Suppurative Temporomandibular Arthritis Caused by Chronic Suppurative Otitis Media: A Case Report.

The clinical diagnosis and treatment, including information such as age, history, clinical symptoms, signs, audiology, imaging examination, mode of operation, and postoperative follow-up, of a patient with suppurative temporomandibular arthritis caused by chronic suppurative otitis media were analyzed. As conservative drug treatment and drainage surgery were ineffective, the patient was treated with microscopic open radical mastoidectomy, tympanoplasty, the plasty of the cavity of auricular concha, facial nerve decompression, coarctation of the mastoid cavity combined with otoendoscpic resecti…

The Anticoagulant Therapy for Otogenic Sigmoid Sinus Thrombophlebitis: A Case Report and Literature Review.

Sigmoid sinus thrombophlebitis is a severe and potentially fatal intracranial complication of acute otitis media and middle ear cholesteatoma. Early administration of broad-spectrum antibiotics and immediate radical mastoidectomy are the recommended standard treatments; however anticoagulant therapy is always an option worthy of clinical consideration. Here, we report a case of middle ear cholesteatoma complicated with sigmoid sinus thrombophlebitis in a patient who received anticoagulant therapy for 1 year before the operation because of the coronavirus disease 2019 pandemic.

Tympanoplasty in children--our experience in Riyadh, Saudi Arabia.

A seven years results of 75 cases of tympanoplasty without mastoidectomies in children is presented and analyzed. The study showed a perforation closure rate of 81.33%, hearing improvement to within 25 dB was 61.33%. The hearing did not improve in 18.66%. There is no higher incidence of recurrent perforation in children as compared to adults. In developing country such as Saudi Arabia experiences, skill, and proper development of facilities for management and post-operative care in distant areas may prove helpful in improving the results of tympanoplasty.

Effectiveness of MESNA on the success of cholesteatoma surgery.

IMPORTANCE: It is important that chronic otitis media with cholesteatoma be treated successfully in patients to protect them from having rep … ese patients underwent CWD (canal wall down), and twenty-two patients underwent CWU (canal wall up) mastoidectomy. For the other 95 subjects, 56 patients with CWD and 39 with CWU mastoidectomy, MESNA was not applied. Residual cholesteatoma rates were found to be significantly higher in MESNA non-applied group than MESNA applied group (p<0.05). Residual choleste …

[Benign intracranial hypertension in childhood following mastoiditis (author's transl)].

Case report concerning a three years old boy with benign intracranial hypertension. The typical symptoms are results of increased intracranial pressure, associated with headache, vomiting, choked disk, occasionally dehiscence of cranial sutures and sixth nerve paresis. In computed tomography there were no tumor signs. In childhood a possible cause in thrombosis of the lateral sinus following an occult mastoiditis after antibiotic therapy. Mastoidectomy, intensive antibiotic therapy as well as serial lumbar punctures led to complete recovery in most cases.

[A rare cause of otitis externa : Manifestation of myeloid sarcoma in the petrous bone].

A 45-year-old male patient presented with pronounced swelling of the external auditory canal and symptoms of otitis externa which did not respond to conservative treatment. Due to concomitant mastoiditis, mastoidectomy was performed in the later course, which did not provide conclusive histological results. In the absence of improvement, biopsy of the external auditory canal and mastoid revision with repeated pathological analysis were carried out. Ultimately, a myeloid sarcoma representing a relapse of previously remitted acute myeloid leukemia (AML) was identified.

Xanthoma of the temporal bone: a unique case of this rare condition.

Xanthoma of the temporal bone is extremely rare; we describe only the fourteenth reported case. Our case is further remarkable because it is the first report of such an occurrence in a patient with familial type III hyperlipoproteinemia. Moreover, while otalgia, infection, hearing loss, and tinnitus were the most common initial symptoms in the previous 13 cases, our patient reported only diplopia, vertigo, and unstable gait. The patient underwent a simple mastoidectomy and debulking, and his diplopia, vertigo, and unstable gait resolved.

Postoperative accumulation of bone in the middle ear: effect on hearing and surgical implications.

Two patients with conductive hearing loss caused by the intraoperative accumulation of bone particles in the middle ear are presented. This complication of otologic surgery may be prevented by an awareness of its possibility and by the temporary blockage of the aditus ad antrum in those operations involving mastoidectomy, even utilizing suction irrigation of the surgical field during the procedure. Once this condition is established, tympanotomy may confirm the presence of bone debris in the middle ear and the material may then be removed, with elimination of the conductive hearing loss.

A Novel Articulating Chip-on-Tip Endoscope for Dynamic Middle Ear Surgical Visualization.

OBJECTIVE: To enhance visualization in pediatric Otolaryngology middle ear surgeries and reduce mastoidectomy instances, we introduce a novel Articulating Chip-on-Tip Endoscope (ACoT Endo). METHODS: The ACoT Endo incorporates a cable-driven distal end camera and off-the-shelf Chip-on-Tip camera to improve visualization. We compared its capabilities with standard endoscopes, evaluating its bending capacity (70 ° ± 2 °) and center axis rotation (360 °). To test the overall functionality of this device, a Mock Ear was created to simulate the anatomy of the human ear, and the ACoT Endo's ability …

The patient underwent an urgent mastoidectomy and retro sigmoid craniotomy to remove a foreign object, involving a multidisciplinary team. A New Stapes-Head Coupler for the Vibrant Soundbridge System.

INTRODUCTION: The Vibrant Soundbridge (MED-EL Medical Electronics, Austria) is an active middle ear implant with a floating mass transducer (FMT) for patients with conductive, sensorineural, or mixed hearing loss. While the FMT is vertically aligned above the stapes head (SH) with the current Vibroplasty Clip coupler (MED-EL Medical Electronics), the new SH coupler was developed to mount the FMT on the inferior side of the stapes and to fit in the reduced middle ear space after canal-wall-down mastoidectomy. METHODS: Using 11 human cadaveric temporal bones (TBs), placements of the new SH coup…

Evaluation of the SAMEO-ATO surgical classification in a Dutch cohort.

PURPOSE: Differences in the definition and classification of cholesteatoma hinders comparing of surgical outcomes of cholesteatoma. Uniform registration is necessary to allow investigators to share and compare their findings. For many years surgical cholesteatoma procedures were divided into two main groups: canal wall up mastoidectomy (CWU) and canal wall down mastoidectomy (CWD). Recently, mastoid obliteration can be added to both procedures. Because of great variation within these main groups, the International Otology Outcome Group (IOOG) proposed the new SAMEO-ATO classification system to…

Eosinophilic granuloma of the temporal bone in children.

Eosinophilic granuloma (EG) is a bony destructive disease that frequently occurs in children; it is a subtype of Langerhans cell histiocytosis. The aims of this study were to detect the presenting features of temporal bone lesions in children and to evaluate the efficacy of surgery combined with radiotherapy in treatment of the disease. A retrospective study on 12 children with EG of the temporal bone was done. Computed tomography and hearing assessment were performed for all patients. All patients were treated with cortical mastoidectomy followed by postoperative radiotherapy. Follow-up was c…
