[Arthroscopic treatment of painful heel syndrome with radio-frequency].

OBJECTIVE: To evaluate therapy effect of radiofrequency in the treatment of painful heel syndrome under arthroscopy. METHODS: From January 2006 to December 2011, 13 patients with painful heel syndrome being admitted into our hospital were studied. There were 5 males and 8 femals, ranging in age from 35 to 68 years. All these surgeries were performed under local anesthesia. After constructing a man-made lacouna in the subcutaneous tissue above plantar fascia, a medial and a lateral portal were established on both sides of plantar fascia and at the anterior edge of calcaneal tuberosity. Foot and ankle injuries in dance.

This review focuses on many of the foot and ankle injuries commonly seen among dancers. These unique athletes place extreme demands on their musculoskeletal system and thereby face a variety of acute and overuse injuries. Conservative treatment is successful in the majority of cases, but these patients often continue to dance while healing--commonly prolonging and at times complicating treatment. When surgery is being contemplated, the dancer's performance level and expectations about returning to dance after surgery should be thoroughly explored. Foot and ankle surgeries that routinely yield

A Less Common Anatomical Variant of Bilateral Multiple Lesser Metatarsal Sesamoids With Radiologic and Clinical Correlation.

Accessory or supernumerary bones are bones that are not normally present in the body but can be found as an anatomical variant. While halluc … vement after six months. Lesser metatarsal sesamoids are usually asymptomatic but can contribute to metatarsalgia. Recognizing these ossicles as anatomical variants is essential to avoid misdiagnosis and unnecessary interventions. This case underscores their potential role in forefoot pain and diagnostic challenges.

Distally based capsule-periosteum interpositional arthroplasty for hallux rigidus. Indications, operative technique, and short-term follow-up.

Twelve patients (15 feet) with severe hallux rigidus underwent distally based capsule-periosteum interpositional arthroplasty of the first m … ral radiographs. None of the patients developed a hallux hammer toe or extensus deformity or lesser metatarsalgia, and none required further surgical intervention. After describing the indications of the procedure and the surgical technique, the authors compare the results with those of the various other procedures available for the surgical treatment of hallux …

Plantar vein thrombosis provoked by mechanical strain to the foot: a rare cause of plantar heel pain.

A 61-year-old mountain hiker presented with acute pain of the medial-plantar aspect of the right foot. As the location, intensity and acute onset of the pain were atypical for fasciitis plantaris, an ultrasound was performed. This demonstrated a segmental plantar vein thrombosis (PVT), which was confirmed on consequent MRI. Patient was a non-smoker, without a medical or family history of coagulopathies. Four months previously, he suffered from metatarsalgia of the left, thus contralateral, foot, which prompted treatment with bilateral orthoses. In addition to metatarsal padding, medial-arch su…

A new concept for an old pain: "Carpalgia"; pain in the wrist.

OBJECTIVE: The idea of this article is to propose to the general use of a new term to semiologically describe musculoskeletal wrist pain. FINDINGS: The use of terms with Greco-Latin roots is common in the medical field. The result is expressed in the newly coined word (neologism) "carpalgia", defined as musculoskeletal pain in the region of the wrist. CONCLUSIONS: The use of other terms related to musculoskeletal pain of the shoulder (omalgia), knee (gonalgia), metatarsus (metatarsalgia), spine (cervicalgia, lumbalgia), or hip (coxalgia) terms have demonstrated their importance through their…

Contribution of a new electrophysiologic test to Morton's neuroma diagnosis.

BACKGROUND: Morton's neuroma causes metatarsalgia due to the interdigital neuropathy. The small nerve diameter compromises their evaluation in image studies. To overcome this problem we propose a new electrophysiological test. METHODS: We conducted a prospective case-control study performing a orthodromic electroneurography using subdermal electrodes in controls and patients to assess the validity. Additionally all patients were tested with magnetic resonance. Some patients required surgery and subsequent histological evaluation. RESULTS: The new ENG procedure showed higher sensitivity and s…

Early results of distal metatarsal osteotomy through minimally invasive approach for mild-to-moderate hallux valgus.

