Neurophysiological correlates of passive movements are speed- and type-dependent.

PURPOSE: The supraspinal involvement in the control of passive movements remains elusive. Previous studies provided electromyographic evidence for speed and –type dependent changes in muscle activity. Based on mechanoreceptor properties, their age-related changes and the somatotopically organized connections between sensory and motor systems, this study aimed to provide electrophysiological evidence for the involvement of frontal cortex inhibition and corticomotor interactions in the control of passive movements. METHODS: Twenty healthy younger and older adults performed passive elbow flexion

Neurophysiological signatures of rumination's impact on opioid withdrawal severity.

Opioid withdrawal symptoms (OWS) represent a major clinical challenge during abstinence, imposing physiological distress and increasing psychological vulnerability to relapse. Psychological factors contributing to individual differences in OWS severity remain understood, and reliable biomarkers for OWS severity are still lacking, highlighting the need for early identification and timely intervention among high-risk heroin abstainers. Rumination is defined as uncontrollable negative thinking, potentially impairing the ability to suppress addiction-related negative thoughts. This study aims to i

Neurophysiological Basis of Mirror Movements in Schizencephaly: A Case Report.

Mirror movements (MMs) are involuntary movements on one side of the body that mimic voluntary movements on the opposite side. Schizencephaly … schizencephaly who presented with MMs. Transcranial magnetic stimulation was used to understand the neurophysiology of MMs. Single-pulse and paired-pulse stimulation were done. The silent period was shortened with absent short-interval intracortical inhibition and reduced intracortical facilitation. This suggests reduced GABAergic and glutamatergic transmission i …

Neurophysiological profiles underlying action withholding and action discarding.

Although inhibitory control is essential to goal-directed behavior, not all inhibition is the same: Previous research distinguished discarding an action plan from simply withholding it, suggesting separate neurophysiological mechanisms. This study tracks the neurophysiological signatures of both using time-frequency transformation and beamforming in n = 34 healthy individuals. We show that discarding an action plan reduces working memory load, with stronger initial theta band activity compared to withholding it. This oscillatory difference was localized in the (para-)hippocampus and anterior t

Neurophysiological biomarkers of networks impairment in amyotrophic lateral sclerosis.

Amyotrophic lateral sclerosis (ALS) is a heterogeneous disease involving motor system as well as cognitive domains. There is an urgent need for objective biomarkers that can subcategorize subjects into homogeneous groups based on disease pathobiology. In this review, we discuss novel neurophysiological techniques that provide detailed, multiscale and multidimensional insights into ALS networks impairment, spanning from the micro-columnar architecture of the motor cortex to motor and cognitive networks. Specifically, Transcranial Magnetic Stimulation (TMS) paradigms can be used to evaluate the s

The Neurophysiological Effects of Dry Needling: An Update of a Narrative Review.

The interest in dry needling as a treatment option for myofascial pain has flourished for the last decades and will probably continue to do so, because multiple clinical effects can be attributed to this technique. However, evidence about the underlying physiological mechanisms of its effects is still underrepresented in scientific research and caution must be taken in generalizing results from acupuncture or animal research. This review offers an overview of the possible mechanisms involved in the pathophysiology of myofascial trigger points, that is, the formation of the taut band, the prese

Neurophysiological Insights into the Pathophysiology of Stiff-Person Spectrum Disorders.

BACKGROUND: Stiff Person Spectrum Disorders (SPSD) are classically defined by the presence of muscle stiffness, spasms and hyperactivity of the central nervous system. There is a notable correlation between neurophysiological features and the clinical hallmark of SPSD, which has greatly encouraged the use of these techniques for diagnostic purposes. Besides, electrophysiological techniques allow for a functional evaluation of the 'hyperactivity of the CNS', thus offering the opportunity to clarify the mechanisms underlying this disorder. This review delves into the current knowledge on the ele

Reliability of Neuroimaging and Neurophysiological Tremor Features in Parkinson's Disease.

BACKGROUND: Tremor is one of the cardinal motor symptoms of Parkinson's disease (PD). Its severity can be quantified with clinical rating scales and neurophysiology. The pathophysiology of PD tremor involves activity in the cerebello-thalamo-cortical circuit (CTC), which can be quantified with concurrent accelerometry-functional magnetic resonance imaging (fMRI). This opens possibilities for fMRI-informed precision medicine, but it remains unclear if tremor-related fMRI activity is a reliable biomarker at the individual level. OBJECTIVES: To assess the test-retest reliability of three PD trem…

CSI and Musculoskeletal Pain: Clinical Validity and Neurophysiological Implications.

