Comparison of clinical characteristics of opium-induced and independent major depressive disorder.

Psychiatric disorders and among them depression are common in substance dependent patients. The aim of this study is to compare the clinical characteristics of those that appear to have substance-induced depression and those that have independent major depression. One-hundred eighty-four independent and 187 opium-induced (OID) depressed male patients that met the DSM-IV criteria for major depressive disorder were randomly selected. Standard demographic data, including age, marital, employment and education status, were collected. The primary measure of depressive signs and symptoms was Hamilto…

Two-year prospective study of major depressive disorder in HIV-infected men.

OBJECTIVE: The risks and factors contributing to major depressive episodes in HIV infection remain unclear. This 2-year prospective study compared cumulative rates and predictors of a major depressive episode in HIV-infected (HIV+) men (N=297) and uninfected (HIV-) risk-group controls (N=90). METHODS: By design participants at entry were without current major depression, substance dependence or major anxiety disorder. Standardized neuromedical, neuropsychological, neuroimaging, life events, and psychiatric assessments (Structured Clinical Interview for DSM III-R) were conducted semi-annually

Diagnosing major depressive disorder: II: is there justification for compound symptom criteria?

The DSM-IV symptom inclusion criteria for the diagnosis of major depressive disorder (MDD) are constructed in three ways: single symptom criteria, compound criteria encompassing opposite variants of the same disturbance, and compound criteria encompassing related problems. In the present report from the Rhode Island Methods to Improve Diagnostic Assessment and Services project, we tested the following three hypotheses: (1) the components of compound-opposite criteria rarely occur simultaneously, (2) the components of the compound-related criteria frequently occur simultaneously, and (3) the co…

Major depressive disorder with psychotic features in an aviator after head trauma.

We present a case in which a Naval aviator suffered an unusual sequence of neuropsychiatric symptoms after head trauma. He demonstrated subtle deficits on several measures 1 mo after the trauma. Repeat testing at 8 mo showed apparent recovery, and the patient was cleared to return to flight status. Unbeknownst to medical staff, the patient was still experiencing difficulties. He was found in a severely debilitated state and exhibiting psychotic features 12 mo after his initial trauma. The patient was hospitalized in a psychiatric ward, and with prolonged inpatient and outpatient treatment, he

Increased prevalence of negative life events in subtypes of major depressive disorder.

The prevalence of negative life events is known to be increased among patients with depression. Little data exist on the specific subtypes of depression that are related to negative life events. Our study aimed to address this issue. We compare 115 patients with major depressive disorder (MDD) to 60 normal controls. MDD patients reported experiencing one (P = .0001) or two (P = .01) negative life events more frequently than controls. Patients reported marital, other personal problems, and medical events significantly more often than controls (P < .01). Patients did not report more positive lif…

Adinazolam, diazepam, imipramine, and placebo in major depressive disorder: a controlled study.

In summary, the clinical results of this double-blind study clearly show that imipramine, as expected, demonstrated significant antidepressive properties in outpatients suffering from major depressive disorders. In contrast, adinazolam showed rather mild and weak antidepressive properties, and in no measures did its response differ significantly from that of diazepam. These findings are quite in contrast to those obtained by the authors in an earlier study with alprazolam (Rickels et al., 1987) in which alprazolam and imipramine produced rather similar results and both were significantly bette…

Major depression and anxiety disorders associated with Norplant.

BACKGROUND: Norplant is a relatively new long-acting subdermal implant system that is used widely for contraception. In a recent report, Wagner and Berenson described two cases of Norplant-associated major depression and panic disorder. These were the first reports of severe psychiatric disorders linked to the use of Norplant. METHOD: Five cases are described of women who developed major depression, two of whom also developed obsessive-compulsive disorder and one of whom also developed agoraphobia, while using the Norplant system. RESULTS: These women who had no prior psychiatric history dev

Psychiatric disorders in relatives of probands with panic disorder and/or major depression.

