Improving Local Health Through Partner Engagement and Policy: Establishing the First Performance Standards for Massachusetts Local Public Health.

CONTEXT: The Massachusetts (MA) local public health system is highly decentralized. All 351 municipalities autonomously manage local public health infrastructure and budget, resulting in service delivery disparities. The MA Department of Public Health (MDPH) recognizes that achieving equitable access to public health services requires systems change, and successful systems change demands active participation and input from partners at all levels. OBJECTIVES: To collaboratively establish the first Performance Standards (PS) with and for MA local public health to reduce inequities in public hea

Rapid drinking water safety estimation in cities: Piloting a globally scalable method in Cochabamba, Bolivia.

BACKGROUND: Systematically collected and comparable data on drinking water safety at city-scale is currently unavailable, despite the stated importance of water safety monitoring at scale under the United Nations Sustainable Development Goals (SDGs). We developed a rapid drinking water quality assessment methodology intended to be replicable across all cities and useful for monitoring towards achieving SDG 6 (Clean Water and Sanitation). METHODS: We collected drinking water samples at the point-of-consumption for basic microbial, physical and chemical water quality analysis and conducted hous

An Acuity Tool for Heart Failure Case Management: Quantifying Workload, Service Utilization, and Disease Severity.

PURPOSE: The cardiology service line director at a health maintenance organization (HMO) in Washington State required a valid, reliable, and practical means for measuring workloads and other productivity factors for six heart failure (HF) registered nurse case managers located across three geographical regions. The Kilgore Heart Failure Case Management (KHFCM) Acuity Tool was systematically designed, developed, and validated to measure workload as a dependent function of the number of heart failure case management (HFCM) services rendered and the duration of times spent on various care duties.

Hospital healthcare experiences of children and young people with life-threatening or life-shortening conditions, and their parents: scoping reviews and resultant conceptual frameworks.

BACKGROUND: Patient experience is a core component of healthcare quality. Patient-reported experience measures (PREMs) are increasingly used to assess this, but there are few paediatric PREMs. This paper reports the first stage of developing two such measures, one for children and young people (0-18 years) (CYP) with a life-threatening or life-shortening condition (LT/LSC), and one for their parents. It comprised parallel scoping reviews of qualitative evidence on the elements of health service delivery and care that matter to, or impact on, CYP (Review 1) and parents (Review 2). METHODS: Med

Using meta-omics of contaminated sediments to monitor changes in pathways relevant to climate regulation.

Microbially mediated biogeochemical processes are crucial for climate regulation and may be disrupted by anthropogenic contaminants. To better manage contaminants, we need tools that make real-time causal links between stressors and altered microbial functions, and the potential consequences for ecosystem services such as climate regulation. In a manipulative field experiment, we used metatranscriptomics to investigate the impact of excess organic enrichment and metal contamination on the gene expression of nitrogen and sulfur metabolisms in coastal sediments. Our gene expression data suggest

Getting results used: evidence from reproductive health programmatic research in Guatemala.

This article reviews 44 operations research projects aiming to improve reproductive health services in Guatemala, conducted by the Population Council from 1988 to 2001. It documents the experience of the research programme, traces the extent to which research results are identifiable in existing programmes, and analyses factors influencing utilization. Utilization of research results occurs as a gradual process of information sharing, where researchers influence decision-makers through a continual stream of information rather than a single set of findings. Utilization depends on leadership, co

Mental health of people with limited access to health services: a retrospective study of patients attending a humanitarian clinic network in Germany in 2021.

BACKGROUND: Germany has a statutory health insurance system. However, a substantial part of the population still suffers from limited access to regular health services. While humanitarian organizations are partially filling this gap, people with limited access show a high prevalence of mental disorders. This study investigates the prevalence, and social determinants of mental disorders in patients attending the clinics of a humanitarian health network in three major cities in Germany, as well as perceived barriers to healthcare access in this population. METHODS: We performed a descriptive, r

Primary admission to a surgical service facilitates early cholecystectomy in acute cholecystitis but does not influence patient outcome.

PURPOSE: Early cholecystectomy is recommended for acute calculous cholecystitis to reduce complications and lower health care costs. However, not all patients admitted to emergency services due to acute calculous cholecystitis are considered for surgery immediately. Our intention was therefore to evaluate patient management and outcome parameters following cholecystectomy depending on the type of emergency service patients are primarily admitted to. METHODS: We performed a retrospective analysis of all patients that were treated for acute cholecystitis at our hospital between 2014 and 2021. O

Reorganization of a private psychiatric unit to promote collaboration with managed care.