BACKGROUND AND AIM: Chronic pain affects about 20% of adults worldwide, severely impacting quality of life and functioning. Many chronic pain syndromes share symptoms such as fatigue, poor sleep, anxiety, and depression, suggesting a common mechanism. Central sensitization, an abnormal amplification of pain by the nervous system, is considered a key underlying factor. Assessment tools like the Central Sensitization Inventory (CSI) and laser-evoked potentials (LEPs) may improve diagnosis and guide targeted treatment strategies. This review evaluates the validity of the CSI for assessing central

Evaluation of brain and neurophysiologic function in isolated congenital anosmia.

PURPOSE: Isolated congenital anosmia (ICA) is a relatively rare condition characterized by a complete lack of olfactory perception dating from birth or early childhood. Congenital deficits in sensory systems appear to have distinct effects on brain function and nervous system pathway, but little is known about ICA. The study aims to explore the neural basis of ICA through measuring brain activation in response to olfactory stimuli and cerebral processing in patients with ICA. METHODS: Retrospective review of medical records of 11 ICA patients from two smell and taste centers. All patients who

Office-based Neurophysiological Assessments for Diagnosis and Prognosis of Concussion.

Patients with concussion often receive insufficient and/or ineffective diagnostic workups. A limiting factor for prompt and accurate diagnosis is the scarcity of practical and effective concussion diagnostics that can be used by private-practice physicians. This gap in the diagnostic process can delay the implementation of crucial interventions. The absence of an affirmative concussion diagnosis leaves patients with few options when follow-up treatment would otherwise be indicated, potentially leading to worsening symptoms, prolonged recovery, and a higher likelihood of developing long-term co

Neurophysiological Markers of Cognitive Failures in Drivers: An Electroencephalography Study.

INTRODUCTION: Cognitive failures (CFs) during driving significantly contribute to traffic accidents and fatalities. This study aimed to investigate neurophysiological markers of CF in drivers using electroencephalography (EEG). METHODS: Thirty taxi drivers were classified into high and low CF groups based on CF questionnaire (CFQ) scores. EEG signals were recorded during eyes-closed rest and eyes-open Go/No-Go (GNG) tasks to assess brainwave patterns and lobe-specific activation. Statistical analyses included t-tests, repeated measures analysis of variance (ANOVA), and Pearson's correlations.

Sleep Fragmentation Modulates the Neurophysiological Correlates of Cognitive Fatigue.

Cognitive fatigue (CF) is a critical factor affecting performance and well-being. It can be altered in suboptimal sleep quality conditions, e.g., in patients suffering from obstructive sleep apnea who experience both intermittent hypoxia and sleep fragmentation (SF). Understanding the neurophysiological basis of SF in healthy individuals can provide insights to improve cognitive functioning in disrupted sleep conditions. In this electroencephalographical (EEG) study, we investigated in 16 healthy young participants the impact of experimentally induced SF on the neurophysiological correlates of

Intraoperative neurophysiological monitoring and spinal cord stimulator implantation.

INTRODUCTION: Spinal cord injury (SCI) is one of the most dreaded complications after spinal cord stimulation (SCS) implantation surgery. As a result, intraoperative neurophysiological monitoring (IONM) has been proposed to avoid accidental damage to nervous structures under anesthesia and confirm positioning for optimal stimulation. Our study uses a large administrative claims database to determine the 30-day risk of SCI after SCS implantation. METHODS: This retrospective cohort study used the IBM MarketScan Commercial and Medicare Supplemental Databases from 2016 to 2019. Adult patients und

Neurophysiological consequences of synapse loss in progressive supranuclear palsy.

Synaptic loss occurs early in many neurodegenerative diseases and contributes to cognitive impairment even in the absence of gross atrophy. Currently, for human disease there are few formal models to explain how cortical networks underlying cognition are affected by synaptic loss. We advocate that biophysical models of neurophysiology offer both a bridge from preclinical to clinical models of pathology and quantitative assays for experimental medicine. Such biophysical models can also disclose hidden neuronal dynamics generating neurophysiological observations such as EEG and magnetoencephalog…

Neurophysiological, Neuroimaging, and Neuropsychological Predictors of Human Alcoholism and Risk.

Over the last several decades, both brain electrophysiological measurements, such as electroencephalogram (EEG) and event-related potentials/oscillations (ERPs/EROs), and neuroimaging measures have immensely contributed to our understanding of neural mechanisms underlying various psychiatric disorders, including alcohol use disorder (AUD). This Special Issue was launched to invite research and review articles that explore the utility of these neural measures to determine the effects of alcohol use (e.g., regular drinking, social drinking, binge/heavy drinking, and chronic drinking) on brain st
