Prolonged (more than ten hours) neuromuscular blockade after cardiac surgery: report of two cases.

PURPOSE: We examine two cases of prolonged neuromuscular blockade (NMB) after cardiac surgery. To the best of our knowledge, these are the first reported cases of complete paralysis lasting more than ten hours after surgery. CLINICAL FEATURES: We attribute the extended durations of NMB (more than ten hours) to high doses of NMB drugs in combination with magnesium sulphate and moderate renal failure. Advanced age, hepatic disease, aminoglycoside exposure, hypocalcemia, and possible interaction between rocuronium and pancuronium may have played minor roles. CONCLUSION: We should avoid administ

Neuromuscular blockade and lung function during resuscitation of infants with congenital diaphragmatic hernia.

BACKGROUND: There is no consensus or evidence as to whether a neuromuscular blocking agent should be used during the initial resuscitation of infants with congenital diaphragmatic hernia (CDH) in the labour ward. OBJECTIVE: To determine if administration of a neuromuscular blocking agent affected the lung function of infants with CDH during their initial resuscitation in the labour ward. METHODS: Fifteen infants with CDH were studied (median gestational age 38 weeks, range 34-41; birth weight 2,790 g, range 1,780-3,976). Six infants had undergone feto-endotracheal occlusion (FETO). Flow, air

The Influence of Athletic Profile on Acute Neuromuscular Response to a Repeated Sprint Protocol in Youth Females: A Pilot Study.

Salter, J, Forsdyke, D, Dawson, Z, Rymer, J, Walsh, L, and Mundy, P. The influence of athletic profile on acute neuromuscular response to a repeated sprint protocol in youth females: A pilot study. J Strength Cond Res 39(7): 760-769, 2025-Athletic profiles influence how athletes tolerate exposure to high-intensity activities and training-induced fatigue. Current understanding of athletic profiles has been derived primarily from adult male populations that limits the application of this potentially important training knowledge to women, and in particular youth women. Therefore, this study aimed

Malignant hyperthermia crisis during scoliosis surgery in a pediatric cerebral palsy patient: a case of early recognition and successful intervention.

BACKGROUND: Malignant hyperthermia (MH) is a rare but life-threatening complication of general anesthesia, characterized by a hypermetabolic response that can lead to severe complications if not managed promptly. Patients with neuromuscular disorders, such as cerebral palsy and neuromuscular scoliosis, may have an increased risk of developing MH due to underlying genetic and physiological factors. Despite its rarity, early recognition and intervention are critical for patient survival. CASE PRESENTATION: We present a case of MH in a 13-year-old girl with cerebral palsy and neuromuscular scoli

Anaesthetic implications of calcium channel blockers.

Clinical uses of calcium channel blockers are expanding. In addition to the established uses in patients with arrhythmias, angina pectoris or hypertension, newer and to some extent investigational uses indicate widespread application. For instance, their use has been reported in hypertrophic cardiomyopathy and cold cardioplegia, as well as in pulmonary hypertension, antiplatelet therapy, asthma, achalasia and oesophageal spasm, increased intraocular pressure and in cerebral vasospasm. Their use in obstetrical practice has been proposed. Thus, the presentation of a patient who is treated with c

Update in Noninvasive Home Mechanical Ventilation: A Narrative Review of Indications, Outcomes, and Monitoring.

Hypercapnic respiratory failure arises due to an imbalance in the load-capacity-drive relationship of the respiratory muscle pump, typically arising in patients with chronic obstructive pulmonary disease, obesity-related respiratory failure, and neuromuscular disease. Patients at risk of developing chronic respiratory failure and those with established disease should be referred to a specialist ventilation unit for evaluation and consideration of home noninvasive ventilation (NIV) initiation. Clinical trials demonstrate that, following careful patient selection, home NIV can improve a range of

The train-of-four count and kinetic-dynamic modelling.

The intraoperative monitoring of neuromuscular blockade usually involves measurement of the muscular responses to motor nerve stimulation. Although researchers have the time and technology to obtain predrug control measurements of the twitch responses, these are seldom available to the clinician. During surgically useful relaxation, there is progressive inhibition of responses to train-of-four (TOF) stimulation in the presence of a nondepolarizing relaxant, and the clinician can count the number of responses to predict the likely time-course of paralysis. At the extremes, a count of four (of f

Rocuronium-sugammadex for electroconvulsive therapy management in neuroleptic malignant syndrome: A case report.

Neuroleptics are a group of drugs widely used in the treatment of psychotic symptoms. Among their adverse effects is the ability to trigger a neuroleptic malignant syndrome (NMS). The diagnosis of NMS is determined by exclusion, and its initial therapeutic management should be the withdrawal of neuroleptics, the administration of benzodiazepines, and electroconvulsive therapy (ECT). ECT is an effective treatment in these patients, and in those cases with a poor response to treatment with antipsychotic drugs. A review is presented on the treatment options and anaesthetic implications of ECT use

Loss of lower limb motor evoked potentials and spinal cord injury during the initial exposure in scoliosis surgery.

