The roles of microRNA-22 in myocardial infarction.

Myocardial infarction (MI) is the leading cause of morbidity and mortality worldwide. The regeneration capacity of the adult mammalian heart is very limited, so that the lost cells are replaced by fibrotic scar. This is followed by remodeling of the surrounding myocardium, which includes cardiac hypertrophy and fibrosis, and makes the ventricular wall thicken and stiffen. This adverse cardiac remodeling leads to impaired cardiac function and eventually leads to heart failure. Extensive studies have revealed that microRNAs (miRNAs) play an essential role in cardiovascular diseases. microRNA-22 …

Atrial fibrillation and the risk of myocardial infarction.

IMPORTANCE: Myocardial infarction (MI) is an established risk factor for atrial fibrillation (AF). However, the extent to which AF is a risk factor for MI has not been investigated. OBJECTIVE: To examine the risk of incident MI associated with AF. DESIGN, SETTING, AND PARTICIPANTS: A prospective cohort of 23,928 participants residing in the continental United States and without coronary heart disease at baseline were enrolled from the Reasons for Geographic and Racial Differences in Stroke (REGARDS) cohort between 2003 and 2007, with follow-up through December 2009. MAIN OUTCOMES AND MEASUR…

Myocardial infarction as a rare cause of otalgia.

Aim. To present a case referred to our clinic with severe right ear pain but without any abnormal finding during otological examination and diagnosed as myocardial infarction and also to draw attention to otalgia which can occur secondary to myocardial infarction. Case Report. An 87-year-old female admitted with right ear pain lasting for nearly 12 hours and sweating on the head and neck region. On otolaryngologic examination, any pathological finding was not encountered. Her electrocardiogram revealed findings consistent with myocardial infarction. Her troponin values were 0.175 ng/L at 1 hou…

Myocardial necrosis and infarction in newborns and infants.

PURPOSE: This study aimed to investigate the incidence, and underlying associations, of myocardial necrosis occurring in infants undergoing … perinatal asphyxia, coronary artery abnormalities, or sepsis. Three cases of apparently idiopathic myocardial infarction were identified and are described in detail. CONCLUSIONS: Myocardial necrosis is relatively common in infants who die and undergo autopsy, and is most commonly associated with congenital heart disease and perinatal asphyxia. Myocardial infarction c …

Myocardial infarction and pregnancy: a case report.

Coronary artery dissection is a rare, sometimes fatal traumatic condition, with 80% of cases affecting women. The coronary artery develops a tear, causing blood to flow between the layers, which force them apart. We describe the case of a 35-year-old multiparous woman with acute myocardial infarction secondary to right coronary artery dissection at 32 weeks' gestation. The patient underwent successful primary percutaneous coronary intervention with the placement of 4 coronary stents. She subsequently experienced preterm labor and preeclampsia 11 days following the procedure. Elective cesarean …

Myocardial infarction in pregnancy and postpartum in the UK.

AIM: Cardiac disease is a leading cause of maternal death in the developed world, responsible for one-fifth of all maternal deaths in the UK. The aim of this study was to estimate the incidence of myocardial infarction (MI) in pregnancy and up to one week postpartum in the UK and describe risk factors, management and outcomes. METHODS: A prospective population-based study with nested case control analysis used the UK Obstetric Surveillance System to identify all women in the UK with MI in pregnancy (in the years 2005-2010). A control group of 1360 women was used for comparison. Multivariable …

[Myocardial infarction as complication of acute pancreatitis].

We report a case of an acute pancreatitis complicated by myocardial infarction with normal coronary arteries on angiography. This observation presents a double interest. First, it illustrates unusual forms of coronary disease represented by the non-atherosclerotic infarction. On the other hand, it shows that the multiple visceral damage associated with acute pancreatitis can include myocardial infarction which complications aggravate prognosis.

The obesity paradox and myocardial infarct size.

OBJECTIVE: Obese subjects have a risk of death from cardiovascular disease higher than those with normal body weight. Obese patients, however, have a better outcome when undergoing coronary revascularisation, and when suffering from heart failure or chronic kidney disease. The term 'obesity paradox' underlines the divergence between increased risk and better outcome in sick obese patients. We tested the hypothesis that the obesity paradox could also occur in myocardial infarction. METHODS: A group of 89 patients (mean age 62 +/- 11 years) with previous myocardial infarction (Q-wave in 72 pati…

Microalbuminuria in patients with previous myocardial infarction.

Microalbuminuria (MA) is a cardiovascular risk factor. The aim of this study was to examine the relationship between MA and the intima-media complex of the carotid and brachial artery, endothelial function, glucose metabolism, haemostatic variables and cardiac hypertrophy in patients with coronary heart disease. A total of 123 patients, aged 31-80 years, with a history of previous myocardial infarction and without known diabetes mellitus were examined with B-mode ultrasound of common carotid and brachial arteries, flow-mediated dilatation of the brachial artery and echocardiography. A standard…

Gout and the risk of acute myocardial infarction.

OBJECTIVE: To determine if hyperuricemia and gouty arthritis are independent risk factors for acute myocardial infarction (MI) and, if so, whether they are independent of renal function, diuretic use, metabolic syndrome, and other established risk factors. METHODS: We performed multivariable logistic and instrumental variable probit regressions on data from the Multiple Risk Factor Intervention Trial (MRFIT). RESULTS: Overall, there were 12,866 men in the MRFIT who were followed up for a mean of 6.5 years. There were 118 events of acute MI in the group with gout (10.5%) and 990 events in the…

Implications of the new definition of myocardial infarction.

The new definition of acute myocardial infarction is based primarily on raised troponin levels because of the sensitivity and specificity of these markers and their correlation with the pathophysiology of acute coronary syndromes with plaque fissuring or rupture and embolisation of platelets causing myocyte necrosis. Raised troponin levels are associated with increased risks of death and recurrent myocardial infarction. Greater treatment benefit with low molecular weight heparin, IIb/IIIa antagonists and revascularisation is seen when troponin levels are raised. There are many implications for…

Acute myocardial infarction in the neonatal period.

We describe two neonates presenting with myocardial infarction, due to two different aetiologies of this extremely rare but potentially treatable condition, and discuss the management. One neonate had myocardial infarction complicating enteroviral myocarditis and recovered completely. The second had fatal myocardial infarction due to thrombosis of the left coronary artery. Although rare, the attending paediatrician should have a high index of suspicion when evaluating a neonate with acute onset of collapse. Electrocardiographic changes are diagnostic, but further echocardiographic assessment a…

[The cause of myocardial infarction in pediatric patients].

Heart infarct in children seems to be more common than it is recognized. In the paper we discussed the most frequent causes of heart infarct as: congenital anomalies of coronary arteries (first of all anomalous origin of the left coronary artery), their post-inflammatory and post-operative stenosis, coronary thrombosis and embolism, coronary spasm, metabolic and proliferative diseases, coronary insufficiency, and the other ones (e.g. heart trauma).

[Acetylsalicylic acid after myocardial infarct].

There is no doubt about the positive influence of acetylsalicylic acid on vascular disease. The antithrombotic therapy with acetylsalicylic acid is a fundamental of secondary prevention of vascular events, especially in nonfatal myocardial infarction, instable angina pectoris, stroke and in patients with amaurosis fugax. Clinical studies done since 1990 about the effect of a low-dose therapy confirmed the experimental results, which showed that 25-50 mg acetylsalicylic acid per day are enough, to suppress platelet function as far as necessary, without influencing the protective function of pro…
