[Pain syndromes with causal participation of the sympathetic nervous system].

The efferent sympathetic nervous system is organized into subsystems that innervate and regulate via separate peripheral sympathic pathways the different autonomic target organs. This review discusses mechanisms through which this efferent system may be causally involved in the generation of pain. Clinical pain syndromes in which this may be the case are "complex regional pain syndromes" (CRPS) type I (previously reflex sympathetic dystrophy) and type II (recently causalgia). The "sympathetically maintained pain" (SMP) is a symptom (and not a clinical entity) that can principally also be prese

The aging brain and cerebrovascular reactivity.

Cerebrovascular reactivity (CVR) is a measure of vascular response to a vasoactive stimulus, and can be used to assess the health of the brain vasculature. In this current study we used different analyses of BOLD fMRI responses to CO2 to provide a number of metrics including ramp and step CVR, speed of response and transfer function analysis (TFA). 51 healthy control volunteers between the ages of 18-85 (26 males) were recruited and scanned at 3T field strength. Atlases reflecting voxel-wise means and standard deviations were compiled to assess possible differences in these metrics between fou

Baroreflex and neurovascular responses to skeletal muscle mechanoreflex activation in humans: an exercise in integrative physiology.

Cardiovascular adjustments to exercise resulting in increased blood pressure (BP) and heart rate (HR) occur in response to activation of several neural mechanisms: the exercise pressor reflex, central command, and the arterial baroreflex. Neural inputs from these feedback and feedforward mechanisms integrate in the cardiovascular control centers in the brain stem and modulate sympathetic and parasympathetic neural outflow, resulting in the increased BP and HR observed during exercise. Another specific consequence of the central neural integration of these inputs during exercise is increased sy

Evidence for temperature-mediated regional increases in cerebral blood flow during exercise.

Aerobic exercise elicits increases in cerebral blood flow (CBF), as well as core body temperature; however, the isolated influence of temperature on CBF regulation during exercise has not been investigated The present study assessed CBF regulation and neurovascular coupling during submaximal cycling exercise and temperature-matched passive heat stress during isocapnia (i.e. end-tidal PCO2 was held constant) Submaximal cycling exercise and temperature-matched passive heat stress provoked ∼16% increases in vertebral artery blood flow, independent of changes in end-tidal PCO2 and blood pressure E…

Emerging functionalized artificial periostea: Diverse origins, multifaceted roles and therapeutic frontiers.

The periosteum, a specialized connective tissue layer enveloping bone surfaces, serves as a critical regulator of bone healing through its integrated neurovascular architecture, stem cell niche properties, and mechanical barrier function. Clinically, traumatic injuries or pathological conditions frequently result in periosteal tearing, avulsion, or complete loss, fundamentally disrupting its biological integrity. In practice, repairing a torn or absent periosteum is difficult, because existing options rarely recreate its coordinated biological cues and protective function. The limited availabi

From hypoxic pockets to daily routines: linking brain oxygenation and cognitive resilience.

The discovery of hypoxic pockets within the cortical regions has transformed the understanding of cerebral oxygen dynamics, revealing their dual role as both contributors to neuronal adaptation and potential precursors to dysfunction. These transient oxygen-deprived microenvironments play a pivotal role in neurovascular coupling, synaptic plasticity, and angiogenesis, processes crucial for maintaining cognitive resilience and neuronal health. Investigating hypoxic pockets within cortical regions is particularly relevant in aging populations and individuals with neurodegenerative conditions. Co…

Chronic neurostimulation of splenic nerve enabled by hydrogel-bioelectronics for wireless electroceutical immunomodulation therapy.

Targeted modulation of the splenic nerve offers a selective alternative to vagus nerve stimulation, avoiding off-target activation of mixed vagal fibers. However, the small diameter and anatomical complexity of the splenic nerve pose great challenges for stable neural interfacing. Here, we report a splenic nerve wireless stimulator (SpNWS) built from stretchable and highly conductive hydrogel for chronic electroceutical immunomodulation therapy. The hydrogel-fiber neural electrodes conform to the geometries of splenic neurovascular bundles (SNVBs) and are secured using a bioadhesive approach,

Interactions between glial cells and the blood-brain barrier and their role in Alzheimer's disease.

