BACKGROUND: The prevalence and clinical relevance of incidental findings (IF(s)) on imaging assessing the pelvis in children has not been well documented.
METHODS: Three-thousand two-hundred thirty-one children (mean age 10.2 (range 8.6-12.9) years) were evaluated with MRI of the hips, pelvis, and lumbar spine, as part of a prospective population-based pediatric cohort study. Scans were reviewed by trained medical staff for abnormalities. IFs were categorized by clinical relevance and need for further clinical evaluation.
RESULTS: 8.3% (n = 267) of children featured at least one IF. One or m

PURPOSE: To retrospectively evaluate the potential utility and accuracy of magnetic resonance (MR)-digital subtraction angiography (DSA) as a preprocedural imaging modality for transcatheter arterial microembolization (TAME) in musculoskeletal pain management, compared with conventional DSA.
MATERIALS AND METHODS: Eleven patients with various musculoskeletal conditions underwent MR-DSA and DSA before TAME. Image evaluation was performed on 33 regions of interest across 11 patients. Three experienced readers evaluated abnormal enhancement (stains) using a 4-point scale (0-3). Interobserver agr

In order to assess the long term effect of treatment for thyroid dysfunction on musculoskeletal symptoms, 102 patients were traced by a computer search; these patients visited two outpatient clinics of rheumatology and were known to have thyroid disorders. Of these 102 patients, 58 patients met the study criteria of having abnormal thyroid function tests at first visit to the outpatient clinic; 46 patients participated in the study. They were interviewed with a structured questionnaire about their past and current musculoskeletal complaints. The 46 patients (45 females, 1 male) had a mean age

Ten women with temporomandibular joint dysfunction and general joint hypermobility (score, 4 to 8) and 10 symptom-free female volunteers without systemic laxity (score, 0 to 2) were selected for the study. A biopsy of connective tissue from arm skin found that the total collagen concentrations were lower and the proteoglycan values were higher in the hypermobile TMJ patients than in the control subjects. The mitral region of the heart was inspected by echocardiography. Eight patients and four controls had slightly abnormal echocardiographic findings. Two patients fulfilled the criteria for mit

In a detailed investigation of a family with musculoskeletal abnormalities observed in four generations, the proband and his mother were found to have central cores and multicores on histochemical and electron microscopic studies of biopsied muscle. A male sibling experienced a malignant hyperthermic reaction during hand surgery, and although similar reactions had not been observed in other family members, unexplained ventricular dysrhythmias did occur in the proband during surgery. The proband and his mother were subsequently shown to be susceptible to malignant hyperthermia by abnormal in vi…

RATIONALE: Sternal cleft is a rare congenital chest wall defect resulting from failed fusion of the sternal bars during embryogenesis. Primary closure is often feasible in the neonatal period due to thoracic wall compliance, but delayed presentations require complex reconstruction using autologous tissues, muscle flaps, and/or biomaterials. PATIENT CONCERNS: A two-year-old male presented with a visible anterior chest wall defect and progressive respiratory distress. DIAGNOSES: Physical examination revealed an isolated superior sternal cleft with visible cardiac pulsations beneath the skin. Thr

Overgrowth syndromes are characterized by global or localized disproportionate growth associated with other anomalies, including vascular malformations and neurological and/or visceral disorders. CLOVES (Congenital Lipomatous asymmetric Overgrowth of the trunk with lymphatic, capillary, venous, and combined-type Vascular malformations, Epidermal naevi, Scoliosis/Skeletal and spinal anomalies) is an overgrowth syndrome caused by mosaic activating mutation in gene PIK3CA, which gives rise to abnormal PI3K-AKT-mTOR pathway activation. These mutations are responsible for the clinical manifestation

We report on two sibs with a lethal form of bone dysplasia with distinctive skeletal findings including rhizomelic and mesomelic limb shortening, hooked clavicles, dumbbell femurs, and absence of talus and calcaneus ossification. Other clinical features include Dandy-Walker malformation, congenital heart defects, joint contractures, genital hypoplasia, and distinctive facial features. These sibs appear to have a previously undescribed skeletal dysplasia, which is most likely inherited in an autosomal recessive fashion.

