The opiate antagonist, naltrexone, in the treatment of trichotillomania: results of a double-blind, placebo-controlled study.

Trichotillomania (TTM) is characterized by repetitive hair pulling resulting in hair loss. Data on the pharmacological treatment of TTM are limited. This study examined the opioid antagonist, naltrexone, in adults with TTM who had urges to pull their hair. Fifty-one individuals with TTM were randomized to naltrexone or placebo in an 8-week, double-blind trial. Subjects were assessed with measures of TTM severity and selected cognitive tasks. Naltrexone failed to demonstrate significantly greater reductions in hair pulling compared to placebo. Cognitive flexibility, however, significantly impro…

[Injectable extended-release naltrexone for the prevention of relapse to opioid dependence following opioid detoxification].

Opioid dependence is a growing, worldwide public health concern. In contrast to opioid ?-agonist (or 'substitution') maintenance treatments, injectable extendedrelease naltrexone (XR-NTX), approved in the USA and Russia, is an opioid antagonist, formulated to address nonadherence, which limits the utility of oral naltrexone for opioid dependence. This article reviews the clinical trial data underlying the approval of XR-NTX for opioid dependence and the agent's clinical use. XR-NTX met all primary and secondary end points in a multicenter, placebo-controlled trial (n=250) conducted in Russia, …

The use of low-dose naltrexone (LDN) as a novel anti-inflammatory treatment for chronic pain.

Low-dose naltrexone (LDN) has been demonstrated to reduce symptom severity in conditions such as fibromyalgia, Crohn's disease, multiple sclerosis, and complex regional pain syndrome. We review the evidence that LDN may operate as a novel anti-inflammatory agent in the central nervous system, via action on microglial cells. These effects may be unique to low dosages of naltrexone and appear to be entirely independent from naltrexone's better-known activity on opioid receptors. As a daily oral therapy, LDN is inexpensive and well-tolerated. Despite initial promise of efficacy, the use of LDN fo…

Effectiveness of naltrexone in the prevention of delayed respiratory arrest in opioid-naive methadone-intoxicated patients.

Acute methadone toxicity is a major public health concern in Iran. Methadone-intoxicated patients are in a great risk of recurrent or delayed respiratory arrest despite the prescription of initial doses of naloxone. This study aimed to evaluate the effectiveness of oral naltrexone in the management of acute methadone overdose in opioid-naive patients and check if it could be a substitute of continuous infusion of naloxone in maintaining adequate ventilation. In a randomized, double-blind, placebo-controlled study, a total of 54 opioid-naive patients with acute methadone toxicity were enrolled.…

Specific and nonspecific effects of naltrexone on goal-directed and habitual models of alcohol seeking and drinking.

BACKGROUND: The opioid-receptor antagonist naltrexone (NTX) reduces goal-directed alcohol drinking in rats presumably by blunting alcohol reward. However, different operant conditioning behavior can be produced by different reinforcement schedules, with goal-directed operant behavior being more sensitive to changes in reward value than less flexible, habit-associated models. We tested the hypothesis that NTX more effectively reduces alcohol drinking and seeking in a goal-directed than in a habit-associated operant model, and more effectively reduces alcohol versus sucrose self-administration, …

Effects of naltrexone on smoking cessation outcomes and weight gain in nicotine-dependent men and women.

This study examined whether the opioid receptor antagonist naltrexone is efficacious in smoking cessation and whether sex moderates the response. We assessed smoking quit rates and weight gain in a double-blind randomized trial comparing oral naltrexone (n = 162) with placebo (n = 154) in nicotine-dependent participants who wanted to quit smoking. The medication was gradually titrated up to 50 mg during the week before the quit date and then maintained at this dose for 12 weeks. For the first 4 weeks after the quit date, all participants received a nicotine patch to mitigate tobacco withdrawal…

Pharmacokinetics, safety, and tolerability of ascending doses of sublingual fentanyl, with and without naltrexone, in Japanese subjects.

This open-label, nonrandomized study assessed single and repeat ascending doses of a new sublingual fentanyl (SLF) formulation in 48 healthy Japanese opiate-naïve subjects (47 completed). Subjects received single-dose SLF 100, 200, 400, or 800 µg followed by 13 doses 6 hourly, at their dose level. Subjects taking repeat-dose 400 and 800 µg were pretreated with naltrexone in order to block opiate-receptor-mediated effects on respiration, monitored by pulse oximetry and transcutaneous pco(2). Sublingual fentanyl was rapidly and consistently absorbed. After single doses, median t(first) was 0.08 …

Naltrexone for the treatment of alcohol dependence among African Americans: results from the COMBINE Study.

These analyses of the COMBINE Study were designed to examine the effects of naltrexone among African Americans during the course of the 16-week treatment. Participants (total n=100; 70% male) who received naltrexone during the 16-week treatment trial (n=51) were compared to those who received placebo (n=49), controlling for acamprosate and behavioral intervention. Results did not support the efficacy of naltrexone on percent days abstinent, time to first heavy drinking day, and global clinical outcome in this subsample of African Americans. These results suggest that further work is needed to …

Effect of extended-release naltrexone (XR-NTX) on quality of life in alcohol-dependent patients.

