Electrodiagnostic reports of median neuropathy at the wrist.

The diagnosis of carpal tunnel syndrome (CTS) is confirmed by electrodiagnostic testing. Practice parameters for electrodiagnostic testing of CTS have been defined in a summary statement published by the American Academy of Neurology, the American Association of Electrodiagnostic Medicine, and the American Academy of Physical Medicine and Rehabilitation. All members of the Industrial Injuries and Prevention Committee of the American Society for Surgery of the Hand provided electrodiagnostic reports from their office practices indicating an electrodiagnosis of CTS or median neuropathy at the wr

Electrophysiologic evaluation of neuromuscular functions during limb lengthening by callus distraction.

We investigated the effects of different rates and extents of lengthening on the affected nerves and muscles in callus distraction, in a rabbit tibial model. Electrophysiologic evaluations measuring the motor nerve conduction velocity of the peroneal and tibial nerves, and compound muscle action potentials of the anterior tibialis and gastrocnemius muscle were performed sequentially on rabbits whose left legs were lengthened at different rates and to different extents over a 5-month period after surgery. We found that neuropathies of the peroneal nerves developed earlier and were more severe t

[The role of paraclinical studies in the diagnosis of hypomagnesemia spasmophilia].

Assuming that Ca+2 and Mg+2 play an important role in the cell (especially the neuronal and muscular ones) membrane excitability, this paper presents the place that the paraclinical specific investigations have in the proper diagnose of normal calcium serum level hypo-magnesium spasmophilia. A number of 52 patients presenting significant clinical spasmophilia, of age between 21 and 55 years old, from different professions and social classes, were selected for this study. Paraclinical investigations (spectrophotometry) showed a normal serum calcium level (an average of 9.22 mg%); also, a decrea

Medial femoral cutaneous nerve conduction.

Medial femoral cutaneous nerve (MFCN), a sensory branch of the femoral nerve, supplies the skin on the anterior medial aspect of the thigh and knee. A conduction study for this nerve is described. A recording electrode was placed anterior-medially in the thigh and 14 cm distal to the stimulating cathode that was placed immediately lateral to the femoral artery just below the inguinal ligament. Sixty-four nerves of 32 adults with a mean age of 40 yr (range, 21-56) were tested. The latencies to the onset and peak were 2.4 +/- 0.2 and 2.9 +/- 0.2 ms, respectively. The amplitude of baseline to neg

The numb arm and hand.

Trauma and compression along the course of the median, ulnar or radial nerve from the brachial plexus to the fingers may cause pain, weakness, numbness or tingling of the upper extremity. Diabetes, smoking, alcohol consumption, rheumatoid arthritis and hypothyroidism are risk factors for nerve entrapment, although these disorders typically produce bilateral symptoms. Carpal tunnel syndrome, the most common nerve entrapment condition, results from median nerve compression at the wrist. The diagnosis is suggested by decreased pain sensation and numbness in the thumb and index and middle fingers;

Sensory nerve conduction during cold pressor response in humans.

A study was carried out to find the correlation, if any of autonomic responses and experimental pain induced during the cold pressor test (CPT) with the median nerve sensory conduction velocity (SCV). In 19 healthy subjects who volunteered for the study, the nondominant hand of each was subjected to the CPT and, pain parameters, heart rate, blood pressure and SCV were recorded. Dominant median nerve SCV was measured by orthodromic stimulation of the index finger and evoked responses were recorded at the wrist. In each subject, 32 evoked action potential responses were averaged to determine the

[Electroneurographic parameters in cubital neuropathy of the elbow].

Using conventional techniques with cutaneous electrodes, 14 parameters of antidromic, motor and mixed segmentary sensory conduction of the cubital nerve were assessed in the first 45 patients presenting to us with some form of electroneurographic abnormality. Changes suggestive of axonal degeneration due to a decrease in amplitude or conduction velocity of the distal segment were found in 17. The remaining 28 patients showed only signs of change in conduction velocity at the elbow and could be classified according to electroneurographic degrees of progressive abnormality. Changes in antidromic

Electro neuro physiological studies in early tuberculoid leprosy.

Electro physiological studies were carried out in early tuberculoid type of leprosy in order to study their utility in detecting nerve damage before the onset of obvious functional deficit. Fifty-three cases showing one mixed nerve thickening in one limb were selected. Nerve conduction studies (both motor and sensory) were done using single blind technique. There was no statistically significant difference between the findings obtained from clinically thickened and non-thickened nerves. There was also no direct relationship between clinical sensory deficit and electro physiological abnormality

Nerve conduction studies in experimental non-freezing cold injury: I. Local nerve cooling.

