Patients with narcolepsy live with a lifelong sleep-wake disorder, impairing their quality of life, productivity, educational and employment outcomes. Clinicians are becoming aware that a significant aspect of the burden of this disease relates to frequent comorbid conditions, including aspects of the patient's emotional, metabolic, sleep and immune health. This review explores the literature describing the comorbidities seen in patients with narcolepsy, to enhance understanding of these often complex presentations. It hopes to encourage a multidisciplinary approach, to collaborate with patien…

PURPOSE: The purpose of this study was to describe a case of central serous chorioretinopathy (CSC) associated with narcolepsy.
METHODS: Case report.
RESULTS: A 34-year-old man was followed for persistent CSC in his left eye for more than 11 months. He did not have any known risk factors for CSC, including obstructive sleep apnea. However, he experienced disrupted sleep because of narcolepsy, which was inadequately treated. After 2 weeks of adequate treatment for his narcolepsy, fundus examination and optic coherence tomography demonstrated complete resolution of his CSC.
CONCLUSION: As thi…

Narcolepsy is characterized by excessive daytime sleepiness and rapid eye movement (REM) sleep abnormalities, including cataplexy. The aim of this study was to assess REM sleep pressure and homeostasis in narcolepsy. Six patients with narcolepsy and six healthy controls underwent a REM sleep deprivation protocol, including one habituation, one baseline, two deprivation nights (D1, D2) and one recovery night. Multiple sleep latency tests (MSLTs) were performed during the day after baseline and after D2. During D1 and D2 REM sleep was prevented by awakening the subjects at the first polysomnogra…

Many neurologic and psychiatric manifestations have been associated with systemic lupus erythematosus. Narcolepsy, currently hypothesized as related to an autoimmune process, has been rarely associated with systemic lupus erythematosus. We report a 36-year-old woman who presented with narcolepsy and who subsequently developed systemic lupus erythematosus. Excessive daytime sleepiness resolved after the administration of four intravenous bolus of cyclophosphamide and methylprednisolone followed by maintenance therapy with hydroxychloroquine, aspirine and prednisone. Narcolepsy should be include…

Narcolepsy is a neurologic illness that typically begins in the second and third decades of life. It is chronic in nature and negatively impacts the quality of life of affected patients. The classic presentation is a tetrad of excessive daytime sleepiness, cataplexy, sleep paralysis, and hypnagogic hallucinations. The exact cause remains unknown, but there is significant evidence that hypocretin deficiency plays an integral role. Some primary conditions that result in secondary narcolepsy include traumatic brain injury, congenital disorders, tumours, and strokes. Some medical and psychiatric d…

OBJECTIVE: While there are a number of observations/quantifications indicating a greater proportion of REM sleep without atonia (RWA) in narcolepsy, the intra-night distribution of this parameter has not been evaluated.
MATERIALS AND METHODS: Thirty-four patients (15 men and 19 women; mean age 44.9 +/- 18.9) with narcolepsy-cataplexy were included in this retrospective study. The clinical diagnosis was confirmed by MSLT, video-polysomnography and HLA typing. Polysomnographic recordings were scored with particular regard to REM sleep without atonia (RWA) across all the nocturnal REM periods. R…

This study was undertaken to investigate regional metabolic abnormalities and to determine the effects of modafinil in narcoleptics on cerebral glucose metabolism using [(18)F] fluorodeoxyglucose positron emission tomography (FDG PET). Eight narcoleptic patients participated in the study. Two [(18)F] FDG PET scans were obtained before and after 2 weeks of modafinil treatment. To identify the effect of modafinil on regional cerebral abnormalities in narcoleptics, pre- and post-treatment PET scans were compared using paired t-statistics with voxel-wised manner. In narcolepsy patients, significan…

A young man, in whom narcolepsy was subsequently diagnosed, had the simultaneous onset of quadriparesis and a rapid eye movement (REM)-sleep polysomnographic pattern. During this REM-sleep pattern, a waking alpha EEG rhythm, appearing when he was asked to close his eyes, immediately attenuated when he was instructed to open his eyes, after which the REM-sleep pattern persisted. The juxtaposition of REM sleep patterns and reactive alpha rhythms are likely unique to sleep paralysis and may prove valuable in diagnosing narcolepsy.

Narcolepsy is a common but underdiagnosed sleep disorder characterized by excessive daytime sleepiness and abnormal manifestations of REM sleep: cataplexy, sleep paralysis and hypnagogogic hallucinations. Sleep onset REM periods (SOREMPs) are diagnostic. Within the last five years the pathogenesis has become clearer: narcolepsy is associated with almost total absence of the neuropeptides hypocretin-1 and -2 in the hypothalamus and CSF. A low level of CSF hypocretin is now recognized as a new diagnostic tool. This review provides updated knowledge of narcolepsy.

OBJECTIVE: NREM sleep patterns of narcoleptic patients with cataplexy were studied, focusing on their sleep 'microstructure', by analyzing the cyclic alternating pattern (CAP).
METHODS: Forty-nine HLA DQB1*0602-positive patients with narcolepsy/cataplexy (32 men and 17 women, aged 18-46 years) were included together with 37 age-matched normal controls. Each subject underwent one polysomnographic night recording after an adaptation night. Sleep stages were scored following standard criteria and CAP A phases were detected and classified into 3 subtypes (A1, A2, and A3). Power spectra for freque…

OBJECTIVE: To investigate the clinical features of early onset narcolepsy.
METHODS: The clinical data of 105 consecutive patients with narcolepsy, 63 of which with an onset age of 9.7 +/- 3.1 on average and 42 with an onset age of 22.8 +/- 9.3 on average. Interrogation, physical examination, CT, and MRI were carried out. Polysomnography was conducted. Then the data were compared between these 2 groups.
RESULTS: All 105 patients showed daytime sleepiness. The incidence rate of cataplexy was 92% in the early onset group, significantly higher than that of the late onset group (P = 0.023). There…

BACKGROUND: Narcolepsy with cataplexy is caused by a selective loss of hypocretin-producing neurons, but narcolepsy can also result from hypothalamic and rostral brainstem lesions.
PATIENT: We describe a 38-year-old woman with severe daytime sleepiness, internuclear ophthalmoplegia, and bilateral delayed visual evoked potentials. Her multiple sleep latency test results demonstrated short sleep latencies and 4 sleep-onset rapid eye movement sleep periods, and her cerebrospinal fluid contained a low concentration of hypocretin. Magnetic resonance imaging showed T2 and fluid-attenuated inversion…

Recently, low levels of a newly identified neuropeptide, hypocretin 1, were described in the cerebrospinal fluid of patients with narcolepsy. This neurochemical finding furthers our understanding of this enigmatic sleep disorder typically characterized by excessive daytime sleepiness, cataplexy, sleep paralysis, and hypnagogic hallucinations. Narcolepsy appears to be fundamentally related to abnormally regulated rapid eye movement sleep. The diagnosis of this disorder remains challenging because of multiple other conditions that can cause daytime sleepiness and the difficulties in recognizing …
