Implementation of Deployment Readiness Training for Gynecologic Care.

INTRODUCTION: Gynecologic care remains a critical but under-addressed component of deployed readiness. Despite the growing number of deployed female service members and the prevalence of reproductive health issues in theater, hospital corpsman receive little to no formal training in women's health topics. This study aimed to develop and implement a brief, reproducible training intervention to improve corpsman knowledge of common gynecologic concerns encountered during deployment. MATERIALS AND METHODS: A 30-minute educational presentation and 15-question assessment were developed by OB/GYN fa

ATTENTIONAL BIAS TEMPORAL DYNAMICS PREDICT POSTTRAUMATIC STRESS SYMPTOMS: A PROSPECTIVE-LONGITUDINAL STUDY AMONG SOLDIERS.

BACKGROUND: Attentional bias (AB) to threat is thought to play a key role in the development and maintenance of posttraumatic stress symptomatology (PTS). Empirical evidence though is inconsistent. Some studies report associations between AB towards, threat and PTS; other studies report associations between AB away from threat and PTS; yet other studies fail to find any association. We propose that prospective-longitudinal study of AB as a dynamic process, expressed from moment to moment in time, may help to understand these mixed findings and the role of AB in PTS. METHODS: We tested cross-s

Postdeployment resilience as a predictor of mental health in operation enduring freedom/operation iraqi freedom returnees.

BACKGROUND: Much of the research on the impact of trauma exposure among veterans has focused on factors that increase risk for mental health problems. Fewer studies have investigated factors that may prevent mental health problems following trauma exposure. This study examines resilience variables as factors that may prevent subsequent mental health problems. PURPOSE: To determine whether military service members returning from Afghanistan and Iraq who exhibit higher levels of resilience, including hardiness (encompassing control, commitment, and challenge), self-efficacy, and social support

Level of perceived social support, and associated factors, in combat-exposed (ex-)military personnel: a systematic review and meta-analysis.

PURPOSE: Combat deployment increases exposure to potentially traumatic events. Perceived social support (PSS) may promote health and recovery from combat trauma. This systematic review and meta-analysis aimed to synthesize studies investigating the level of PSS and associated factors among (ex-)military personnel who served in the Iraq/Afghanistan conflicts. METHODS: Five electronic databases were searched in August 2023 and searches were restricted to the beginning of the Iraq/Afghanistan conflicts in 2001. The search was conducted according to the Preferred Reporting Items for Systematic Re

Effectiveness of a specific trauma training on war-related truncal injury management: A pre-post study.

BACKGROUND: Non-Compressible Torso Hemorrhage (NCTH) is the leading cause of preventable death in combat casualty care. To enhance the French military surgeons' preparedness, the French Military Health Service designed the Advanced Course for Deployment Surgery (ACDS) in 2008. This study evaluates behavioral changes in war surgery practice since its implementation. METHODS: Data were extracted from the OPEX® registry, which recorded all surgical activity during deployment from 2003 to 2021. All patients treated in French Role 2 or 3 Medical Treatment Facilities (MTFs) deployed in Afghanistan,

The Role of Military Plastic Surgeons in the Management of Modern Combat Trauma: An Analysis of 645 Cases.

BACKGROUND: Plastic surgery has historically been linked to war. Between 2008 and the end of combat operations in Afghanistan in 2014, British military plastic surgeons formed part of the multinational military surgical team at the Role 3 Medical Treatment Facility, Camp Bastion, Helmand Province. The present study aimed to analyze the activity of these surgeons objectively and to determine the utility of their deployment. METHODS: Data were gathered prospectively from four periods (2009 to 2012). This coincided with different surgeons, types of combat activity, wounding patterns, and mission

Association of Child Maltreatment and Deployment-related Traumatic Experiences with Mental Disorders in Active Duty Service Members and Veterans of the Canadian Armed Forces: Association de la Maltraitance des Enfants et des Expériences Traumatisantes Liées au Déploiement Avec les Troubles Mentaux Chez les Membres du Service Actif et Les Anciens Combattants des Forces Armées Canadiennes.

OBJECTIVE: Mental health among military personnel is an important public health priority. It is known that military personnel experience a high prevalence of child maltreatment and deployment-related traumatic events (DRTEs) and both are related to mental health outcomes. However, few, if any, studies have examined the cumulative and interactive effects of child maltreatment and DRTEs on mental health disorders in a sample of active duty service members and military veterans. METHODS: Data were from the Canadian Armed Forces (CAF) Members and Veterans Mental Health Follow-up Survey collected

Thinking Outside the Blood Box: Reimagining the Military Treatment Facility as a Power Projection Platform for Blood Logistics.

Future large-scale combat operations threaten to outpace and overwhelm forward blood supply chains, exposing a critical vulnerability in combat casualty care. Although the emergency use of fresh whole blood and "buddy transfusions" drawn from active-duty service members is a proven, common practice in deployed environments, the military currently underutilizes a massive, untapped resource: the healthy, non-mobilized active-duty population stationed in-garrison. This article proposes a conceptual shift to reimagine stateside military medical treatment facilities as medical power projection plat

Efficacy of Fatigue Countermeasures on Cognitive Performance and Operational Safety in Military Pilots: A Systematic Review.