A prospective study was performed on 24 patients to evaluate the early results of our experience with minimally invasive distal metatarsal o … 10 and 5.40, respectively (p < 0.001). No cases showed nonunion, malunion, overcorrection, transfer metatarsalgia or osteonecrosis. The technique appeared in this study to be an effective, safe and simple option for correction of a painful mild-to-moderate hallux valgus deformity.

Modified Mitchell osteotomy for hallux valgus.

From 1988 to 1995, 96 patients (161 feet) underwent a modified Mitchell distal metatarsal osteotomy performed for mild-to-moderate hallux va … nfections, two delayed unions, and two correction losses. The most common late sequela was transfer metatarsalgia of the lesser toes, which occurred in 20 feet (12.4%), leading to some dissatisfaction. The Mitchell osteotomy can be used on cases with less than 20 degrees of intermetatarsal angle, offering a stable construct with easy postoperative care.

A quantitative assessment of the effect of metatarsal pads on plantar pressures.

The treatment of metatarsalgia secondary to the plantar prominence of the metatarsal heads has lead to the development of numerous orthotic devices designed to decrease pressures in these areas. In spite of the considerable cost of some of these devices, there has yet to be much objective evidence of their efficacy. This study assessed the effect of a simple metatarsal pad on pressures transmitted to the metatarsal heads. Quantitative measurements of dynamic peak pressures for 10 asymptomatic subjects with and without metatarsal pads were made using the pedobarograph. Female volunteers had a r…

[Helal's metatarsal osteotomy. Indication and technic with reference to shape and function of the foot].

Metatarsal osteotomy according to Helal is a successful method of treating metatarsalgia. Using this method, 72% of our patients, followed up between 1974 and 1986, (114 patients with a total of 336 osteotomies) were relieved of pain, the result depending on the patients' age. On reviewing the unsuccessful results using pedobarographic measuring, the major part of failures was due to an unequal distribution of weight on the forefoot. Two main types of faulty weight-bearing were stated: 1. Excessive weight-bearing on non-osteotomized adjacent metatarsals (domino-effect). In concern of the overa…

Amputation of the great toe.

Four patients who had undergone reimplantation of the great toe to create a thumb were studied. The follow-up period ranged from 29 to 62 months. In all cases, the great toe was disarticulated at the metatarsophalangeal joint. The patients felt their feet functioned at approximately 85% of normal, and none had any significant complaint of metatarsalgia. The one patient who participated in sports after the surgery noted that it was difficult to "push off" moving away from the involved foot. X-ray examination demonstrated retraction of the sesamoids, which accounted for the increased plantar fle…

Sclerosing alcohol injections for the management of intermetatarsal neuromas: A systematic review.

An intermetatarsal neuroma is a plantar digital neuritis causing metatarsalgia of the affected inter-metatarsal space. At present the evidence to support the management of the condition is poor with only some quality evidence supporting the short-term management of intermetatarsal neuromas using steroid injections. Some authors have supported the use of alcohol sclerosing intra-lesional injections to treat intermetatarsal neuromas. Following a search of the evidence 11 articles were identified. The systematic review found that alcohol injections appear to be safe although some papers report a …

[Anatomical reconstruction of malunited metatarsals and adjacent joints].

BACKGROUND: Malunited fractures of the metatarsals seldom need correction because the adjacent joints proximally and distally are not axially loaded but they may cause significant pain when a subcapital fracture is malunited too far in a plantar direction. METHOD: Even if a metatarsal head is malunited too dorsally the neighboring head signals transfer metatarsalgia. Therefore, reorientation osteotomy with the intraoperative help of a minidistractor and stable fixation with a small locking plate is needed. Painful nonunion, especially of the proximal fifth metatarsal needs improvement of the …
