[Therapy of osteomyelitis in the metacarpal region with local pedicle m. interosseus dorsalis II and III-flap-plasty. Anatomic principles and case report].

Both experimental and clinical studies showed that muscle flap transposition is a reliable method for treating chronic infections of the bone. Advantages of muscle flaps are: treatment of infection with well perfused autogenous tissue obviating the use of implants, cure of infection usually within four weeks and consequently, short immobilisation time. Disadvantages of muscle flaps are loss of muscle function with possible donor-site morbidity. The first dorsal interosseous muscle and the abductor digiti minimi muscle have been used successfully for the treatment of osteomyelitis of the metaca

Symbrachydactyly involving both the hand and foot. A report of two cases.

Symbrachydactyly involving both the hand and foot is extremely rare. Two such cases are described. The first patient was a two-month-old female with symbrachydactyly of the left hand and foot. The second patient was a one-year-old female with symbrachydactyly of the right hand and foot. No other abnormalities were identified. The diagnosis of this congenital anomaly is discussed and the literature reviewed.

Digital lengthening in congenital hand deformities.

12 hands with congenital short finger in 11 patients were treated with various types of metacarpal bone lengthening. These included three patients with brachydactyly, seven with transverse deficiency, and one with constriction ring syndrome. All cases involved metacarpal lengthening and surgery was performed 16 times in 15 digits. Single-stage lengthening was performed in seven cases, on-top plasty in three cases, and distraction lengthening in six cases. The length gained ranged from 2 to 10 mm in single-stage lengthening, 3 to 17 mm in on-top plasty, and 12 to 30 mm in distraction lengthenin

Functional fracture bracing in metacarpal fractures: the Galveston metacarpal brace versus a plaster-of-Paris bandage in a prospective study.

A total of 133 patients with fractures of the second through the fifth metacarpal bones were randomized to receive either a functional brace (the Galveston metacarpal brace) or a dorsal/ulnar plaster cast. Only 42% of the patients in the metacarpal-brace group completed the treatment, in contrast to 81% of the patients in the plaster-cast group. Complications with the brace were due to 60% of the exclusion. No difference according to gender, age, fracture type, hand affected (right/left), or mechanism of injury was observed between the patients who completed the treatment and those who were ex

Hand reconstruction with allograft demineralized bone: twenty-six implants in twelve patients.

A long-term study of 26 phalangeal or metacarpal defects that were reconstructed with allogeneic demineralized bone implants demonstrates healing comparable to that which follows autogenous bone grafting. Average follow-up was 54 months. Five patients had multiple enchondromas (Ollier's syndrome), five children had congenital hand deformities, and all of these had previously had bone grafts harvested for associated craniofacial reconstructions. With the use of demineralized bone implants, tourniquet and operative times were significantly reduced and potential donor site morbidity was eliminate

[Fractures of the mid-hand area--classification, management, results and problems].

Of the various types of hand fractures, metacarpal fractures occur quite frequently, predominantly in younger people. In order to maintaining hand function man's most important tool, the treatment of coice in recent years has shifted from predominantly conservative measures to more surgical procedures. From 1980 to 1988, 33 patients with metacarpal fractures were treated: 66% received surgical and 34% conservative treatment. We report on the results as regards hand function, the length of unemployment, the patient's subjective complaints and the peri- and postoperative complications.

Treatment of unstable metacarpal and phalangeal fractures with tension band wiring techniques.

The techniques of tension band fixation are applied in the treatment of metacarpal and phalangeal fractures. The technique uses standard, readily available materials. The variability of the fixation techniques allows for secure fixation of virtually any long bone fracture in the hand, even when other methods are not applicable. Nonunion, malunion, loss of fixation, infection, or tendon rupture have not occurred. The technique allows for early active motion and return to full activity usually within for to six weeks. Most patients achieve full painless motion in that time.

Overlapping of fingers due to malunion of a phalanx corrected by a metacarpal rotational osteotomy--report of two cases.

Overlapping of fingers on flexion of the hand caused by malunion of a phalanx was corrected in two patients by a rotational osteotomy of the base of the corresponding metacarpal, avoiding a more delicate operation on the phalanx, with good results.

Translunate, transmetacarpal, scapho-radial fracture with perilunate dislocation. A case report.

