Healing Process of Old Mandibular Condylar Head Fractures: A Report of Two Cases.

A condylar head fracture in the mandible is often misdiagnosed as a contusion of the temporomandibular joint (TMJ). Moreover, they are often overlooked by general practitioners as the fragments are small, making them difficult to identify. This report describes 2 cases of old mandibular condylar head fractures: one involving an 84-year-old woman referred to our dental office with suspected medication-related osteonecrosis of the jaw; and another involving a 23-year-old man who visited with the chief complaint of slight pain in the left TMJ. An old condylar head fracture with dislocation or dis

The Retromandibular Transparotid Approach for Reduction and Internal Fixation of Mandibular Condylar Fractures.

INTRODUCTION: The retromandibular transparotid approach is most useful for condylar and subcondylar fractures and provides the best access to the joint and ascending ramus. The study aims at evaluating the ease of access and outcomes encountered on using the retromandibular transparotid approach to access the fracture site for the open reduction and internal fixation (ORIF) of condylar and sub-condylar fractures. MATERIALS AND METHODS: An observational clinical study was carried out among 10 patients with unilateral and bilateral condylar fracture requiring ORIF. Maximal interincisal mouth op

Endoscope-Assisted Transoral Fixation of Mandibular Condyle Fractures: Submandibular Versus Transoral Endoscopic Approach.

In recent years, endoscope-assisted transoral approach for condylar fracture treatment has attracted much attention. However, the surgical approach is technically challenging: the procedure requires specialized instruments and the surgeons experience a steep learning curve. During the transoral endoscopic (TE) approach several instruments are positioned through a narrow oral incision making endoscope maneuvering very difficult. For this reason, the authors changed the entry port of the endoscope from transoral to submandibular area through a small stab incision. The aim of this study is to ass

Clinical and radiological outcomes of transoral endoscope-assisted treatment of mandibular condylar fractures.

Fractures of the mandibular condyle are one of the most common craniofacial fractures. However, the diagnosis and treatment of these fractures is controversial because of the multiple surgical approaches available. The purposes of this study were to identify surgery-related technical tips for better outcomes and to evaluate the results as well as complications encountered during 7 years of endoscope use to supplement the limited intraoral approach in the treatment of mandibular condylar fractures. Between 2005 and 2012, 50 patients with condylar fractures underwent endoscope-assisted reduction

Management of paediatric mandibular condylar fractures with screw-based semi-rigid intermaxillary fixation.

This study was designed to evaluate the feasibility and safety of screw-based semi-rigid intermaxillary fixation (IMF) combined with a specially designed occlusal splint in the conservative treatment of paediatric mandibular condylar fractures. Thirteen paediatric patients with 20 sides of condylar fractures treated with semi-rigid IMF were analyzed retrospectively. Semi-rigid IMF was achieved by inserting self-drilling IMF screws into the anterior alveolar bone of the maxilla and mandible suspended with elastic bands. An occlusal splint with a molar fulcrum was used for functional repositioni

[Mandibular fractures due to implant management--incidence, mechanism of injury and therapeutic characteristics].

We report of 5 of 337 patients (1.48%), in whom a fracture of the edentulous atrophied mandible occurred after dental implants had been inserted. In 79 patients a sandwich osteotomy (anterior and/or lateral mandible) had been carried out together with the insertion of implants. The reason for a fracture was infection in 3 patients and weakening of the atrophied mandible by the additional osteotomies in 2 patients. One of the latter two was seen intraoperatively and fixed immediately using miniplates. The other occurred about 4 weeks postoperatively and was not discovered until callus had been
