[Cardiac inotropic reserve examined by postextrasystolic potentiation and redistribution of exercise thallium-201 scintigraphy].

Evaluation of regional contractile reserve and the viability of an infarcted segment of the myocardium is very important in determining the indications for aorto-coronary bypass after myocardial infarction and in predicting the prognosis. Regional wall motion of the left ventricle after postextrasystolic potentiation (PESP) was studied in 18 patients with old myocardial infarction, and compared with indices of redistribution of thallium after exercise. Equilibrium radionuclide angiocardiography (RNA) using Tc99m HSA was performed at rest and after PESP produced by a programmable cardiac stimul

[Two-dimensional echocardiographic recognition of dilated cardiomyopathy: comparison with postmortem studies].

To elucidate the pathophysiology of dilated cardiomyopathy (DCM), the relationship of two-dimensional echocardiographic wall motion abnormalities (asynergy) to histopathological findings was evaluated in autopsied patients including seven with DCM, five with old myocardial infarction (OMI) and three with the normal heart. The DCM cases were classified morphologically in two groups, namely four of type I and II and three of type III, according to Shozawa's classification. Three short-axis views of the left ventricle were divided into 19 segments; the wall motion was assessed visually and classi

[Unstable angina pectoris: indication for coronary angiography].

When a patient continues to have anginal attacks despite optimal medical therapy including intravenous nitroglycerin and balloon counterpulsation, unstable angina pectoris becomes a surgical disease. Coronary angiography must be performed as rapidly and as efficiently as possible. If the signs and symptoms of acute myocardial ischemia continue, the angiographic procedures should be carried out within 1-2 hours after the onset of symptoms, thus permitting surgical or transluminal revascularization of the most endangered myocardial regions within the acceptable interval of 4 hours. Myocardial pr

Computer analysis of vectorcardiograms in myocardial infarction with special reference to polar vector and planarity of the QRS and T loops.

Spatial characteristics of the QRS and T loops in 110 patients with old myocardial infarction were analysed in comparison with 221 normal subjects. Measured were (1) QRS and T polar vectors, (2) initial 20- and 30-msec segmental QRS polar vectors, and (3) length, width, thickness, and ratios of width/length and thickness/length of the QRS and T loops in edgewise and broadside projections. Broadside and edgewise projections were obtained by transformation of the reference frame of the Frank lead system to a patient's own frame based on the polar vector. The recognition rates of abnormality in t

[Changes in left ventricular function with effort in the coronary patient. l].

The altered haemodynamics of the coronary patient have been investigated in 30 patients both at rest and under conditions of maximal effort as carried out on the bicycle ergometer under the usual conditions for an exercise electrocardiogram. Patients with angina of effort but no previous infarction have normal left ventricular function at rest; under the ischaemia induced by exercise there is acute dysfunction of the left ventricle as witnessed by a reduction in maximal cardiac output, a raised end-diastolic pressure, and changes in compliance which are more marked than those in contractility.

The Clinical Role of Electrocardiographic Morphology of Premature Ventricular Contractions for Prognostic Outcomes in Children.

Background and Objectives: Premature ventricular contractions are among the most common arrhythmias encountered in clinical practice. However, this disorder can be associated with arrhythmia-induced cardiomyopathy or be the first sign of primary myocardial diseases. Certain morphologies of premature ventricular contractions are associated with a higher risk for sudden arrhythmia and cardiac dysfunction in the adult population. There is data on the clinical value and significance of the contraction morphology in adults, but there is a lack of such data for children. Materials and Methods: This

Foetal premature atrial contractions during the second and third trimester are not associated with foetal breathing.

Foetal premature atrial contractions (PACs) are the most commonly encountered and also the most benign foetal arrhythmia. A retrospective cohort study was conducted with the objective to assess whether the presence of foetal breathing was associated with the presence of foetal PACs. A further objective was to evaluate whether this association would affect neonatal outcomes at a high volume referral centre. The diagnosis of PACs was based on the observation of a premature atrial contraction followed by a ventricular contraction on ultrasound myocardial M-mode. Trained ultrasonographers document

Mechanism of action and systemic and regional hemodynamics of the potassium channel activator BRL34915 and its enantiomers.

