Carpal tunnel syndrome--a comprehensive review.

PURPOSE: To provide a comprehensive review of the management of carpal tunnel syndrome. METHODS AND RESULTS: A systematic literature review is provided of the history, anatomy, pathophysiology, epidemiology, diagnostic criteria, investigative surgical techniques, results and complications for carpal tunnel syndrome. CONCLUSION: Surgery for carpal tunnel syndrome requires meticulous attention to history-taking, investigation, counseling, training and surgical technique if unsatisfactory results and complications are to be avoided.

Amyotrophic lateral sclerosis.

In this case study, we describe the symptoms, neurological examination, and pathology of a woman with amyotrophic lateral sclerosis (ALS). ALS is a rare disorder leading to degeneration of the voluntary motor system and death in, on average, 3 to 4 years. The loss of motor neurons in the brain and spinal cord causes the progressive symptoms of muscular weakness, atrophy, fasciculation (muscle twitching), spasticity, and hyperreflexia. Signs of disease in both upper and lower motor neurons are required for a definitive clinical diagnosis. Pathology shows degeneration of the lateral corticospina

Are the El Escorial and Revised El Escorial criteria for ALS reproducible? A study of inter-observer agreement.

INTRODUCTION: For accurate diagnosis, inter-observer agreement of criteria is important. METHODS: Using case records, the reproducibility of the original and revised El Escorial diagnostic criteria for amyotrophic lateral sclerosis were tested in a consecutive series of people referred to the Scottish Motor Neuron Disease Register. RESULTS: Agreement between independent observers was similar (weighted kappa: 0.783, 95% CI 0.656 to 0.911 (original criteria), 0.681, 95% CI 0.485 to 0.878 (revised)). CONCLUSIONS: Serious errors are unlikely, but the revised criteria may be less reproducible as

Confounding effects of mimicking disorders in the early diagnosis of amyotrophic lateral sclerosis.

A diagnosis of amyotrophic lateral sclerosis (ALS) should be given to the patient only when the doctor or the neurologist feels confident or certain about the diagnosis. Certainty of a diagnosis of ALS is important because there are a number of disorders that could mimic the symptoms of ALS but differ in prognosis and treatment. Early treatment of ALS is critical to offer optimal therapy to the patient. However, ALS is difficult to diagnose at an early stage. The fact that ALS is a rare disease means doctors and neurologists do not always have the necessary experience to diagnose confidently.

[Dysautonomia in Guillain-Barré syndrome].

About 20% of all GBS patients have symptoms of dysautonomia: labile hypertension, orthostatic hypotension, sinustachycardia or sinus arrest. This rate rises to 75% in patients with tetraplegia. Proprioceptive loss predicts dysautonomia independently from the severity of weakness. It is frequently responsible for dysautonomia. The afferent limb of cardiovascular regulation contains more myelinated fibers than the sympathetic and parasympathetic efferences, which determine the common classification of dysautonomia. The frequence of mixed sympathetic and parasympathetic hyperactivity is hard to e

[MRI findings in Wernicke encephalopathy].

Wernicke's encephalopathy (WE) is a consequence of vitamin B1 (thiamine) deficiency and in the majority of cases due to alcoholism. We report here the case of a 26-year-old male alcoholic who had stayed helplessly at home for 4 days until hospital admission. Clinical diagnosis was difficult due to major disturbance of consciousness. MRI showed an increase in signal intensity (T2-, FLAIR-weighted) around the third ventricle, the quadrigeminal bodies, the fornices, the mamillary bodies, the floor of the fourth ventricle and around the aqueduct. These findings were indicative of WE although of un

[The role of hyperventilation and neuromuscular excitability in the clinical picture and pathogenesis of migraine headaches].

The comparison of two groups of migrainous patients with or without hyperventilation syndrome (HS) provided the conclusions that HS provokes more severe migrainous attacks and psychovegetative symptoms in migrainous cephalalgia (MC). The findings also support the existence of stem structure disfunction in MC patients.

The false localizing signs of increased intracranial pressure.

False localizing signs, although rare, indicate significant increased intracranial pressure with tissue shift, has occurred. A review of the historical perspective highlights the discovery of these false localizing signs, and exploration of the anatomic and physiologic causes provides important insight for neuroscience nurses. Critical thinking skills, and a world view paradigm for clinical practice are developed upon an in-depth understanding of neurologic dysfunction. Collaborative relationships are enhanced by an interdisciplinary approach to assessment of the critically ill neuroscience pa

[The otoneurological symptoms in giant adenomas of the hypophysis].

Analysis of the otoneurological symptomatology in 32 patients with hypophyseal tumors showed that they extended most frequently into the sphenoidal sinus, the cells of the ethmoidal sinus, the superoposterior parts of the nose, and the nasopharynx. The focal symptoms of compression of the adjoining parts of the brain (frontal, temporal) were manifested in 11 among 32 cases by olfactory disorders (loss of olfaction in 4 patients, diminished olfaction in 2, olfactory hallucinations in 4 patients, and impaired recognition of odors in 1 patient). Asymmetric action of the tumor on the subcortical s

[Diagnostic and clinical aspects of neuropathy of small nerve fibers].

Small fibre neuropathy, a phenomenon that neurologists have always been familiar with as an accidental or independent pattern of signs and symptoms in "sensitive" or "painful" neuropathy, has become interesting once again as a result of progress in diagnostic methods. The A-delta and C fibres as well as the B fibres (of the vegetative nervous system) are known as "small", i.e. thin, weak or non-myelinated nerve fibres of slow conductivity. More than 80% of the nerve fibre mass of the peripheral somatic nerve is accounted for by the small fibres; moreover, the small fibres make up the periphera

Mosaic 47,XY,+8/48,XXYY in a mentally non-retarded man with phenotypical and neurological abnormalities.

This report describes a non-retarded human male, mosaic for a 47,XY,+8 and a 48,XXYY cell line. The 47,XY,+8 cell line is present in approximately 70% of both lymphocytes and skin fibroblasts; 30% of the cells in both tissues have a 48,XXYY karyotype. Clinical abnormalities correspond mostly with the mosaic trisomy 8 syndrome.

[Craniosynostoses: importance of the measurement of intracranial pressure and the application of J Jane's surgical technic].

We report the studies, treatment and follow-up of two children with the diagnosis of craniosynostosis for early closure of sagittal and coronal sutures. We paid special attention to the intracranial pressure (ICP) monitoring. Prognosis of the mental functions (Gessell's test) is related with findings of ICP and surgical treatment. We have performed as neurosurgical approach, a modification of the Jane's technic. The advantages of this technic and our results are discussed.

Myelopathy in Reiter's disease.

A 21-year-old male presented with fevers, polyarthritis, dysuria and rash. Neurologic examination indicated myelopathic changes below the T12 spinal cord level. These changes were present during the acute course of his illness but were not evident after 8 weeks of treatment. We are unaware of reports of myelopathy associated with Reiter's disease.