In rabbits, the tibial nerve was exposed in the lower thigh under general anesthesia and cooled in a metal trough at 1 to 2 degrees C or 5 degrees C for 2, 3, or 4 hours. Nerve conduction studies showed local failure of conduction at the site of cooling which persisted after rewarming, and which was followed by distal degeneration of affected fibers. No persistent conduction block was seen. Changes in maximal velocity indicated that the fastest-conducting motor and afferent axons had been preferentially affected. Histological findings in nerves examined at different intervals after cooling con

Ulnar nerve palsy after supracondylar humerus fracture.

Six patients with cubitus varus deformity after a supracondylar fracture of the humerus had ulnar nerve palsy 7 (3-14) years following the fracture. All the patients showed anterior dislocation of the ulnar nerve during elbow flexion. In cubitus varus deformity, medial shifting of the triceps muscle occurs, which pushes the ulnar nerve anteriorly and frequently causes ulnar-nerve dislocation. Five of the 6 patients underwent surgery with subsequent improvement.

[Measurement of carpal tunnel pressures in the idiopathic carpal tunnel syndrome without motor deficit].

The pressure in the carpal tunnel was measured in ten patients suffering from purely painful carpal tunnel syndrome. Five normal individuals were also investigated as controls. It was shown that the pressure was always high in involved patients. A rise of pressure on flexion and extension of the wrist was much greater in patients than in controls. It is concluded that true compression plays the main role in carpal tunnel syndrome. A localised tenosynovitis was constantly found at operation. It was responsible for the rise of pressure inducing a functional ischaemia of the nerve.

Isolated proximal musculocutaneous nerve palsy: case report.

Isolated musculocutaneous nerve injury is extremely rare. The few reported cases were associated with heavy exercises and violent extension of the forearm. The lesions in those cases were below the coracobrachialis muscle, involving the biceps, brachialis, and cutaneous nerves of the forearm. This patient's lesion was unique, located proximal to the coracobrachialis. There was no antecedent trauma. At follow-up six weeks and four months after onset of symptoms, significant improvement was noted both clinically and electrophysiologically. Musculocutaneous nerve palsy is important to distinguish

Intraoperative evoked electromyography in Bell's palsy.

The technique of intraoperative evoked electromyography is described in detail. Direct intraoperative stimulation of the exposed facial nerve identified the sites of nerve impulse conduction blocks in 16 of 18 patients who needed decompression for Bell' s palsy. In order to determine the site of impulse conduction block the test must be performed before 100 per cent nerve degeneration occurs. The lesions extended only a few millimeters and were found to be proximal to the geniculate ganglion in 15 (94 per cent) of these patients. Schirmer's test correctly identified the sites of the lesions in

Electrodiagnostic evaluation of hypomagnesemia in sheep.

Experiments were conducted to permit electrophysiological characterization of hypomagnesemia in sheep. For 6 weeks, adult wethers were fed: (1) a diet deficient in magnesium and calcium, (2) a diet deficient in magnesium, (3) a diet deficient in calcium and (4) a control diet. Weekly blood samples were collected to determine plasma concentrations of calcium (CA++), magnesium (Mg++) and potassium (K+). During the 6-week feeding period, electromyography (EMG), nerve conduction velocity (NCV) and strength-duration (S-D) determinations were conducted. At the end of a 6-week period, cerebrospinal f

Delayed nerve conduction velocities in children with protein-calorie malnutrition.

Protein-calorie malnutrition in human beings and animals affects the myelination and growth of the nervous system. The effects of PCM on the developing nervous system were evaluated by measuring the nerve conduction velocities in 93 (38 marasmus, 13 kwashiorkor, and 42 control) children in ulnar, median, peroneal, and posterior tibial nerves. The children were divided into three age groups: Group I, six to 12 months; Group II, 13 to 24 months; Group III, 25 to 48 months. All 13 children with kwashiorkor demonstrated irritability, delayed milestones, and muscular wasting; and six also had hypoa

Effects of strychnine on the sodium conductance of the frog node of Ranvier.

Strychnine blocks sodium conductance in the frog node of Ranvier. This block was studied by reducing and slowing sodium inactivation with scorpion venom. The block is voltage and time dependent. The more positive the axoplasm the greater the block and the faster the approach to equilibrium. Some evidence is presented suggesting that only open channels can be blocked. The block is reduced by raising external sodium or lithium but not impermeant cations. A quaternary derivative of strychnine was synthesized and found to have the same action only when applied intracellularly. We conclude that str

[Report on clinical positive evidence of the nerve excitability test in facial palsy (author's transl)].

Comparative studies of case controls of Bell's palsy, posttraumatic and postoperative palsies in relation to the clinical positive evidence of the nerve excitability test are demonstrated. While an absolutely parallel reaction of clinical function restitution and electrical excitability is observed in Bell's palsy and in posttraumatic late palsy, on the other hand this congruence will not be observed in postoperative palsies (in intact anatomical conduction of the nerve) in most of the investigated cases. In these cases the clinical restitution of isolated movements of the mimetic muscles has