BRL34915 (BRL) is a vasodilator with a novel structure. Its mechanism of action, its effects on depolarization-induced and receptor-mediated blood vessel contraction, and its hemodynamic effects were investigated. In the rat portal vein, BRL inhibited spontaneous mechanical activity [IC50 0.013 +/- 0.001 microM (mean +/- SEM) for (-)-BRL], the initial effect being a reduced frequency of contraction. At higher concentrations, the spontaneous contractions were abolished and 86Rb+ efflux was increased. These results suggest that BRL preferentially acts on the pacemaker cells, the K+ channels in o

Managing Ventricular Wall Rupture as a Complication of Late Presentation MI in a Complex Patient with Ankylosing Spondylitis.

Ventricular free wall rupture is an infrequent but serious complication of myocardial infarction with high mortality despite surgical intervention. In recent years with the COVID-19 pandemic, observational studies have reported a rise in this complication most likely due to patient hesitation in seeking urgent medical assistance for fear of contracting COVID-19 in a hospital setting. This case report highlights the early recognition and diagnosis of ventricular wall rupture by the heart team with a good surgical outcome in a complex patient with ankylosing spondylitis. Ventricular rupture shou

High-intensity interval training improves myocardial work and cardiac function in women with obesity: a randomized controlled study.

The purpose of this study was to investigate the effects of high-intensity interval training (HIIT) and moderate-intensity continuous training (MICT) on cardiac function and myocardial work (MW) in women with obesity. Twenty-five women with obesity (Age: 28 ± 5 years, BMI: 35.03 ± 3.19 kg/m²) were randomized to 8 weeks of either HIIT (n = 11, 4×4-min at 85-95% of HRmax) or MICT (n = 14, 41-min at 65-75% of HRmax). Cardiac function and strains were evaluated using two-dimensional speckle tracking echocardiography. MW indices were computed via dedicated software. HIIT significantly improved myoc

Cardiac Myosin-Binding Protein C in Cardiovascular Disease.

Cardiac myosin-binding protein C (cMyBPC) is an important sarcomeric regulatory protein that plays a key role in myocardial contraction and relaxation. Through its interactions with thick and thin filament proteins, cMyBPC helps regulate cross-bridge cycling and contributes significantly to overall cardiac performance. Alterations in its expression, structure, or phosphorylation state have been associated with several forms of cardiovascular disease. Pathogenic variants in the MYBPC3 gene represent one of the most common genetic causes of hypertrophic cardiomyopathy (HCM). These mutations have…

Challenges and opportunities in improving left ventricular remodelling and clinical outcome following surgical and trans-catheter aortic valve replacement.

Over the last half century, surgical aortic valve replacement (SAVR) has evolved to offer a durable and efficient valve haemodynamically, with low procedural complications that allows favourable remodelling of left ventricular (LV) structure and function. The latter has become more challenging among elderly patients, particularly following trans-catheter aortic valve implantation (TAVI). Precise understanding of myocardial adaptation to pressure and volume overloading and its responses to valve surgery requires comprehensive assessments from aortic valve energy loss, valvular-vascular impedanc

Patient-specific left ventricular hypertrophy under severe hypertension: mechanistic insights from hill-type computational simulations.

Hill's force-velocity relationship is a fundamental property of myocardial contraction, yet its mechanical role in patient-specific left ventricular hypertrophy (LVH) remains underexplored. This study systematically evaluated the consequences of including versus omitting Hill-type velocity dependence in magnetic resonance imaging derived three-dimensional finite element models of three hypertrophic left ventricles subjected to stage 3 hypertensive loading. Incorporating velocity dependence moderated peak systolic pressure, tempered early fibre shortening, sustained mid-systolic deformation, an…

An integrative approach to identify novel miRNA-mRNA interaction networks in LMNA-cardiomyopathy.

