A cross-sectional study of emergency care utilization and associated costs of violent-related (assault) injuries in the United States.

BACKGROUND: Violent-related (assault) injuries are a leading cause of death and disability in the United States. Many violent injury victims seek treatment in the emergency department (ED). Our objectives were to (1) estimate rates of violent-related injuries evaluated in United States EDs, (2) estimate linear trends in ED visits for violent-related injuries from 2000 to 2010, and (3) to determine the associated health care and work-loss costs. METHODS: We examined adults 18 years and older from a nationally representative survey (the National Hospital Ambulatory Medical Care Survey) of ED vi

The Cost of Inaction in Treating Obesity in the U.S.

OBJECTIVE: To quantify the preventable disease burden and foregone economic benefits resulting from maintaining current limited access to obesity treatment among U.S. adults. RESEARCH DESIGN AND METHODS: Using microsimulation modeling, we projected 10-year health and economic outcomes under three scenarios: current limited treatment access, expanded access to intensive lifestyle management or earlier-generation medications, and expanded access to glucagon-like peptide 1 (GLP-1) and glucose-dependent insulinotropic polypeptide (GIP)/GLP-1 receptor agonists. The model simulated individual-level

Readability assessment of online peripheral artery disease education materials.

OBJECTIVE: Online resources can be a valuable source of information for patients and have been shown to result in more inquiry during medical office visits, following physician medical recommendation more closely, and making self-directed lifestyle changes. The accessibility to these resources is limited by the readability level of the article and the literacy level of the population. Peripheral artery disease (PAD) is estimated to affect between 8 and 12 million people in the United States with greater disease severity among under insured or uninsured populations. As PAD continues to increase

A Retrospective Chart Review of Uninsured Encounters in a Primary Care Hospital in Japan.

Introduction Lack of health insurance is known to delay care-seeking and contribute to preventable morbidity. Although Japan has established a universal health insurance system, more than 10% of households have unpaid insurance premiums, and approximately 2.5% possess insurance certificates with restricted or no validity. Under these circumstances, preventable deaths attributable to delayed medical care for financial reasons have been reported. Individuals facing unstable employment or job loss are particularly vulnerable to premium non-payment, even when willingness to pay exists. The actual

Undertreatment of opioid use disorder in patients hospitalized with injection drug use-associated infections.

OBJECTIVE: To evaluate the association between medication for opioid use disorder (MOUD) initiation and addiction consultation and outcomes for patients hospitalized with infectious complications of injecting opioids. METHOD: This was a retrospective cohort study performed at four academic medical centers in the United States. The participants were patients who had been hospitalized with infectious complications of injecting opioids in 2018. Three hundred and twenty-two patients were included and their individual patient records were manually reviewed to identify inpatient receipt of medicati

Follow-up After Proximal Femur Fracture Fixation-We Need to Do Better.

BACKGROUND: Long-term follow-up after proximal femur (femoral neck) fracture is required to screen for complications, especially avascular necrosis (AVN). The purpose of this study was to evaluate sociodemographic characteristics as risk factors for lost-to-follow-up (LTFU) status following proximal femur fractures. METHODS: This retrospective case series included patients under the age of 18 years treated at a children's hospital for proximal femur fractures. Data were collected from the electronic medical record (EMR). Area deprivation index (ADI) was determined using patient addresses. Pat

Analysis of Oklahoma amputation trends and identification of risk factors to target areas for limb preservation interventions.

OBJECTIVE: Annual trends of lower extremity amputation due to end-stage chronic disease are on the rise in the United States. These amputations are leading to massive expenses for patients and the medical system. In Oklahoma, we have a high-risk population because access to care is low, the number of uninsured is high, cardiovascular health is poor, and our overall health care performance is ranked 50th in the country. But we know little about Oklahomans and their risk of limb loss. It is, therefore, imperative to look closely at this population to discover contemporary rates, trends, and stat

Outcomes in patients visiting hospital emergency departments in the United States because of periodontal conditions.

