A retrospective analysis of maternal and neonatal mortality at a teaching and referral hospital in Kenya.

OBJECTIVE: To measure the incidence of maternal and early neonatal mortality in women who gave birth at Moi Teaching and Referral Hospital (MTRH) in Kenya and describe clinical and other characteristics and circumstances associated with maternal and neonatal deaths following deliveries at MTRH. METHODS: A retrospective audit of maternal and neonatal records was conducted with detailed analysis of the most recent 150 maternal deaths and 200 neonatal deaths. Maternal mortality ratios and early neonatal mortality rates were calculated for each year from January 2004 to December 2011. RESULTS: B…

Models of care that have reduced maternal mortality and morbidity in Sri Lanka.

Sri Lanka, a non-industrialized country with limited resources, has been able to achieve a maternal mortality ratio that is markedly lower than the ratios of similar countries. Many factors have contributed to Sri Lanka's success story. A political commitment to the cause and implementation of clear policies through well-structured and organized community-based and institutional healthcare services--expanded to cover the whole country and provided free of charge--have been the foundation of maternal and child health (MCH) services in the country. The healthcare programs have been well accepted…

[Maternal mortality due to pre-eclampsia/eclampsia in a state in southern Brazil].

PURPOSE: to identify the profile, tendency and causes of maternal death by pre-eclampsia/eclampsia in Paraná. METHODS: descriptive, transversal cohort study on maternal death by pre-eclampsia/eclampsia from 1997 to 2005. Data were obtained from case studies prepared by Maternal Death Committees that employ the Reproductive Age Mortality Survey Method to examine all the cases of death among women in fertile age. The general and specific maternal death rate (MDR) by pre-eclampsia/eclampsia were considered. To evaluate the tendency, triennial periods have been compared, two by two, taking into c

Maternal mortality, stillbirth and measures of obstetric care in developing and developed countries.

OBJECTIVE: Maternal mortality and stillbirths are important adverse pregnancy outcomes, especially in developing countries. Because underlying causes of both outcomes appeared similar, the relationship between maternal mortality, stillbirth and three measures of obstetrical care were studied. METHODS: Using data provided by the World Health Organization from 188 developed and developing countries, correlations and linear regression analyses between maternal mortality and stillbirth rates and cesarean section rates, skilled delivery attendance, and >or=4 prenatal visits) were developed. RESUL…

Factors associated with anaesthesia-related maternal mortality in a tertiary hospital in Nigeria.

BACKGROUND: Maternal mortality related to anaesthesia is low compared with that resulting from obstetric factors in developed countries. The role of anaesthesia in maternal mortality in developing countries is obscure. The purposes of this study were to determine the incidence of maternal mortality related to anaesthesia, to analyse the causes and to suggest measures to improve anaesthetic safety for parturients. METHODS: The hospital surgical registry was reviewed from 1 January 1991 to 31 December 2000 to identify patients who had undergone surgical procedures in pregnancy or puerperium. Da…

Cause-specific mortality during and after pregnancy and the definition of maternal death.

As part of a study to determine whether maternal mortality in Canada is under- reported, we explored the validity of including deaths not directly related to pregnancy. We linked live birth and stillbirth registrations to death registrations of women of reproductive age from 1988 through 1992. We calculated standardized mortality ratios, by cause, from deaths in women known to have been pregnant and deaths in same-aged women not known to have been pregnant within the same time period. Women known to have been pregnant were approximately half as likely to die as would be expected in each of two…

Maternal mortality in a rural district of southeastern Tanzania: an application of the sisterhood method.

BACKGROUND: Deaths from maternal causes represent the leading cause of death among women of reproductive age in most developing countries. It is estimated that the highest risk occurs in Africa, with 20% of world births but 40% of the world maternal deaths. The level of maternal mortality is difficult to assess especially in countries without an adequate vital registration system. Indirect techniques are an attractive cost-effective tool to provide estimates of orders of magnitude for maternal mortality. METHOD: The level of maternal mortality estimated by the sisterhood method is presented f…

A study of perinatal mortality and associated maternal profile in a medical college hospital.

