[Lamotrigine in the treatment of mental disorders].

BACKGROUND: Bipolar and other mood disorders often do not respond satisfactorily to lithium, valproate, carbamazepine or antidepressants. MATERIAL AND METHODS: On the basis of six years of clinical experience with lamotrigine in the treatment of psychiatric disorders, supplemented by three case reports and a comprehensive literature review of empirical studies, we discuss the possible indications for lamotrigine in psychiatric disorders. RESULTS: Lamotrigine seems to have a stronger antidepressant effect in bipolar 1 disorders than gabapentine, carbamazepine and valproate, but a weaker effec

[Alpha-interferon and mental disorders].

UNLABELLED: The interferon alpha stands as a reference both in oncology and virology. But its efficiency is limited by frequent somatic as well as neuropsychic side effects. As a matter of fact, the reduction or the ending of a chemotherapy treatment come chiefly from the psychiatric complications caused by the use of interferon. For about 30% of patients, various psychic disorders are noticed: personality disorders, mood disorders, anxiety states, suicidal tendencies, manic and psychotic symptoms. We thus propose a review which shall be completed by a discussion on wether the interferon is re

Childhood mental ability and dementia.

OBJECTIVE: To examine links between childhood mental ability and dementia using data from a 1932 survey of the mental ability of the 1921 Scottish birth cohort. METHOD: Patients with dementia from the 1921 Scottish birth cohort were located in 1) a national survey of early-onset dementia (1974-1988), 2) local mental health services, and 3) a survey of 264 of 519 surviving Aberdeen residents who took the 1932 test. Control subjects were identified in the 1932 Scottish Mental Survey. RESULTS: Mean 1932 ability score for the Scottish 1921 cohort did not differ from early-onset dementia. Early-o

Identification and evaluation of mental retardation.

Mental retardation in young children is often missed by clinicians. The condition is present in 2 to 3 percent of the population, either as an isolated finding or as part of a syndrome or broader disorder. Causes of mental retardation are numerous and include genetic and environmental factors. In at least 30 to 50 percent of cases, physicians are unable to determine etiology despite thorough evaluation. Diagnosis is highly dependent on a comprehensive personal and family medical history, a complete physical examination and a careful developmental assessment of the child. These will guide appro

Ophthalmologic abnormalities in mentally retarded.

PURPOSE: To determine the prevalence and types of ophthalmologic abnormalities in mentally retarded subjects. METHODS: Seventy-three institutionalized mentally retarded subjects (41 boys and 32 girls) aged 5 to 19 years (mean: 11.5 years) were examined ophthalmologically during a three month period (from May to July 1999). The eye examination consisted of visual acuity testing (illiterate Snellen E-chart or acuity card procedures), pupillary reflex and motility evaluation, cover test for phoria and tropia, examination of adnexa and anterior segment (magnifying glass or slit-lamp biomicroscopy

Recurrence risks in mental retardation.

Despite improvements in diagnostic techniques and progress made in mapping genes associated with syndromal mental handicap, the estimation of recurrence risks in non-syndromal mental retardation is still dependent on empirical data. Unfortunately, few studies are available to guide the clinician and their results differ significantly. For example, recurrence risks to all sibs of a male index patient with severe mental retardation vary between 3.5% and 14% in commonly quoted series. The present review highlights the problems involved in interpreting the previous work in this area and discusses

Homelessness, mental illness and the law.

One major difficulty that faces both researchers and practitioners who work with the homeless is the lack of definitions and frameworks that assist better understanding of such people's problems and society's response to them. This article reviews the legislation dealing with homeless people's social, civil and legal problems. We tried to give this legislation's historic and social background, including a section on psychiatric disorders in this group because of their relevance to current debate on homeless offenders. We then reviewed the legislation relevant to homeless people in general and

[Cocaine abuse and mental disease].

The comorbidity between psychiatric diseases and cocaine abuse is extensive. Several studies show that the prevalence of specific personality disorders, eating disorders, attention-deficit-disorder and affective disorders is high in cocaine abusers. Excessive cocaine consumption can lead to a toxic schizophrenic psychosis. The comorbidity has been understood in several ways, using many theoretical approaches. The different hypotheses should not be considered contradictive. Explanations involving genetic, psychosocial, psychological and biological approaches are necessary for a complete underst

[Mental dysfunction in Parkinson's disease].

