Primary resection and anastomosis for complicated meconium ileus: a safe procedure?

Various options are available for the surgical treatment of meconium ileus (MI). This paper examines the use of resection and primary anastomosis as the favoured option for the treatment of complicated meconium ileus. This was a retrospective study. All patients (13 children) with MI treated with primary resection and anastomosis (RA) in the 10-year period (1996-2005) at St Mary's Hospital in Manchester were identified. The case notes were retrieved. The gestational age, type of surgery, length of bowel resection and complications were recorded. Out of 13 (3 males, 10 females), 7 had severely …

Direct in vitro effects of meconium on airway reactivity in adult rabbits.

INTRODUCTION: Meconium aspiration syndrome (MAS) is a serious neonatal disease with multifactor pathogenesis and long-term sequelae on the developing respiratory system. Since the altered airway reactivity may play a role in MAS, this study investigated whether 1 hour-lasting in vitro incubation of rabbit tracheal and lung tissue strips with meconium would also increase the airway reactivity. METHODS: Trachea and lungs of adult healthy rabbits were excised. Smooth muscle reactivity was estimated by an in vitro method using organ chambers. After the adaptation of tissue strips, 1 ml of suspend…

Detection of cotinine in neonate meconium as a marker for nicotine exposure in utero.

Neonate meconium cotinine level was evaluated as a marker of prenatal exposure to nicotine from tobacco smoking by mothers. Mothers admitted to a maternity hospital in Alexandria, Egypt, were divided into 3 groups: 10 active smokers, 10 passive smokers and 10 with no tobacco exposure during pregnancy. Urine and saliva samples were collected from mothers and first-day meconium samples from their neonates. Mean maternal urinary cotinine levels, measured using radioimmunoassay, differed significantly between the 3 groups, as did mean salivary cotinine and mean cotinine levels in meconium. There w…

Inguinal hernia as a rare manifestation of meconium peritonitis: report of a case.

The usual manifestations of meconium peritonitis confined to the inguinoscrotal region are soft hydroceles, hard nodules in the scrotum, and, occasionally, calcified nodules in the wall of a hernia sac, with or without calcification on abdominal X-ray. Inguinal hernia is an extremely rare manifestation of healed meconium peritonitis. An unusual presentation of meconium peritonitis encountered during hernia repair is described herein to alert the pediatric surgeon of this possibility to avoid unnecessary abdominal exploration.

Is meconium passage a risk factor for maternal infection in term pregnancies?

OBJECTIVE: To study the association between meconium and maternal infection. METHODS: This was a retrospective cohort study of 678 pregnant women. All term deliveries during a 31-month period with meconium passage were included. Each meconium delivery was matched with a consecutive delivery without meconium at the same gestational age. RESULTS: The overall infection rate was 16%, with 13% of the infections directly related to the pregnancy, labor, and delivery. The endometritis rate was 5%, with 7.1% and 3% in the meconium and no-meconium group, respectively. The chorioamnionitis rate was 8.…

Effect of meconium ileus on the clinical prognosis of patients with cystic fibrosis.

The objective of the present study was to determine the possible prognostic factors which may explain the difference in the survival of patients with cystic fibrosis (CF) with and without meconium ileus. Over a period of 20 years, 127 patients with CF, whose diagnosis was confirmed by typical clinical characteristics and altered sweat chloride levels, were studied retrospectively. The patients were divided into two groups: group 1 consisted of patients who presented CF and meconium ileus (N = 9), and group 2 consisted of patients with CF without meconium ileus (N = 118). The characteristics st…

The impact of meconium ileus on the clinical course of children with cystic fibrosis.

The present study was designed to compare the clinical course of children diagnosed with cystic fibrosis (CF) in infancy due to the presence of meconium ileus (MI) with children diagnosed by way of a newborn screening programme (non-MI). A matched case-control study design was used. Matching was performed on the basis of sex and date of birth. All children born in New South Wales, Australia after 1980 and who had attended the CF clinic at The Children's Hospital at Westmead since diagnosis were included as possible cases or controls. Parameters pertaining to the clinical course were compared i…

Effects of magnesium sulfate in a newborn piglet meconium aspiration model.