BACKGROUND: Panic disorder and major depression (MDD) are both highly familial disorders that co-occur in individuals but do not cosegregate in families. Evidence concerning their familial aggregation with other psychiatric disorders, including phobias, substance abuse, and antisocial personality, has been contradictory. In part, the contradictory findings may be due to failure to account for the effects of proband comorbidity on risks in relatives. METHODS: A family study of 1047 adult first-degree relatives of 193 probands in four diagnostic groups (panic disorder without MDD, panic disorde

Screening for adjustment disorders and major depressive disorders in cancer in-patients.

The Hospital Anxiety and Depression Scale (HADS), a four-point, 14-item questionnaire, was tested as a screening method for adjustment disorders and major depressive disorders in a sample of 210 cancer in-patients. A receiver operating characteristic (ROC) analysis was performed, giving the relationship between the true positive rate (sensitivity) and the false positive rate (1-specificity). This makes it possible to choose an optimal cut-off point that takes into account the costs and benefits of treatment of psychological distress. For screening for major depressive disorders only, a cut-off…

Insula network connectivity mediates the association between childhood maltreatment and depressive symptoms in major depressive disorder patients.

Childhood maltreatment (CM) is a major risk factor for developing the major depressive disorder (MDD), however, the neurobiological mechanism linking CM and MDD remains unclear. We recruited 34 healthy controls (HCs) and 44 MDD patients to complete the childhood maltreatment experience assessment with Childhood Trauma Questionnaire (CTQ) and resting-state fMRI scan. Multivariate linear regression analysis was employed to identify the main effects of CM and depressive symptoms total and subfactors scores on bilateral anterior and posterior insula functional connectivity (IFC) networks, respecti…

The structure of depressive symptoms and characteristics and their relation to overall severity in major depressive disorder.

Although many investigators have examined symptoms of major depressive disorder (MDD), the multivariate relations among these features of depression and their relative associations with overall severity of depression are not well understood. The present study is the first to examine the underlying factor structure of depression across a broad set of constructs and to model the multivariate association of these factors with the overall severity of depression. We conducted a large-scale factor analysis and multiple regression in a sample of participants diagnosed with MDD (N = 233) and healthy c…

From brain networks to peripheral signatures: candidate biomarkers for major depressive disorder.

Major depressive disorder (MDD) is a leading cause of disability worldwide, yet its neural basis and biomarkers remain elusive. In this study, we combined neuroimaging, transcriptomics, proteomics, and machine learning to bridge large-scale brain network dysfunction with peripheral molecular signatures. Resting-state functional magnetic resonance imaging (fMRI) from 544 MDD patients and 569 healthy controls (HC) revealed altered network topology involving thalamic, fronto-striatal, and parietal regions. Connectome-transcriptomic association analysis of these network alterations, in conjunction…

Attachment Styles in Major Depressive Disorder: A Systematic Review and Meta-Analysis.

Major depressive disorder (MDD), a leading cause of disability, is associated with difficulties in close relationships. Attachment theory links early caregiving to internal working models shaping emotion regulation, interpersonal functioning and depression vulnerability. Attachment patterns in clinically diagnosed MDD have not been synthesised. Following PRISMA (PROSPERO: CRD42017068580), we searched for studies of adults with standardised diagnoses of MDD or postpartum depression (PPD) and attachment measures. Random-effects meta-analyses expressed categorical outcomes as odds ratios (ORs) an…

Ferroptosis in major depressive disorder: Molecular mechanisms, cellular vulnerability, and therapeutic opportunities.

Major depressive disorder (MDD) is a heterogeneous psychiatric disorder involving oxidative stress, neuroinflammation, mitochondrial dysfunction, impaired neuroplasticity, and metabolic dysregulation. Ferroptosis, an iron-dependent form of regulated cell death driven by phospholipid peroxidation and antioxidant defense failure, has recently attracted attention as a potential mechanism linking these pathological processes. This review summarizes the molecular basis of ferroptosis in MDD, focusing on iron dyshomeostasis, lipid peroxidation, and disruption of the System Xc-/glutathione (GSH)/glut…