OBJECTIVE: To investigate the effect on the fetus of intrauterine exposure to quinolones in terms of teratogenicity, with special focus on the musculoskeletal system.
METHODS: We studied 38 pregnant women who received quinolones and consulted the Motherisk Program in Toronto from 1989-1992. Perinatal complications, birth weight, birth defects, and developmental milestones, with particular emphasis on the musculoskeletal system, were compared with those of controls matched for both maternal age and indication for antibacterial therapy.
RESULTS: Thirty-five women (92%) treated with norfloxacin

Wolf-Hirschhorn syndrome is a rare genetic disease caused by a chromosomal deletion of the distal short arm of Chromosome 4. It is associated with multisystem abnormalities, including delayed growth, characteristic facial features, epilepsy, and skeletal abnormalities. We report three patients who developed hip displacement, and describe the occurrence of delayed and nonunion in patients who underwent corrective proximal femoral osteotomy for hip displacement. We also performed a literature review identifying common musculoskeletal presentations associated with the condition. Patients with Wol

Beside standard radiographs, bone scintigraphy and MR imaging, ultrasonography plays an important role in assessing abnormalities of the musculoskeletal system. Ultrasonography is widely available, inexpensive and provides high-contrast resolution; however, interobserver variability is a major problem. Ultrasonography should only be employed by experienced examiners and for proven indications in order to prevent inadequate results. Most peer-reviewed publications have evaluated the role of ultrasonography for abnormalities of the rotator cuff and the achilles tendon. Ultrasonography also commo

This study examined sleep patterns in female adolescents with chronic musculoskeletal pain. Twenty-six participants with chronic musculoskeletal pain completed questionnaires during their clinic visit, and three 24-Hour Sleep Patterns Interviews during the following 2 weeks. Compared to normative data (Acebo & Carskadon, 2002), adolescents with chronic pain reported similar total sleep time (TST) and bedtimes. However, study participants reported significantly longer sleep onset latency, more night wakings, a later morning wake time, and more symptoms of daytime sleepiness. Pain improved after

Congenital lipomatous overgrowth, vascular malformations, epidermal nevi, spinal/skeletal anomalies and/or scoliosis syndrome is a PIK3CA-related overgrowth spectrum presenting with congenital, asymmetric, disproportionate overgrowth associated with dysregulated adipose tissue, enlarged bony structures, and mixed primarily truncal vascular malformations. We present this case to raise awareness that very thin body habitus (lipoatrophy) contrasting with areas of overgrowth can be an important clinical feature of this syndrome and, if not recognized, can lead to unnecessary investigations.

The patient is a unique case presenting with presumed Restrictive Dermopathy (RD) and intracranial and adrenal calcifications, an association not previously described in the literature. This case postulates the possibility of additional radiographic features that can be included in the spectrum of RD or as secondary events from the underlying pathophysiology of RD.

Focal dermal hypoplasia (FDH), also known as Goltz syndrome, is a rare condition in which congenital anomalies result in a multitude of defects that affect many systems of the body. These defects can involve the eyes, skin, teeth, and cardiovascular, skeletal, and gastrointestinal systems. There have been many associated abnormalities reported in the literature. An appreciation of the clinical presentation of Goltz syndrome is important because physicians rely heavily on recognition of key physical characteristics in order to help make a diagnosis. In this report, we summarize the clinical fin

A newborn presented to genetics with complex skeletal abnormalities, joint contractures, and bilateral corneal clouding with sclerocornea. The patient survived for 8 months before succumbing to respiratory failure. Exome sequencing revealed a compound heterozygous mutation in theB3GALT6gene. Mutations in this gene have been associated with both Ehlers- Danlos syndrome, progeroid type 2 and spondyloepimetaphyseal dysplasia with joint laxity type 1. These diagnoses encompass the skeletal and joint findings. Our patient expands the phenotype of these diagnoses, as anterior segment eye anomalies h

A cleft of the sternum is a rare congenital anomaly. We report a rare case of complete sternal cleft in a 25-year-old woman. After surgical correction of the pectus excavatum, complete closure by direct approximation of the freshened sternal remnants to the midline, without interposition of tissue graft or inert prosthesis, was achieved in our patient. The functional and cosmetic result was excellent after complete healing.

We present a case of opsismodysplasia, a very rare skeletal dysplasia, in a term newborn female who had short length, short extremities and markedly short fingers. Radiological studies demonstrated severe platyspondyly, absence of epiphyseal ossification centers, short tubular bones, especially severe in hands and feet, with metaphyseal cupping. She also had hydrocephaly, a rare finding in opsismodysplasia. In our literature review we have found 24 cases, 17 born alive and seven terminations of pregnancy (TOPs).
