Pre-surgery Stress Monitoring Using Heart Rate Variability Measures.

Pre-surgery stress is common in patients hospitalized for undergoing surgeries. High levels of stress could prolong post-operative recovery time, increasing the duration of hospitalization. Abnormally high stress levels could sometimes have irreversible impacts, leading to post-operative physiological and psychological disorders. Continuous monitoring of patients during the pre-operative period could help in taking necessary measures to control the stress levels. Electrocardiogram (ECG) is one of the signals which is usually monitored continuously for patients in clinical settings. The usabili

Clozapine-induced cardiomyopathy and myocarditis monitoring: A systematic review.

The use of clozapine requires monitoring the absolute neutrophil count because of the risk of agranulocytosis, but other potentially fatal adverse events associated with clozapine (specifically, myocarditis and cardiomyopathy) do not have mandatory procedures. We performed a systematic review of English-language articles to synthesize an evidence-based approach for myocarditis and cardiomyopathy monitoring. Articles published from January 1988 through February 2017 were identified through a search of Ovid MEDLINE, Ovid Embase, Ovid Cochrane Database of Systematic Reviews, Web of Science, Scopu

The Spectrum of Ambulatory Electrocardiographic Monitoring.

Since its introduction as a clinical investigative tool, the 12-lead electrocardiograph (ECG) has been the gold standard for recognition of cardiac arrhythmias. The resting 12-lead ECG, however, gives only a rhythm snapshot in time, whereas arrhythmias maybe short-lived, paroxysmal and even asymptomatic making documentation in many patients very difficult. To overcome this, ambulatory ECG monitoring has been developed as a means of recording the ECG in patients over a set period of time, whether it be short-, medium- or long-term. With the miniaturisation of recording devices and advances in s

A Behaviour Monitoring System (BMS) for Ambient Assisted Living.

Unusual changes in the regular daily mobility routine of an elderly person at home can be an indicator or early symptom of developing health problems. Sensor technology can be utilised to complement the traditional healthcare systems to gain a more detailed view of the daily mobility of a person at home when performing everyday tasks. We hypothesise that data collected from low-cost sensors such as presence and occupancy sensors can be analysed to provide insights on the daily mobility habits of the elderly living alone at home and to detect routine changes. We validate this hypothesis by desi

Technical note: Accuracy of an ear tag-attached accelerometer to monitor rumination and feeding behavior in feedlot cattle.

Early identification of sick cattle increases treatment success and decreases mortality. Continuous automated records of behavior can be used to identify sick cattle early in the disease process. The objective was to evaluate accuracy of an ear-attached accelerometer (SensOor) that quantified ear movements and estimated feeding and rumination time through a proprietary algorithm. Accelerometers were attached to the ear tag of 18 steers with an initial mean BW of 326 ± 46 kg. The manufacturer's proprietary software was used to determine time spent "feeding," "ruminating," "active," and "resting

Ten good reasons why everybody can and should perform cardiac ultrasound in the ICU.

Critical care ultrasonography (CCUS) has been defined as an ultrasound evaluation of the heart, abdomen, pleura and lungs at the bedside by the intensivist, 24/7. Within CCUS, critical care echocardiography (CCE) is used to assess cardiac function and more generally haemodynamics. Experts in haemodynamics have published a 'consensus of 16' regarding an update on haemodynamic monitoring. They reported the ten key properties of an 'ideal' haemodynamic monitoring system, which perfectly match the ten good reasons we describe here for performing CCE in critically ill patients. Even though unfortun

Technical note: Monitoring grazing bites and walking activity with pedometers.

Three types of pedometers installed on loosely fitted neck collars were investigated to determine the accuracy with which the devices measured the number of grazing bites performed by cows. The pedometer memory stored the summed number of bites over 1-h intervals for up to 10d, and the battery had a life of more than 3 mo. The values recorded by the pedometers were linearly related to the number of bites measured by visual observation and were unaffected by rumination. The correlation coefficients between the pedometer values and the number of bites were all >0.9. Circadian and day-to-day vari

Low-cost remote patient monitoring system based on reduced platform computer technology.

Telemedicine is a well-accepted method providing healthcare benefits to people over long distances. However, in normal telemedicine practices, dedicated complex hardware and network backbones for data collection and communication make the system unintelligible to the common man. Centralization of telemedicine units also makes it accessible only to the immediate surrounding community. In an attempt to address these issues, a study aimed at developing a low-cost remote patient monitoring (RPM) system based on reduced platform computer technology has been carried out. The main focus of the work w

Micro-Raman spectroscopy and univariate analysis for monitoring disease follow-up.

Micro-Raman spectroscopy is a very promising tool for medical applications, thanks to its sensitivity to subtle changes in the chemical and structural characteristics of biological specimens. To fully exploit these promises, building a method of data analysis properly suited for the case under study is crucial. Here, a linear or univariate approach using a R2 determination coefficient is proposed for discriminating Raman spectra even with small differences. The validity of the proposed approach has been tested using Raman spectra of high purity glucose solutions collected in the 600 to 1,600 c

The use of observational scales to monitor symptom control and depth of sedation in patients requiring palliative sedation: a systematic review.

