clinical picture is variable; small lesions are often incidentally detected through imaging studies, while larger lesions can result in compression of organs, causing pain or rupture even after

A case of stasis papillomatosis associated with psoriasis vulgaris.

A markedly obese, 41-year-old Japanese man who had suffered from psoriasis vulgaris for several years visited us with elephantiasis-like swelling of his lower legs of three months' duration. His right lower leg showed marked papillomatosis with thick scales, and the left lower leg was eroded and papillomatous. Although direct lymphography of his lower extremities showed no abnormality, indirect lymphography revealed local lymphatic damage in the involved skin. Histological examination showed hyperkeratosis, marked papillomatosis, proliferation of capillaries in the upper dermis, and lymphectas

Sensitivity and Specificity of the Stemmer Sign for Lymphedema: A Clinical Lymphoscintigraphic Study.

BACKGROUND: The Stemmer sign is a physical examination finding used to diagnose lymphedema. If the examiner cannot pinch the skin of the dorsum of the foot or hand then this positive finding is associated with lymphedema. The purpose of the study was to determine the accuracy of the Stemmer sign to predict lymphedema. METHODS: All patients referred to our Lymphedema Program between 2016 and 2018 were tested for the Stemmer sign and underwent lymphoscintigraphy to define the patient's lymphatic function. Patient age, lymphedema type (primary and secondary), disease location (arm and leg), lymp

Oncogenic hypophosphataemic osteomalacia: biomarker roles of fibroblast growth factor 23, 1,25-dihydroxyvitamin D3 and lymphatic vessel endothelial hyaluronan receptor 1.

Oncogenic osteomalacia (OOM) is characterised by tumour production of fibroblast growth factor 23 (FGF23) that results in hypophosphataemia and renal phosphate wasting, reduced 1,25-dihydroxyvitamin D3 (1,25(OH)2D3) synthesis and osteomalacia. Here, we demonstrate the roles of serum FGF23 and 1,25(OH)2D3, together with the lymphatic vessel endothelial hyaluronan receptor 1 (LYVE-1), as biomarkers for OOM. A previously well 52-year-old man presented with a 2-year history of generalised musculoskeletal pain and proximal myopathy. He had hypophosphataemia, elevated serum alkaline phosphatase acti

Lymph node metastasis from ductal carcinoma in situ with microinvasion.

BACKGROUND: Widespread use of mammography has increased the detection of ductal carcinoma in situ with microinvasion (DCISM) in pathology specimens. Currently there is disagreement regarding the incidence of axillary metastasis from DCISM. The controversy centers on whether complete lymphadenectomy is indicated for axillary staging, given its morbidity and the reportedly minimal rate of axillary involvement in these patients. Intraoperative lymphatic mapping and sentinel lymphadenectomy (SLND) may obviate complete axillary lymph node dissection in selected breast carcinoma patients. In intraop

A systematic review of magnetic resonance lymphography for the evaluation of peripheral lymphedema.

OBJECTIVE: Visualization of the lymphatic system is necessary for both early diagnosis and associated treatments. A promising imaging modality is magnetic resonance lymphography (MRL). The aim of this review was to summarize different MRL protocols, to assess the clinical value in patients with peripheral lymphedema, and to define minimal requirements necessary for visualization of lymphatics. METHODS: A systematic literature search was conducted in PubMed, Embase, and the Cochrane Library in December 2018. Studies performing MRL in patients with peripheral lymphedema or healthy participants

[Role of imaging techniques in the evaluation of chyluria].

Lymphoscintigraphy is a first-line imaging technique for the assessment of abnormal lymphatic drainage in chyluria. It can provide information about the site of origin of chyluria, and may be helpful in selecting patients for surgery and assessing treatment efficacy. We report the case of a 59-year-old male with chyluria correctly diagnosed with lymphoscintigraphy.

CHFR aberrant methylation involves a subset of human lung adenocarcinoma associated with poor clinical outcomes.

Excluding epidermal growth factor receptor (EGFR) mutation, v-Ki-ras2/Kirsten rat sarcoma viral oncogene homolog (KRAS) mutation, and echinoderm microtubule-associated protein-like 4-anaplastic lymphoma kinase (EML4-ALK) fusion, the genetic alterations involved in lung adenocarcinogenesis, especially those linked to poor clinical outcomes, are still unknown. In this study, we analyzed abnormal checkpoint gene with forkhead-associated domain and ring finger (CHFR) methylation along with the above 3 mutations in 165 lung adenocarcinomas, evaluated the spectrum of each molecular abnormality, and