PURPOSE: To report a case of motor evoked potential changes and spinal cord injury during the initial dissection in scoliosis surgery. METHODS: Motor evoked potentials to transcranial electrical stimulation were recorded from multiple muscles. Somatosensory evoked potentials to limb nerve stimulation were recorded from the scalp. RESULTS: Clear motor evoked potentials were initially present in all monitored muscles. The patient was then pharmacologically paralyzed for the initial dissection. More than usual bleeding was encountered during that dissection, prompting transfusion. As the neurom

Clinical progression of X-linked muscular dystrophy in two German Shorthaired Pointers.

CASE DESCRIPTION: 2 full-sibling male German Shorthaired Pointer (GSHP) puppies (dogs 1 and 2) with X-linked muscular dystrophy and deletion of the dystrophin gene (gene symbol, DMD) each had poor growth, skeletal muscle atrophy, pelvic limb weakness, episodic collapse, and episodes of coughing. CLINICAL FINDINGS: Initial examination revealed stunted growth, brachygnathism, trismus, and diffuse neuromuscular signs in each puppy; clinical signs were more severe in dog 2 than in dog 1. Immunohistochemical analysis revealed a lack of dystrophin protein in both dogs. During the next 3 years, each

Phrenic Nerve Involvement in Neuralgic Amyotrophy (Parsonage-Turner Syndrome).

Neuralgic amyotrophy is a poorly understood neuromuscular disorder affecting peripheral nerves mostly within the brachial plexus distribution but can also involve other sites including the phrenic nerve. In the classic form of the syndrome it causes proximal upper limb and neck pain on the affected side with subsequent muscle weakness that can be highly heterogeneous. Nocturnal noninvasive ventilation support is a first-line treatment after phrenic mononeuropathy. The regular monitoring of diaphragm function with spirometry and diaphragm ultrasound can help determine prognosis and inform decis

Posttetanic potentiation and fade in the response to tetanic and train-of-four stimulation during succinylcholine-induced block.

UNLABELLED: We designed this study to confirm anecdotal observations that neuromuscular block after a single administration of succinylcholine is characterized by fade to train-of-four (TOF) or tetanic stimulation, as well as posttetanic potentiation. This prospective, randomized, 2-center observational study involved 100 patients. Patients were allocated to 1 of 5 groups and received 0.1, 0.3, 0.5, 0.75, or 1.0 mg/kg succinylcholine during propofol/fentanyl/nitrous oxide anesthesia. Neuromuscular function was monitored by TOF using mechanomyography. At 10%-20% spontaneous recovery of the firs

Exploring molecular signatures in PURA syndrome using muscle proteomics and serum biomarkers.

BACKGROUND AND PURPOSE: Dominant PURA variants (encoding purine-rich element-binding protein A) cause a neurodevelopmental disorder with hypotonia, cognitive impairment, and variable neuromuscular symptoms. Clinical presentations and response to pyridostigmine, moreover, highlighted neuromuscular junction (NMJ) involvement. However, NMJ architecture, underlying molecular mechanisms, and potential minimally invasive biomarkers in PURA syndrome remain poorly characterized. This study aimed to profile PURA-related disease using integrated clinical, histological, ultrastructural, transcriptional,

The effect of sugammadex on patient morbidity and quality of recovery after general anaesthesia: a systematic review and meta-analysis.

BACKGROUND: Residual neuromuscular block is associated with increased patient morbidity. Therefore prevention of residual neuromuscular block is an important component of general anaesthesia where neuromuscular blocking agents are used. Whereas sugammadex improves reversal based on neuromuscular twitch monitoring parameters, there have been no prospective, adequately powered definitive studies demonstrating that sugammadex is also associated with less patient morbidity. METHODS: We performed a systematic review of randomised trials comparing sugammadex with anticholinesterase-based reversal o

Reduced running performance and greater perceived exertion, but similar post-exercise neuromuscular fatigue in tropical natives subjected to a 10 km self-paced run in a hot compared to a temperate environment.

Environmental heat stress impairs endurance performance by enhancing exercise-induced physiological and perceptual responses. However, the time course of these responses during self-paced running, particularly when comparing hot and temperate conditions, still needs further clarification. Moreover, monitoring fatigue induced by exercise is paramount to prescribing training and recovery adequately, but investigations on the effects of a hot environment on post-exercise neuromuscular fatigue are scarce. This study compared the time course of physiological and perceptual responses during a 10 km

Advanced neuromonitoring techniques for medical and neurological ICU patients.

Newly developed neurological complications are not confined to patients admitted to the neurology ICU but also occur in the medical ICU. In addition to patients with traumatic brain injuries, critically ill patients with sepsis, encephalopathy, and post-cardiopulmonary resuscitation are at risk of developing neurological complications. The situation can worsen when patients are sedated and intubated, which prevents the clinical manifestations to be presented. In this regard, non-convulsive seizures and status epilepticus, new onset strokes, and neuromuscular weakness syndromes could be missed