Fatigue poses a significant risk to operational safety and performance in military aviation due to irregular schedules, night operations, and deployment environments. A range of countermeasures is employed, but their efficacy, specifically for military pilots, requires further exploration. This systematic review evaluated the current evidence on the effectiveness of behavioral, operational, and pharmacological fatigue countermeasures specifically for military pilots, to inform practice and policy. A systematic search of PubMed, Scopus, ScienceDirect, Research Gate, Cochrane Library, Google Sch

Sexual victimization, health status, and VA healthcare utilization among lesbian and bisexual OEF/OIF veterans.

BACKGROUND: Many lesbian and bisexual (LB) women veterans may have been targets of victimization in the military based on their gender and presumed sexual orientation, and yet little is known regarding the health or mental health of LB veterans, nor the degree to which they feel comfortable receiving care in the VA. OBJECTIVE: The purpose of this study was to examine the prevalence of mental health and gender-specific conditions, VA healthcare satisfaction and trauma exposure among LB veterans receiving VA care compared with heterosexually-identified women veterans receiving. Seroepidemiologic survey for Coxiella burnetii among US military personnel deployed to Southwest and Central Asia in 2005.

We used a seroepidemiologic study to estimate Q fever (Coxiella burnetii) seroprevalence, seroincidence, and risk factors for seroconversion in two deployed military populations in 2005. The first study group resided in an area with a known Q fever outbreak history (Al Asad, Iraq). Of this population, 7.2% seroconverted for an incidence rate of 10.6 seroconversions per 1,000 person-months. The second population included personnel transiting through Qatar on mid-deployment leave from southwest/central Asia. In this group, we found 2.1% prevalence with 0.92 seroconversions per 1,000 person-month

Cutaneous and presumed visceral leishmaniasis in a soldier deployed to Afghanistan.

Leishmaniasis has been frequently diagnosed in US military personnel returning from duty in Southwest Asia. The majority of cases have demonstrated cutaneous disease, although a few cases of visceral disease have been documented. We present the case of an immunocompetent, HIV-negative, US Army soldier who suffered both visceral and cutaneous manifestations of leishmaniasis after returning from deployment in Afghanistan. Overlap of cutaneous and visceral involvement is rare and has not been reported in our cohort. Latent Plasmodium vivax infection may have been an exacerbating cofactor. We disc

Outcomes of Patients with Positive Latent Tuberculosis Screening in the San Antonio Military Health System.

INTRODUCTION: Latent tuberculosis infection (LTBI) screening plays a vital role in global efforts to eliminate tuberculosis (TB). While LTBI treatment has been associated with lower rates of reactivation of TB and all military branches screening programs for LTBI screening, there is limited military data on factors associated with screening positivity and treatment completion. MATERIALS AND METHODS: This retrospective study examined positive Quantiferon-Plus (QFT-Plus) assays in the San Antonio Military Health System between January 2022 and June 2024. Electronic medical records were reviewed

An examination of chronotype associations with symptoms of depression, PTSD, and sleep disturbances in US military veterans.

PURPOSE: Chronotype is associated with depression and PTSD, which are common mental health conditions among military veterans. Few studies have examined chronotype in this population, which is known to also have more severe sleep difficulties than non-veteran community-residing adults. METHODS: In the current study, self-reported sleep and mental health symptoms were examined by chronotype using data collected from US military veterans (N = 144) participating in a multi-site post-deployment health repository. Analysis of covariance was used to assess symptom measure differences by chronotype

Impact of Laser Vision Correction on Readiness and Operational Performance Among Active Duty Service Members.

INTRODUCTION: Optimal visual performance is essential for modern military operations. Traditional forms of vision correction, including spectacles and contact lenses, may present operational limitations during deployment or field training. The Department of Defense established the Warfighter Refractive Eye Surgery Program (WRESP) to provide laser vision correction (LVC) to improve operational readiness by reducing reliance on corrective eyewear among active duty service members (ADSM). This study evaluated ADSMs' perceptions of overall satisfaction, visual outcomes, operational performance, an

A Systems Science Approach to Understanding Polytrauma and Blast-Related Injury: Bayesian Network Model of Data From a Survey of the Florida National Guard.

We sought to further define the epidemiology of the complex, multiple injuries collectively known as polytrauma/blast-related injury (PT/BRI … t of deployment per se. Blast exposure, not mild traumatic brain injury (TBI), acted as the primary military deployment-related driver of PT/BRI symptoms. Blast exposure was indirectly linked to mild TBI via other deployment-related traumas and was a significant risk for a high level of posttraumatic stress disorder (PTSD) arousal symptoms. PTSD arousal symptoms and …

Navigating fragmented and contingent support: A qualitative study of British Armed Forces families raising neurodivergent children.

British Armed Forces families raising neurodivergent children must coordinate health, education, welfare and informal support within a service context characterised by mobility and deployment. This study examined how families understood, accessed and sustained support, and how these processes shaped caregiving and family life. Twelve parents and caregivers of children aged 3-17 years with diagnosed or suspected neurodevelopmental conditions participated in online semi-structured interviews. Guided by critical realism, data were analysed using reflexive thematic analysis. The analysis showed th

Numerical modeling and analysis of neck injury induced by parachute opening shock.

Neck injuries from parachute opening shock (POS) are a concern in skydiving and military operations. This study employs finite element modeling to simulate POS scenarios and assess cervical spine injury risks. Validated against various conditions, including whiplash, the model replicates head/neck kinematics and soft tissue responses. POS simulations capture body/head motions during parachute deployment, indicating minimal risk of severe neck injuries (Abbreviated Injury Score/AIS ≥ 2) and low risk of soft tissue tears. Vertebral stress analysis during a rougher jump highlights high stress at
