Most hypophosphatemic patients will not require immediate replacement therapy with phosphate. In only the patients with acute severe hypophosphatemia (<1 mg/dL) or symptoms, intravenous phosphate replacement therapy is indicated. On the other hand, the patients with chronic hypophosphatemia require the medical treatment with oral administration of calcitriol and neutral phosphate since this condition causes hypophosphatemic rickets/osteomalacia. However, the treatment regimen has not been standardized, and nephrocalcinosis and tertially hyperparathyroidism are the complication of the long-term…

OBJECTIVES: Hypercalcemia in infancy is rare and often presents with nonspecific clinical signs, making early recognition challenging. This … while the second infant had been previously healthy. Both had demonstrated sonographic evidence of nephrocalcinosis. Laboratory evaluation revealed markedly elevated serum calcium levels (up to 4.73 mmol/L), hypercalciuria, and suppressed PTH concentrations in both cases. Standard treatment of hypercalcemia was ineffective, while a single dose of zoledronic acid …

Furosemide is the most commonly used diuretic in preterm infants despite an incompletely understood relationship between dosing, exposure, and safety within this population. The goals of this study were to characterize furosemide population pharmacokinetics (popPK) in preterm infants and to evaluate safety by relating simulated furosemide exposure to events of ototoxicity, nephrocalcinosis, and nephrolithiasis. A total of 146 plasma furosemide concentrations from 51 preterm (23-28.9 weeks' gestational age) infants across two studies (one randomized, placebo-controlled, dose-escalating safety t…

In the thick ascending limb, three functionally distinct cell populations express different claudin combinations, with claudin-10b driving paracellular Na + permeability and claudin-16/19 mediating Ca 2+ and Mg 2+ reabsorption. INTRODUCTION: Variants in the Ras-related GTPase D (RRAGD) gene have been associated with autosomal dominant kidney hypomagnesemia (ADKH) characterized by hypokalemia, nephrocalcinosis, and dilated cardiomyopathy (DCM). RRAGD, which encodes for the RagD protein, is involved in the activation of the mechanistic target of rapamycin complex 1 (mTORC1). Owing to the limited characterization of patients' phenotypes, the understanding of RRAGD-associated ADKH (ADKH-RRAGD) remains incomplete. Consequently, available treatment strategies are primarily symptomatic and insufficient.
METHODS: In the pre…

CONTEXT: The monogenic disorder autoimmune polyendocrine syndrome type 1 (APS-1) or autoimmune polyendocrinopathy-candidiasis-ectodermal dystrophy (APECED) manifests frequently with hypoparathyroidism, which requires treatment with oral supplementation with calcium and active vitamin D analogs. The majority of APS-1/APECED patients also suffer from intestinal malabsorption, which complicates the management of hypoparathyroidism and may lead to refractory severe hypocalcaemia. In such situations, reliance on intravenous calcium carries a high risk of nephrocalcinosis and renal damage.

The Gitelman variant is uniformly caused by mutations in the gene for the thiazide-sensitive NaCl-cotransporter NCCT (SLC12A3) of the distal tubule, while HPS/aBS is caused by mutations in the gene for either the furosemide-sensitive NaK-2Cl-cotran …

BACKGROUND: Primary hyperoxaluria type 1 (PH1) is a rare inherited metabolic disorder leading to the formation of kidney stones, nephrocalcinosis, and kidney failure. Besides, PH1 poses the risk of developing systemic oxalosis, a life-threatening condition with oxalate deposits in multiple organ systems. The rarity of the disorder combined with recent major additions to therapeutic options based on small interfering RNA (siRNA) therapeutics make a formal assessment of current practice and implementation of treatment recommendations an important asset.

Primary hyperoxaluria (PH) occurs due to an autosomal recessive hereditary disorder of the metabolism of glyoxylate, which causes excessive oxalate production. The most frequent and serious disorder is due to enzyme deficit of alanine-glyoxylate aminotransferase (PH type I) specific to hepatic peroxisome. As oxalate is not metabolised in humans and is excreted through the kidneys, the kidney is the first organ affected, causing recurrent lithiasis, nephrocalcinosis and early renal failure. With advance of renal failure, particularly in patients on haemodialysis (HD), calcium oxalate is massive…