BACKGROUND: Extended-release naltrexone (XR-NTX) is a once-a-month injectable formulation for the treatment of alcohol dependence previously shown to reduce drinking and heavy drinking relative to placebo (Garbutt et al., 2005). A 24-week, randomized, double-blind, placebo-controlled study established the efficacy and safety of XR-NTX in this patient population. In this report, the effect of XR-NTX on quality of life (QOL) was examined. METHODS: Alcohol-dependent patients were randomly assigned to receive XR-NTX 380 mg (N = 205), XR-NTX 190 mg (N = 210), or placebo (N = 209), combined with a …

Reduction of ethanol intake by chronic treatment with Hypericum perforatum, alone or combined with naltrexone in rats.

Acute treatment with extracts of Hypericum perforatum, the common plant usually called St. John's Wort, reduces voluntary ethanol intake in Marchigian Sardinian alcohol-preferring (msP) rats and acts synergistically with opioid receptor antagonists to further attenuate ethanol consumption. The present study evaluated the effect of chronic (once a day for 12 days) intragastric administration of a CO2 Hypericum perforatum extract (HPCO2), given alone or combined with naltrexone (NTX), on ethanol intake offered 2h/day in msP rats. Chronic treatment with HPCO2 markedly reduced ethanol intake at th…

Influence of chronic prenatal and postnatal administration of naltrexone in locomotor activity induced by morphine in mice.

The influence of chronic pre- and postnatal naltrexone exposure on the sensitivity of offspring to the locomotor effects of morphine was investigated in C-57 Black mice. Pregnant mice were injected subcutaneously (sc) with either saline (0.1 ml/10 g) or naltrexone (10 mg/kg) twice daily during gestation and throughout lactation, 21 days postpartum. One, three and seven weeks after birth, male offspring were tested for locomotor activity. At 7 weeks of age, dose-response curves were obtained with morphine (10, 31.6, and 100 mg/kg) and amphetamine (0.31, 10 and 31.6 mg/kg) in naltrexone-pretreat…

Prolonged opioid blockade with naltrexone and luteinizing hormone modifications in women with polycystic ovarian syndrome.

OBJECTIVE: To investigate whether enhanced LH levels of women with polycystic ovarian syndrome (PCOS) are the consequence of an absent hypothalamic opioid inhibitory control and/or an increased sensitivity of gonadotroph to GnRH, induced by sensitizing effects of circulating opioid peptides. DESIGN: Pulsatile LH secretion (10-minute sampling for 6 hours) and GnRH-stimulated (10 micrograms) LH release were investigated in 14 women with PCOS before and after the 5-day administration of placebo (n = 7) or the opioid antagonist naltrexone (50 mg/d; n = 7). Seven age- and weight-matched normal cyc…

Ultrasonic vocalizations are elicited from rat pups in the home cage by pentylenetetrazol and U50,488, but not naltrexone.

Although isolated rat pups emit ultrasonic vocalizations (USVs), those kept warm and undisturbed in the home cage with their littermates seldom do. Drugs were administered to 10-day-old pups in the home cage to determine whether pharmacological agents can elicit USV in this familiar environment. Ten-day-old Wistar rats were injected with U50,488, a highly selective kappa opioid agonist; pentylenetetrazol (PTZ), an anxiogenic drug that binds at the GABA-benzodiazepine receptor complex; or naltrexone (NLX), an opiate receptor blocker, and then were returned to their littermates in the home cage.…

Examining the effects of varenicline and naltrexone on delay discounting among individuals with alcohol use disorder.

AIMS: Individuals with alcohol use disorder (AUD) often exhibit heightened delay discounting, a behavioral marker associated with poor treatment outcomes. Medications such as naltrexone and varenicline influence reward-related decision-making, but their effects on delay discounting remain unclear. This study examined whether these medications influence delay discounting rates. METHODS: We conducted a double-blind, randomized, placebo-controlled trial in 34 treatment-seeking adults with AUD. Participants were assigned to naltrexone (50 mg/day), varenicline (2 mg/day), or placebo and completed …

Use of Naltrexone for Patients With Stimulant Use Disorder in Malaysia: Protocol for a Retrospective Cohort Study.

BACKGROUND: Naltrexone is an opioid receptor antagonist. Naltrexone is used to block the euphoric and sedative effects of drugs such as heroin, codeine, and morphine. The medication helps to bind and block opioid receptors to decrease opioid cravings. In Malaysia, naltrexone has been used for maintenance treatments for heroin and alcohol since 1996. However, since 2011, naltrexone has been used as an off-label stimulant use disorder (StUD) treatment to achieve abstinence. OBJECTIVE: This study aims to determine the abstinence among StUD and non-StUD patients treated and without naltrexone. M…