Study of perinatal mortality in a Medical College Hospital revealed stillbirth rate as 38.4, early neonatal death rate as 29.3 and overall perinatal morality rate as 67.7 per 1000 live births. More than half (53.6%) of the perinatal deaths were in primipara and another 22.8% in mothers of parity more than 3. Most mothers (85.9%) did not receive adequate antenatal care services. On admission 35.1% mothers presented with some risk factors. The major risk factors identified were toxaemia of pregnancy (14.8%), severe anaemia (13%) and antepartum haemorrhage (2.6%).

[Maternal and fetal morbidity and mortality in premature abruptio placentae--a retrospective analysis].

The frequency of abruptio placentae has nearly tripled in our clinic since 1974 and now stands at 1.4% of all deliveries. In a retrospective analysis it was investigated whether improved diagnostic and therapeutic methods have decreased the morbidity and mortality of this severe obstetrical complication. During 3 stages of observation--1974-1979, 1980-1985 and 1986-6/1990--we registered 123 cases. The division into time intervals was based on the introduction of sensitive ultrasonic methods in 1980 and the vaginal scan in 1986. Important risk factors of abruptio placentae were found to be blee

Harare Hospital maternal mortality report for 1987 and a comparison with previous reports.

There were 39 maternal deaths at Harare Hospital during 1987, giving a maternal mortality rate of 122/100,000 live births. If women who lived outside the Harare Municipality were excluded, the maternal mortality rate for the Greater Harare Maternity Unit was 53/100,000 live births. The cases were reviewed at monthly meetings in the Department of Obstetrics and Gynaecology. Hypertensive disease in pregnancy caused 28pc of the deaths with haemorrhage, puerperal sepsis and abortion accounting for 18pc each. Avoidable factors were felt to be present in 88pc of cases and these are discussed.

Maternal employment and infant mortality: an examination of the role of breastfeeding as an intermediate factor.

In a survey of 2111 mothers in Ile-Ife, Nigeria, the relationship between infant mortality and maternal employment was examined. By the use of logit regression taking socioeconomic variables and breastfeeding into account, it was shown that children of working class mothers are likely to experience higher rates of infant death than those of non-working mothers, and these rates are higher in mothers working in the informal than in the formal sector. No effect of duration of breastfeeding was detectable, probably due to the universal practice of long lactation among the Yoruba. Intensity of brea

[The impact of a new strict obstetrical strategy on maternal mortality and obstetrical results].

In an attempt to reduce the caesarean section rate without an adverse effect on the obstetrical outcome, new guidelines for the management of dystocia, previous caesarean delivery, fetal distress, and breech presentation were introduced in September 1984 in a provincial hospital in Zimbabwe, Africa. Comparison of the two-year periods before and after September 1984 showed that the caesarean section rate had dropped from 16.8 to 8.0%, the maternal mortality rate from 2.0 to 0.5%, and the perinatal mortality rate from 71.9 to 56.2%. During the latter period use of oxytocin increased from 3.4 to …

Viral hepatitis as a major cause of maternal mortality in Addis Ababa, Ethiopia.

Causes of maternal mortality were investigated in Addis Ababa, Ethiopia, from September 1981 to September 1983. Viral hepatitis ranked third among the leading causes of maternal mortality behind septic abortion and puerperal sepsis. There were 26 deaths from viral hepatitis during the 2-year study period for a hospital maternal mortality rate of 91.0 per 100,000 live births. Although 30% of women who died of all maternal causes received antenatal care in Addis Ababa, only 13% of women who died from viral hepatitis in our hospital study received antenatal care. Low socio-economic status (SES) h…

Maternal mortality due to the hepatorenal syndrome of pre-eclampsia. A case report.

A maternal death in the 23rd wk of pregnancy is described. The patient was gravida 8, with 6 previous abortions and one growth-retarded term infant. The clinical symptoms and biochemical findings strongly suggest that the mode of death was septic shock, with the hepatorenal syndrome due to pre-eclampsia as the underlying cause.