A case translunate, transmetacarpal, scapho-radial fracture with perilunate dislocation occurred as a young man drove his motorcycle into the side of a car. Closed reduction was performed initially. Open reduction was performed with a screw in the lunate. Eighteen months later the screw was removed and after two and a half years x-rays revealed no signs of avascular necrosis or arthrosis. The patient fully recovered. This case stresses the necessity of open reduction in cases of complicated carpal fracture dislocations.

Growth arrest recovery lines in hypoparathyroidism.

Dense lines paralleling vertebral end plates with a "bone-in-bone" appearance were seen on radiographs of five patients with idiopathic hypoparathyroidism. The lines are similar to those seen in cases in which there is recovery from a growth arrest. The cause of the lines is unclear and may be related not only to the metabolic process itself, but also to its complications and treatment. The finding, although nonspecific, should bring the possibility of idiopathic hypoparathyroidism to mind.

Carpometacarpal dislocations.

Dislocations at the carpometacarpal joints are uncommon and can be overlooked due to the subtle nature of the radiographic findings. The diagnosis requires a proper understanding of the normal radiographic anatomy of the carpometacarpal joints. To aid in our understanding of the injury, anatomic dissections of the hand were performed. From these, we drew several conclusions about the mechanism of injury. Diagnostic criteria were then developed and applied in the review of a series of CMC dislocations. The incidence and diagnostic characteristics are presented. A systematic approach emphasizing

[Treatment of intra-articular metacarpal and phalangeal fractures with an external fixation minidevice].

Open reduction of intraarticular fractures involving the metacarpals and phalanges and fixation with the small external fixation apparatus is a promising new method which fulfills a need. Full return of function in seven patients and a small limitation of function in three patients can be regarded as a satisfactory result. All patients returned to work after a short time without any complications. The external mini-fixation device which has already been useful in the treatment of open and closed fractures, infected nonunions and in performing arthrodeses has now found another application in th

Periosteal transection for correction of angular limb deformities in foals.

Seventeen foals with angular limb deformities of the distal end of the radius (valgus: 26 limbs), distal end of the tibia (valgus: 6 limbs), and distal end of the 3rd metacarpal bone/metatarsal bone (varus: 4 limbs) were treated surgically by means of hemicircumferential division of the periosteum proximal to the involved growth plate, at the concave side of the bone. There was some correction in all cases. The best results were seen in the radius, where 22 of 25 limbs were corrected to a deviation of 4 degrees or less. Satisfactory correction was obtained in the 6 tibias, the 2 third metacarp

Benign osteoblastoma as a cause of osteomalacia. A report of two cases.

Two patients, a Japanese man and woman both aged eighteen, developed symptoms and signs of osteomalacia over a period of five years. Each of them had a benign osteoblastoma, one in the right fourth metacarpal and the other in the uppermost third of the right humerus. Resection of the tumours without any treatment by vitamin D resulted in rapid cure of the osteomalacia. Attempts to prove a phosphaturic humoral substance or vitamin D antagonist in the osteoblastoma of the humerus were unsuccessful, probably due to prompt excretion from the tumour cells.

Osteoporosis after oophorectomy for non-malignant disease in premenopausal women.

The role of oophorectomy in the development of osteoporosis was assessed retrospectively in 258 women who had been hysterectomized premenopausally for non-malignant disease. Bone density was assessed using the radiographic density of the third metacarpal. Oophorectomy before the age of 45 years was found to be associated with a significantly increased prevalence of osteoporosis within three to six years of operation. The bone density of women oophorectomized after the age of 45 years was indistinguishable three to six years after operation from that found in healthy women with intact ovaries.

Cryptodontic brachymetacarpalia.

Cryptodontic brachymetacarpalia is presumably a new syndrome comprising a) shortened metacarpals and metatarsals, b) short terminal thumbs. c) short straight clavicles, d) multiple impacted teeth and e) autosomal dominant transmission. Affected individuals have a marked clinical similarity to patients with pseudohypoparathyroidism, but in cryptodontic brachymetacarpalis the somatic and mental growth is normal, cataracts and ectopic calcification are absent and there are no evident abnormalities in serum calcium, phosphorus or response to parathormone. A family is presented with proven male-to-