BACKGROUND: Palliative sedation is the intentional lowering of consciousness of a patient in the last phase of life to relieve suffering from refractory symptoms such as pain, delirium and dyspnoea. AIM: In this systematic review, we evaluated the use of monitoring scales to assess the degree of control of refractory symptoms and/or the depth of the sedation. DESIGN: A database search of PubMed and Embase was performed up to January 2010 using the search terms 'palliative sedation' OR 'terminal sedation'. DATA SOURCES: Retro- and prospective studies as well as reviews and guidelines contain

Preliminary evaluation of a novel intraparenchymal capacitive intracranial pressure monitor.

OBJECT: Intracranial pressure (ICP) monitors are currently based on fluid-filled, strain gauge, or fiberoptic technology. Capacitive sensors have minimal zero drift and energy requirements, allowing long-term implantation and telemetric interrogation; their application to neurosurgery has only occasionally been reported. The aim of this study was to undertake a preliminary in vitro and in vivo evaluation of a capacitive telemetric implantable ICP monitor. METHODS: Four devices were tested in air- and saline-filled pressure chambers; long-term capacitance-pressure curves were obtained. Devices

An automated discontinuous venous blood sampling system for ex vivo glucose determination in humans.

BACKGROUND: Intensive insulin therapy reduces mortality and morbidity in critically ill patients but places great demands on medical staff who must take frequent blood samples for the determination of glucose levels. A cost-effective solution to this resourcing problem could be provided by an effective and reliable automated blood sampling (ABS) system suitable for ex vivo glucose determination. METHOD: The primary study aim was to compare the performance of a prototype ABS system with a manual reference system over a 30 h sampling period under controlled conditions in humans. Immune-mediated liver diseases: programmed cell death ligands and circulating apoptotic markers.

Primary biliary cirrhosis, primary sclerosing cholangitis and autoimmune hepatitis are the three major immune-mediated liver diseases. The etiologies of primary biliary cirrhosis, primary sclerosing cholangitis and autoimmune hepatitis are largely unknown, but seem to be influenced by genetic and environmental factors. Autoantibodies can be found in nearly all patients with primary sclerosing cholangitis and autoimmune hepatitis, and in the vast majority of patients with primary sclerosing cholangitis. In addition, autoimmune hepatitis is associated with high concentrations of serum globulins.

Human thermoregulation and measurement of body temperature in exercise and clinical settings.

This review discusses human thermoregulation during exercise and the measurement of body temperature in clinical and exercise settings. The thermoregulatory mechanisms play important roles in maintaining physiological homeostasis during rest and physical exercise. Physical exertion poses a challenge to thermoregulation by causing a substantial increase in metabolic heat production. However, within a non-thermolytic range, the thermoregulatory mechanisms are capable of adapting to sustain physiological functions under these conditions. The central nervous system may also rely on hyperthermia to

Intraabdominal pressure monitoring during fluid resuscitation.

PURPOSE OF REVIEW: Elevated intraabdominal pressure is commonly encountered in the critically ill, has detrimental effects on all organ systems, and is associated with significant morbidity and mortality. Serial intraabdominal pressure measurements are essential to the diagnosis, management, and fluid resuscitation of patients who develop intraabdominal hypertension and/or abdominal compartment syndrome. RECENT FINDINGS: Over the past year, the importance of intraabdominal pressure measurements and their accurate determination has been further defined. Several nonoperative therapies have been

Management of intracranial hypertension.

Effective management of intracranial hypertension involves meticulous avoidance of factors that precipitate or aggravate increased intracranial pressure. When intracranial pressure becomes elevated, it is important to rule out new mass lesions that should be surgically evacuated. Medical management of increased intracranial pressure should include sedation, drainage of cerebrospinal fluid, and osmotherapy with either mannitol or hypertonic saline. For intracranial hypertension refractory to initial medical management, barbiturate coma, hypothermia, or decompressive craniectomy should be consid

Hemodynamic monitoring using PiCCO system in a 10 months old infant suffering from serious burn injury.

In this study, authors present a case report of a 10 months old patient with burn injuries involving 20.5 % body surface area, grades 2a and 2b. According to the continuous monitoring of hemodynamic parameters by PiCCO (Pulsion Medical Systems, Munich, Germany), fluid therapy was successfully managed. Despite of an increasing EVLWI (extravascular lung water index) it was possible to preserve the child from lung edema and subsequent artificial lung ventilation by the early targeted therapeutic interventions. Moreover, the study analyzes the possibilities and indications of less invasive hemodyn

Endpoints of resuscitation: what should we be monitoring?

Major resuscitation goals in the management of shock include restoration of adequate tissue perfusion and oxygen balance and normalization of cellular metabolism. Identification of the most appropriate endpoints of resuscitation is difficult and often debated in the literature. Traditional endpoints, such as heart rate, blood pressure, mental status, and urine output are useful in the initial identification of inadequate perfusion, but are limited in their ability to identify ongoing, compensated shock. Many clinicians continue to use these parameters as indicators that systemic oxygenation im

Hereditary spherocytosis is more frequent than expected: what to tell the patient?

Modern double beam laser technique allows screening for hereditary spherocytosis in the course of routine hematology. An incidence of 1:150 men and 1:800 women has been determined. The anomaly is symptomless in the majority of the cases. This explains the discrepancy between our values and the incidence of 1:5,000 reported in the literature. The diagnosis of hereditary spherocytosis should be reported to the physician and the patient, as it may be wayleading in case of unexpected, unspecific complications such as anemia, jaundice, cholelithiasis, liver cell damage and iron overload.