In Parkinson's disease, mental disturbances frequently accompany the typical motor disorder. Subcortical dementia develops in 10 to 20% of patients. Contrary to the dementia of Alzheimer's type, the apraxia-aphasia-amnesia syndrome is uncommon. Depression and specific impairment of speech, visuospatial functions, and memory are present in an important proportion of patients. As a principal feature of cognitive disorder, deficit of executive functions can be characterised by decreased mental flexibility and inability to cope with changing external conditions. Further, psychiatric complications

[Mental prognosis of Apert syndrome].

BACKGROUND: Mental retardation, considered as common in Apert syndrome could be in part due to associated brain abnormalities. POPULATION AND METHODS: Sixty patients (32 males, 28 females) were included in the study. Patient age at the last examination was over 3 years (mean 10 years, range 3-28 years) in 38 patients. IQ was assessed from psychometric tests adapted for age. Brain anatomy was studied by MRI. Age at operation and quality of familial environment were also evaluated. RESULTS: The IQ was over 70 in 12 patients (32%), over 90 in five (13%) and the mean IQ was 62 (10-114). Thirty p

Mental health of adolescent mothers.

OBJECTIVE: To propose an assessment and intervention model for adolescent mothers that evaluates the influence of physical, cognitive, affective, and interpersonal development on such areas of psychological dysfunction as unprotected sexual activity, substance abuse, and psychopathology. METHOD: More than 100 studies were reviewed to identify correlations between discrete developmental events and evidence of psychological dysfunction in adolescent mothers. RESULTS: The adolescent's nascent developmental skill in one domain of functioning (e.g., cognitive) frequently exerts a synergistic effe

[Psychological trauma and mental disorders].

Fifty-eight female impatients consecutively hospitalized in the University Hospital Department of Psychiatry in Tours were interviewed with a clinician battery of instruments (among them, the SI-PTSD by Davidson). The diagnoses leading to hospitalization were: severe disorders of psychotic type (schizophrenic, schizophreniform, schizo-affective, schizoid and paranoid delusional disorders as well as bipolar disorder), borderline and narcissistic personalities for 7%, and other disorders for 53%. Among the results, we observed that 59% of these patients had experienced at least one major stressf

Alopecia/mental retardation syndrome.

We report on an African-American patient with alopecia universalis, microcephaly, hypogonadism, and mental and growth retardation, and compare his phenotype to others with recessive alopecia/mental retardation syndromes in the literature. Our patient represents the first case reported from nonconsanguineous African-American parents.

Emotion specificity in mental retardation.

The emotion-specificity hypothesis states that mental retardation is associated with deficits in decoding facially expressed emotions that cannot be fully accounted for by MA. Research has demonstrated repeatedly that subjects with mental retardation do not perform as well on emotion recognition tasks as do control subjects but not whether those performance deficits are specific to affective cues. The emotion-specificity hypothesis was tested further with three groups of 16 subjects: adults with mild to moderate mental retardation, similar age adults without mental retardation, and children wi

[Hyperlipoproteinemia in mentally retarded children].

Screening revealed a significantly higher incidence of hyperlipoproteinaemia in mentally retarded children in an Institute for Social Welfare than in the healthy child population (P 1%). In the aetiology participate most markedly secondary influences ensuing in particular from complications of the basic diagnosis of oligophrenia. It is mainly a question of the action of antiepileptics, neuroleptics, the influence of viral hepatic infections and other endogenous and exogenous factors. Early diagnosis of hyperlipoproteinaemia is possible so far only by biochemical methods. It is essential for ge

Elective mutism and mental retardation.

The association of elective mutism and mental retardation is noted. Two cases of elective mutism associated with mental retardation are presented. Both individuals had long-standing patterns of inhibition, shyness, and anxiety manifested by general refusal to talk outside the home, particularly at school. The importance of assessment of intellectual functioning and linguistic skills for the establishment of intervention priorities and clinical management is discussed. The possibility of the presence of mental retardation in individuals with elective mutism should be considered.

Mental impairment in the West Midlands.

The case records of 55 people from the West Midlands (UK) fulfilling the Mental Health Act 1983 criteria for mental impairment or severe mental impairment, were studied. Most were young men with mild mental retardation. 73 per cent were resident in (or on leave from) mental handicap hospitals, and 27 per cent resident in special hospitals. 29 per cent were subject to a Restriction Order. Most had lived in hospital for more than six years. The commonest problem behaviours were aggression, property offences and inappropriate or offending sexual behaviour. 31 per cent were mentally ill or had a p
