Pattern of neurosurgical cases and procedures in Gilgit Baltistan: two-year experience at a newly established neurosurgical department.

BACKGROUND: In Pakistan, there are a total of 160 neurosurgeons that constitute a ratio of one neurosurgeon for a 5.5 million population. Gilgit Baltistan being a backward region does not have a single tertiary care facility. A new department of neurosurgery was established at Regional Headquarter City Hospital (RHQ) Gilgit with only one neurosurgeon. This study aimed to determine which neurosurgical diagnoses are common and which surgical interventions were performed at RHQ. METHODS: This is a retrospective cross-sectional study conducted at Regional Headquarter Hospital (RHQ) Gilgit in whic…

Effect of Tranexamic Acid on Blood Loss and the Quality of Surgical Field in Transsphenoidal Pituitary Surgeries: Double-Blind Placebo-Controlled Randomized Control Trial.

BACKGROUND AND AIMS: Pituitary adenomas are common intracranial neoplasms and several cases require surgery, radiotherapy or radiosurgery. Transsphenoidal access to the pituitary gland is the commonest surgical approach. In microscopic or endoscopic approach to the pituitary, even modest bleeding can significantly worsen the surgical field for the neurosurgeon, lengthen intra-operative time and lead to potentially catastrophic complications. METHODS: The investigators hypothesized that administration of tranexamic acid (TXA) would improve the quality of the surgical field and reduce bleeding

Comparative sensitivity and specificity of change in third ventricular diameter or global ventricular change for detecting pediatric ventriculoperitoneal shunt malfunction: a 10-year retrospective cohort study.

OBJECTIVE: Clinical features of shunt malfunction are often nonspecific and neuroimaging is a well-established aid in diagnosis. In practice … ing. Quantitative change was compared to the overall impression of change in ventricular caliber by neurosurgeons for detecting shunt malfunction. Shunt malfunction was defined as the need to replace one or more shunt components at the time of surgery (diagnostic gold standard). Diagnostic performance was assessed using comparison of areas under the receiver op …

Introduction: surgical management of skull base meningiomas.

Meningiomas represent the most common primary intracranial neoplasm treated by neurosurgeons. Although multimodal treatment of meningiomas includes surgery, radiation-based treatments, and occasionally medical therapy, surgery remains the mainstay of treatment for most symptomatic meningiomas. Because of the intricate relationship of the dura mater and arachnoid mater with the central nervous system and cranial nerves, meningiomas can arise anywhere along the skull base or convexities, and occasionally even within the ventricular system, thereby mandating a catalog of surgical approaches that …

Fetal ventriculomegaly: prognosis in cases in which prenatal neurosurgical consultation was sought.

OBJECT: The prognosis of fetal ventriculomegaly (FVM) varies because of the disease's heterogeneity and the diversity of accompanying anomalies. Moreover, the cases that are referred to neurosurgeons may have different clinical features from those typically encountered by obstetricians. The object of this study was to delineate the prognosis of FVM in cases for which neurosurgical consultation was sought. METHODS: Forty-four cases of FVM that were diagnosed before birth and referred to neurosurgeons for prenatal consultation were analyzed retrospectively. Twenty-five of the 44 patients had ac…

Three-Dimensional Printed Skull Base Simulation for Transnasal Endoscopic Surgical Training.

OBJECTIVE: Transnasal endoscopic skull base surgery (SBS) presents a major challenge for inexperienced neurosurgeons because of the complicated anatomic structures, 2-dimensional endoscopic view, limited operative field, and required skills. We designed a personalized multimaterial and multicolor three-dimensional (3D)-printed SBS simulation to reproduce the complex anatomy of the skull base. The fidelity and feasibility for anatomic education and surgical training were assessed. METHODS: Two-dimensional computer tomography and magnetic resonance images were collected from a 42-year-old healt…

Pioneering a Neurosurgical Digital Subtraction Angiography (DSA) Program: Lessons From 100 Cases Using 3D C-arm Technology.

INTRODUCTION: Digital subtraction angiography (DSA) remains the gold standard for diagnosing and managing cerebrovascular diseases, despite the increasing use of noninvasive imaging modalities. Traditionally performed in catheterization labs by interventional radiologists, the integration of DSA into neurosurgery operating theaters (OT) using 3D C-arm technology offers neurosurgeons greater control over diagnosis and treatment planning, reducing dependency on external specialists. OBJECTIVE: This study aims to describe the initiation and implementation of a DSA program in a neurosurgery OT us…

Skin glomic tumors referred for local pain and cured by surgical removal.

BACKGROUND: Under the definition "glomus tumors" are often erroneously enclosed neoplasms that are absolutely unlike for origin, location, and behavior. Glomus tumors (GTs) are small but extremely painful skin tumors of mesenchymal origin. GTs derive from the neuromyoarterial glomus in adults of middle-age and are generally benign. Due to their small size, diagnosis is often difficult and patients harboring these tumors usually consult many physicians, including sometimes neurosurgeons. More familiar to neurosurgeons are neoplasms as glomus jugular, glomus vagale, and glomus tympanicum that in…