BACKGROUND: The chances of presenting to hospital emergency departments (EDs) are significantly higher in individuals who ignore regular dental care and in those with medical conditions. Little is known about nationwide estimates of hospital-based ED visits caused by periodontal conditions in the United States. The objective of this study is to determine the incidence of ED visits caused by periodontal conditions that occurred in a 2006 nationwide sample and to identify the risk factors for hospitalization during the ED visits. METHODS: The Nationwide Emergency Department Sample (NEDS) for 20

Increasing Syphilis Diagnoses Among Females Giving Birth in US Hospitals, 2010-2014.

BACKGROUND: National trends in syphilis rates among females delivering newborns are not well characterized. We assessed 2010-2014 trends in syphilis diagnoses documented on discharge records and associated factors among females who have given birth in US hospitals. METHODS: We calculated quarterly trends in syphilis rates (per 100,000 deliveries) by using International Classification of Diseases, Ninth Revision, Clinical Modification codes on delivery discharge records from the National Inpatient Sample. Changes in trends were determined by using Joinpoint software. We estimated relative risk

Beyond Health Effects?: Examining the Social Consequences of Community Levels of Uninsurance Pre-ACA.

The lack of health insurance is traditionally considered a problem faced by individuals and their families. However, because of the geographically bounded organization and funding of healthcare in the United States, levels of uninsurance in a community may affect everyone living there. Health economists have examined how the effects of uninsurance spillover from the uninsured to the insured, negatively affecting healthcare access and quality for the insured. We extend research on uninsurance into the domain of sociologists by theorizing how uninsurance might exacerbate social inequalities and

Complex routes into HIV care for migrant workers: a qualitative study from north India.

Migrant workers are designated a bridge population in the spread of HIV and therefore if infected, should be diagnosed and treated early. This study examined pathways to HIV diagnosis and access to care for rural-to-urban circular migrant workers and partners of migrants in northern India, identifying structural, social and individual level factors that shaped their journeys into care. We conducted a qualitative study using in-depth interviews with HIV-positive men (n = 20) and women (n = 13) with a history of circular migration, recruited from an antiretroviral therapy centre in one district

Insurance status of U.S. organ donors and transplant recipients: the uninsured give, but rarely receive.

Organ transplantation is an expensive, life-saving technology. Previous studies have found that few transplant recipients in the United States lack health insurance (in part because patients may become eligible for special coverage because of their disability and transplant teams vigorously advocate for their patients). Few data are available on the insurance status of U.S. organ donors. The authors analyzed the 2003 National Inpatient Sample (NIS), a nationally representative 20 percent sample of U.S. hospital stays, and identified incident organ donors and recipients using ICD-9-CM diagnosis

Healthcare insurance status and hypertension control before and after the Affordable Care Act.

BACKGROUND: Blood pressure control among adults <65 years old with public, private, or no healthcare insurance after the US 2010 Affordable Care Act (ACA) is unknown. METHODS: National Health and Nutrition Examination Surveys were analyzed pre- (2001-2010) and post-ACA (2015-2023) on adults aged 20-64 years (pre-Medicare eligibility) with hypertension (systolic ≥140 or diastolic blood pressure ≥90 mmHg, or antihypertensive therapy). Relationships of blood pressure control (<140/<90 mmHg) to healthcare insurance and social determinants (race/ethnicity, low income [<200% federal poverty]) were

Healthcare Inequities and Structural Racism as Determinants of Treatment and Survival among Individuals Recently Diagnosed with Cervical Cancer from Two Diverse Population-Based Cancer Registries: The ACHIEVE Study.

Persistent inequities in cervical cancer burden disproportionately impact marginalized communities and reflect social and structural barriers to high-quality care. Guided by socioecological models, the Assessing Cervical Cancer Healthcare Inequities in Diverse Populations (ACHIEVE) Study examines social and structural influences on cancer treatment among survivors (aged 21-79 years, diagnosed with any stage of cervical cancer between 2021 and 2025, who can complete study surveys in English, Spanish, Chinese, or Haitian Creole) identified through two population-based cancer registries: the New