OBJECTIVE: To examine the hemodynamic effects of magnesium sulfate (MgSO4) in a neonatal model of meconium aspiration syndrome. DESIGN: A prospective animal study. PATIENTS AND PARTICIPANTS: Eight anesthetized neonatal piglets. METHODS: Animals were instrumented under fentanyl anesthesia for the determination of pulmonary and systemic blood pressures and cardiac index. A model of meconium aspiration was produced by instilling 6 to 7 ml of 20% fresh human meconium in normal saline down the endotracheal tube, resulting in hypoxemia (mean arterial O2 saturation 73 SD, 10%), respiratory acidosi…

Is gastric lavage needed in neonates with meconium-stained amniotic fluid?

UNLABELLED: We compared the incidence of complications from meconium-containing gastric fluid in a group of neonates born with meconium-stained amniotic fluid (MSAF) who did not routinely have gastric lavage prior to feeds, versus a group who had elective gastric lavage before the first feed. In the first group, 275 neonates born with MSAF were fed without prior gastric lavage. While 13 developed feeding problems, the other 262 infants (95%) who did not undergo routine gastric lavage remained free of later feeding difficulties or secondary meconium aspiration. In the second group, all 227 neon…

Meconium hydrocele in a female newborn: an unusual cause of a labial mass.

Meconium peritonitis results from in utero perforation of the bowel and subsequent spillage of meconium into the peritoneal cavity. Free communication of the peritoneal space with the processus vaginalis during gestation permits formation of a meconium hydrocele. Meconium hydrocele has been reported in the newborn scrotum but to our knowledge there has been no previous report of meconium hydrocele in the labium of a female neonate. The predominance of meconium hydrocele in the male infant may be due to the obliteration of the processus vaginalis occurring later in the male than in the female f…

Meconium ileus--a rare cause of neonatal intestinal obstruction in Malaysia.

Meconium ileus, rare in Malaysia, accounts for 3.7% of all neonatal intestinal obstructions (excluding imperforate anus) seen in the University Hospital, Kuala Lumpur, from 1980-1990. This paper retrospectively reviews our clinical experience with 5 cases of meconium ileus seen over a 12-year period from 1980-1991 in the University Hospital, Kuala Lumpur. Three of the neonates were Malays, and two were Punjabis. Four of them were full-term and one preterm. The birth weights ranged from 1900 to 3700 g, with a mean of 2670 g. One of them also had a sibling with meconium ileus. Two of them were f…

[Relationship of chest roentgenographic features and outcome in meconium aspiration syndrome].

Aspiration of meconium may produce respiratory distress of various severity and outcome. It is the purpose of this study to evaluate the relationship between roentgenographic feature of initial chest roentgenogram (less than 10 hours old) and outcome in infants with meconium aspiration syndrome (MAS). We analyzed retrospectively the clinical data and initial chest roentgenograms of 65 infants who had MAS and who were admitted to NICU of Mackay Memorial Hospital in 1987. The incidence and the mortality rate in this hospital were 0.33% and 15.4% respectively. Infiltration was seen in 52, consoli…

Leiomyosarcoma of the transverse colon in a neonate: a rare cause of meconium peritonitis.

A rare case of a newborn infant with leiomyosarcoma of the transverse colon is reported. The condition was associated with meconium peritonitis due to a perforation proximal to a portion of the transverse colon that was completely surrounded by the tumor. The 12 previously published cases of leiomyosarcoma of the colorectum in childhood are reviewed, and the pathogenesis of meconium peritonitis is discussed.

Meconium periorchitis: a clinicopathologic study of four cases with a review of the literature.

Perforation of the bowel wall in utero, with meconium gaining access to the tunica vaginalis, results in meconium periorchitis. The clinical and pathologic features in four infants were studied; the findings were compared with those in the 20 cases reported in the literature. In the majority of cases, the intestinal perforation had apparently healed before birth, and the scrotal mass became increasingly obvious following birth. Cystic fibrosis was documented in only one case. A yellowish green paratesticular mass with multifocal dystrophic calcification was the principal gross finding. Lobules…

Technical aspects in the management of the meconium aspiration syndrome with extracorporeal circulation.

Sixteen neonatal patients diagnosed as having the meconium aspiration syndrome were selected for management with extracorporeal circulation with a membrane oxygenator (ECMO) with 8 survivors over 4 yr. All patients weighed greater than 2 kg. Each was placed in the 100% mortality group according to a Neonatal Pulmonary Insufficiency Index (NPII) based on hourly pH and FiO2 determinations. The typical patient course on ECMO was stabilization for the first 12 hr then improvement on high bypass flow rates for 12-24 hr to maintain a pAO2 for 50-60 mm Hg with minimal ventilator settings with an FiO2…