Alzheimer's disease (AD), which is an irreversible neurodegenerative disorder characterized by senile plaques and neurofibrillary tangles, is the most common form of dementia worldwide. However, currently, there are no satisfying curative therapies for AD. The blood-brain barrier (BBB) acts as a selective physical barrier and plays protective roles in maintaining brain homeostasis. BBB dysfunction as an upstream or downstream event promotes the onset and progression of AD. Moreover, the pathogenesis of AD caused by BBB injury hasn't been well elucidated. Glial cells, BBB compartments and neuro

Mild Propofol Sedation Reduces Frontal Lobe and Thalamic Cerebral Blood Flow: An Arterial Spin Labeling Study.

Mechanisms of anesthetic drug-induced sedation and unconsciousness are still incompletely understood. Functional neuroimaging modalities provide a window to study brain function changes during anesthesia allowing us to explore the sequence of neuro-physiological changes associated with anesthesia. Cerebral perfusion change under an assumption of intact neurovascular coupling is an indicator of change in large-scale neural activity. In this experiment, we have investigated resting state cerebral blood flow (CBF) changes in the human brain during mild sedation, with propofol. Arterial spin label…

Brain Metabolic Alterations in Alzheimer's Disease.

The brain is one of the most energy-consuming organs in the body. Satisfying such energy demand requires compartmentalized, cell-specific metabolic processes, known to be complementary and intimately coupled. Thus, the brain relies on thoroughly orchestrated energy-obtaining agents, processes and molecular features, such as the neurovascular unit, the astrocyte-neuron metabolic coupling, and the cellular distribution of energy substrate transporters. Importantly, early features of the aging process are determined by the progressive perturbation of certain processes responsible for adequate bra

Transcranial low-intensity pulsed ultrasound in neurological disorders: mechanisms, therapeutic applications, and translational challenges.

Transcranial low-intensity pulsed ultrasound (LIPUS) is an emerging non-invasive modality with high spatial precision, substantial tissue penetrability, and favorable biosafety. Acting predominantly through mechanical rather than thermal bioeffects, LIPUS modulates mechanosensitive ion channels, intracellular calcium signaling, synaptic transmission, glial activation, neurovascular coupling, and, in selected settings, blood-brain barrier permeability. These features support its growing application in neurological disorders. In this review, we summarize the mechanistic basis of transcranial LIP…

Beyond Osteoarthritis: Alternative Applications and Indications for Genicular Artery Embolization.

Genicular artery embolization (GAE) is a minimally invasive therapy for knee osteoarthritis (OA), originally performed in patients with recurrent hemarthrosis, that targets pathologic synovial hypervascularity and neurovascular coupling to reduce pain and inflammation. However, these biological mechanisms are not specific to degenerative disease. Synovial hyperemia, angiogenesis, and capillary fragility are also described in patients with persistent pain following total knee arthroplasty and patients with inflammatory arthritides, suggesting a mechanistic role for embolization in these patient…

Spinal ultrasound imaging implied the vascular dynamics in chronic and acute pain.

Neurovascular coupling links local neural activity to cerebral blood flow changes, crucial for pain transduction. In this undertaking, we present an advanced spinal functional ultrasound imaging that exhibits precise observation capabilities of the influence of analgesic medications on spinal cord blood flow, encompassing both acute and chronic pain scenarios. Utilizing gabapentin as the primary analgesic agent, we have observed that intrathecal administration of gabapentin notably augments the blood flow intensity in neuropathic rats while exerting minimal influence on the blood flow of the s…

Imaging Agonist-Induced D2/D3 Receptor Desensitization and Internalization In Vivo with PET/fMRI.

This study investigated the dynamics of dopamine receptor desensitization and internalization, thereby proposing a new technique for non-invasive, in vivo measurements of receptor adaptations. The D2/D3 agonist quinpirole, which induces receptor internalization in vitro, was administered at graded doses in non-human primates while imaging with simultaneous positron emission tomography (PET) and functional magnetic resonance imaging (fMRI). A pronounced temporal divergence between receptor occupancy and fMRI signal was observed: occupancy remained elevated while fMRI responded transiently. Anal