Dilated cardiomyopathy caused by variants in the LMNA gene leads to malignant arrhythmogenic events, faster phenotype progression and high risk of sudden cardiac death. The pathophysiological mechanisms triggering disease progression remains poorly understood. We investigated the mRNA and miRNA transcriptome in the myocardial tissue of 50-week-old LMNAR249W mice developing dilated cardiomyopathy. We found 2148 genes and 53 miRNAs that were differentially expressed in LMNAR249W hearts. Gene ontology and pathway enrichments showed that differentially expressed genes were enriched mainly for fatt

Relationship of Inotropic Effects of Stimulation of β1- and β2-Adrenergic Receptors of Isolated Myocardial Fragments with Echocardiography Parameters in Coronary Heart Disease.

We studied the relationship of inotropic responses of the isolated myocardium to stimulation of β1-and β2-adrenergic receptors (β-AR) with echocardiography parameters in 28 patients with coronary heart disease (CHD). Myocardial fragments (trabeculae of the right atrial appendage) were obtained during coronary artery bypass surgery. The inotropic response of the trabeculae was assessed in an isometric mode. Stimulation of β1-and β2-AR with agonists was performed against the background of preliminary α-AR blockade. In case of preserved ejection fraction, significant inotropic response of the tra

Generation of a homozygous MYH7 gene knockout human embryonic stem cell line (WAe009-A-69) using an episomal vector-based CRISPR/Cas9 system.

Myosin heavy chain 7 (MYH7) encodes the human heart myosin heavy chain subunit, which plays an important role in myocardial contraction. MYH7 is the main pathogenic gene that causes Hypertrophic cardiomyopathy (HCM) and Dilated cardiomyopathy (DCM). In this experiment, a MYH7 homozygous knockout human embryonic stem cell (hESC) line, WAe009-A-69, was generated using an episomal vector-based CRISPR/Cas9 system. It can be an ideal tool to further study the function of MYH7. The cell line was confirmed with pluripotency, normal karyotype and trilineage differentiation potential.

[Unmotivated malfunction of implanted cardioverter-defibrillator and ways of its correction].

The study included 149 patients with implanted cardioverters-defibrillators (CD) at high risk of development of ventricular tachyarrhythmia. It was found that unmotivated malfunction of CD occurs in supraventricular tachycardia, when a short episode of ventricular tachycardia spontaneously ends before CD starts to work, when T-wave is mistaken for ventricular contraction, in defective isolation, electrode dislocation. A method to reduce frequency of CD malfunction is proposed. To prevent unmotivated discharges it is recommended to use attendant drug therapy, upgrading equipment and algorithms

Imaging for sudden cardiac death risk stratification: Current perspective and future directions.

Sudden cardiac death (SCD) accounts for one fifth of global deaths, and occurs when a trigger (e.g. myocardial ischemia, premature ventricular contraction) interacts with an arrhythmic substrate (e.g. myocardial scar, dilated cardiomyopathy). Multimodality imaging (echocardiographic, cardiac magnetic resonance and nuclear techniques) can potentially visualize many predisposing substrates and triggers. Implantable cardioverter-defibrillator (ICD) is the most effective approach to primary prevention of SCD, and current guidelines regarding ICD implantation are based on a left ventricular ejectio

Cardiac mechano-electric coupling research: fifty years of progress and scientific innovation.

With its conceptualisation nearly fifty years ago, cardiac mechano-electric coupling (MEC) has developed from a collection of anecdotal reports into a field of research that - in spite of early scepticism - is now an accepted part of cardiac structure-function considerations. Throughout this development, MEC studies have been both driver and beneficiary of technological innovation: from sharp electrode recordings for the study of in situ cell responses to cell isolation and patch clamp; from early approaches to mechanical stimulation of tissue using photographic diaphragms to modern force-leng

Assessment of atrial electromechanical delay and left atrial mechanical functions in patients with psoriasis vulgaris.

OBJECTIVES: Increased frequency of atrial fibrillation (AF) has been demonstrated in psoriasis cases. Prolongation of the duration of atrial electromechanical delay (AEMD) is a well-known characteristic of the atrium, which is vulnerable to AF. In the current study, our aims are to investigate AEMD durations and mechanical functions of the left atrium (LA) in patients with psoriasis. METHODS: A total of 90 patients, 45 with psoriasis vulgaris and 45 as the control group, were included in the study. Atrial electromechanical coupling (PA) and intra- and inter-atrial electromechanical delay (IA-
