A 45-year-old Costa Rican woman was seen for a jejunostomy tube malfunction. There was no evidence of tube malposition or intestinal obstruction. During endoscopy, a long worm was retrieved from the distal duodenum; it was later confirmed to be Ascaris lumbricoides. After treatment with mebendazole, no further episodes of tube occlusion were observed. This case reminds us of the importance of considering helminthic infections and their atypical manifestations in patients from endemic regions.

In corroboration of previous research results our own investigations show that advances in liver surgery are achieved above all by early and considerably extended clinical diagnosis as well as by decisively improved surgical technique. Supporting measures such as mebendazole medication in the surgical management of echinococcosis have an important share in successful treatment. However, there is no doubt that, even today, liver surgery is not the routine operation ranking equal with biliary or gastric surgery.

Thirty-three patients with alveolar echinococcosis treated by surgery or with mebendazole were observed up to six years. Of three immunoassays (IgG ELISA, complement fixation, indirect haemagglutination) correlation with the clinical course was achieved by IgG ELISA only. Serum antibody levels tended to increase during the first year and decreased during the second year after therapy had been started, Long persisting antibody was most sensitively indicated by indirect haemagglutination.

Five cases of hydatid disease affecting the vertebrae are described. Clinical features, radiologic appearance, and poor prognosis are emphasized. Four patients had some degree of neurologic deficit at the time of presentation and in three cases there was an increasing kyphosis. In two patients, spinal instrumentation with Harrington apparatus supplemented removal of hydatid material, and its use improved the previous neurologic deficit with stabilization of kyphosis. The antihelmintic drug mebendazole was used in two cases.

Results of surgery in 52 cases of hepatic hydatid disease are described. Ultrasound examination proved to be the most useful and accurate diagnostic tool. All the patients were treated by enucleation of cyst and closed suction drainage of the cavity lined by pericyst. Mebendazole was given routinely pre-operatively for 15-30 days and postoperatively for 3 months. This mode of treatment had greatly reduced the morbidity and mortality as compared to conventional hydatid cyst surgery.

In an experimental larval infection of Ancylostoma caninum in mice, the efficacy of various anthelmintics against the larvae migrated and established in the brain is reported for the first time. Albendazole and flubendazole were the most effective drugs. Thiabendazole, benacil, phenacizole, oxfendazole and mebendazole showed significant larvicidal activity. Tetramisole, levamisole, fenbendazole, Sch 18099, pyrantel pamoate, morantel tartrate and oxantel pamoate did not show any significant activity even at relatively high dose levels.

The larva of hookworm matures into the adult stage in the small intestine, causing chronic intestinal blood loss and iron-deficiency anemia. Hookworm infestation of colon has not been reported previously. We report a 35-year-old man who presented with diarrhea with blood and mucus in stools. Colonoscopy revealed several hookworms in the colon firmly adherent to the mucosa, with oozing of blood and surrounding mucosal erosions. He was treated with mebendazole and symptoms recovered completely.

The whipworm (Trichuris trichiura) is an occasional finding in anorectal disease. All cases of anorectal discharge should include a test for ova and parasites. Trichuris infestations can be eliminated by the use of mebendazole (Vermox), a specific drug which has almost no side effects. It should be remembered that structural diseases of the anorectum, occurring during an infection such as fissure, fistula, and abscess, may persist after the infection has been cured and usually must be treated surgically.

Ascaris lumbricoides is a worldwide intestinal infestation that may cause various complications. Biliary ascariasis, however, is a rare condition. We describe a child with biliary ascariasis. The patient's clinical symptoms were pain, vomiting and abdominal tenderness, and she was thought to have acute appendicitis. However, laboratory examination revealed high serum alkaline phosphatase and amylase levels, and ultrasonography and percutaneous cholangiography demonstrated biliary ascariasis. The patient was successfully treated with mebendazole and antispasmolytic drugs.

The paper describes the synthesis of 6-[4-alkylpiperazinyl-1)phenylamino]-1,2,5-thiadiazolo[3,4-h ]quinolines where methyl (Drug G-1574) and ethyl (Drug G-1569) are alkyls. The two agents are as effective as mebendazole against the larval stage of Echinococcus multilocularis infection. Drug G-1574 has been demonstrated to ensure 100% recovery of spontaneously Hymenolepis nana-infected albino mice given doses 2.5-5 times lower than the effective dose of phenasal (niclosamide).

Hamsters infected with laboratory-adapted preadult Necator americanus were dosed with 6 reference anthelmintics. Their efficacy was measured in terms of percentage cure of infected animals as well as percentage worm reduction following treatment. Mebendazole and pyrantel were equally effective in this system. Other anthelmintics, including anti-hookworm compound, bephenium hydroxynaphthoate, were less effective. The comparative results revealed that the N. americanus model is sensitive and reliable for identifying and characterizing new anti-parasite preparations.

A patient with multiple liver cysts, erroneously diagnosed as suffering from echinococcal infection, was treated with mebendazole for four months. When fever appeared, she was treated with antibiotics, but her condition further deteriorated. On laparotomy polycystic liver disease was diagnosed and four liters of pus (due to peptostreptococcal infection) was evacuated from one of the cysts, which was also deroofed. The other cysts were excised and the patient recovered. These unusual events are discussed in the light of the pertinent literature.