Managed care organizations have become significantly involved in health care in the Denver metropolitan area. Their presence has challenged psychiatric hospitals to reduce costs and length of stay. In 1990, a locked private psychiatric unit was reorganized into locked, open, and partial care services through which patients progress at individualized rates. One treatment team manages patients in all settings, allowing a reduction in staffing costs and flexibility in treatment design. The hospital administration takes an active role in facilitating collaborative decision making between hospital

Neurosurgical disease encountered in a psychiatric service.

One hundred and seven patients with neurosurgical disease treated in a combined neurosurgical, neurological, and psychiatric unit within a psychiatric service were reviewed retrospectively. Most patients had acute confusional states or dementia without gross localizing signs and in only three cases did the neurosurgical illness closely resemble a non-organic psychiatric syndrome. The great majority showed abnormalities on physical examination and simple investigations. A past history of alcoholism and/or other psychiatric illness was common. Many apparently had been referred to psychiatric hos

Long-Range Air Transportation for High-Consequence Infectious Diseases: Findings from a Global Tabletop Exercise on Patients with Viral Hemorrhagic Fever.

OBJECTIVES: Air medical services evacuation of patients with viral hemorrhagic fevers (VHFs) is a complex process. The United States National Emerging Special Pathogens Training and Education Center held an in-person tabletop exercise (TTX) in June 2023 to review and evaluate global processes and plans for long-range VHF air transportation capabilities. The TTX sought to test the coordination, prioritization, capacities, and plans for using VHF transportation capabilities when multiple countries simultaneously request support in air medical services evacuation of their sick or exposed citizens

Antenatal care in rural Bangladesh: current state of costs, content and recommendations for effective service delivery.

BACKGROUND: Measurement of antenatal care (ANC) service coverage is often limited to the number of contacts or type of providers, reflecting a gap in the assessment of quality as well as cost estimations and health impact. The study aims to determine service subcomponents and provider and patient costs of ANC services and compares them between community (i.e. satellite clinics) and facility care (i.e. primary and secondary health centers) settings in rural Bangladesh. METHODS: Service contents and cost data were collected by one researcher and four interviewers in various community and facili

Epidemiology, pathophysiology and contemporary management of cardiogenic shock - a position statement from the Heart Failure Association of the European Society of Cardiology.

Cardiogenic shock (CS) is a complex multifactorial clinical syndrome with extremely high mortality, developing as a continuum, and progressing from the initial insult (underlying cause) to the subsequent occurrence of organ failure and death. There is a large spectrum of CS presentations resulting from the interaction between an acute cardiac insult and a patient's underlying cardiac and overall medical condition. Phenotyping patients with CS may have clinical impact on management because classification would support initiation of appropriate therapies. CS management should consider appropriat

Emergency medical preparedness and response for mass gatherings: Papua New Guinea emergency medical team's experience during Pope Francis's 2024 visit.

PROBLEM: Papua New Guinea has hosted several mass gatherings in recent years. Yet, there is limited data on the involvement of emergency medical teams in these events, let alone in other low- and middle-income countries. Prior to Pope Francis's state visit in September 2024, the national emergency medical team (EMT) had not simulated its updated clinical patient flow systems. CONTEXT: During Pope Francis's visit, an estimated 70 000 attendees gathered at the national stadium. Using Arbon's mass gathering model, the patient presentation rate was calculated to be 330 over 2 days, or 4.7 per 100

Relationship between Medicaid coverage design and receipt of medication for alcohol use disorder (MAUD): Probability of receipt increases based on comprehensiveness of plan.

Alcohol use disorder (AUD) affects one in ten Americans. As one of the largest payers of AUD treatment in the United States, Medicaid managed care plays a key role in facilitating access to AUD treatment services and medications. However, little is known about how AUD coverage in Medicaid managed care organizations (MCOs) affects treatment receipt. We examined the relationship between the comprehensiveness of Medicaid MCO plan coverage of AUD treatment and receipt of medications for AUD (MAUD). We used Medicaid claims data from Kentucky (2016-2019); our final analytic sample consisted of 202,2

Laparoscopic Management of an Intrauterine Device Perforating the Uterus and Adherent to the Rectum and Ovary: A Case Report.

Management of intrauterine device (IUD) perforation can be complex when adjacent pelvic organs are involved. Laparoscopic removal is commonly performed; however, cases involving bowel adherence may require intraoperative assistance from additional surgical services. We report the case of a 31-year-old gravida 2, para 2 (G2P2) woman with a history of cesarean delivery who presented with pelvic pain five weeks postpartum and was found to have an IUD perforating the posterior uterine wall, with adherence to the rectum and left ovary. The patient underwent robotic-assisted laparoscopic removal of
