Congenital diaphragmatic hernia: Part I.

The infant with a congenital diaphragmatic hernia presents many challenges to those caring for him. A thorough understanding of both anatomic and physiologic components can enable the caregiver to accurately assess the patient and employ interventions which will minimize complications. This article discusses the incidence, mortality, embryology, diagnosis, pathophysiology, and conventional management. Part II will discuss alternatives to conventional management, outcome predictors, current research, and follow-up of the infant with a congenital diaphragmatic hernia.

Congenital hip dislocation: the importance of early detection and comprehensive treatment.

Hip dislocation is the musculoskeletal condition most commonly missed during neonatal examinations. Failure to diagnose the condition can lead to long-term disability and is a common target in pediatric legal suits concerning damage to the musculoskeletal system. Early neonatal assessment for hip dislocation includes an examination using the Barlow and Ortolani tests. Later examinations include assessment of gluteal folds, knee height and the degree of hip abduction. The preferred treatment is use of the Pavlik harness, an outpatient treatment regime that provides effective reduction in 90 per

Nursing Care Plan for a Newborn with the Defect of Congenital Gastroschisis in the Postoperative Period Using ICNPTM and the Dedicated Software.

BACKGROUND: Congenital defect gastroschisis manifests as a defect in the sheath in the intestine of the newborn, which is not covered by the hernia sac. In this case, the priority task of the neonatal nurse is to diagnose patient care problems quickly and accurately. Choosing the correct care plan elements has a significant impact on shortening the duration of hospitalization, reducing the number and severity of complications, and preventing their recurrence. The purpose of this study was to formulate a care plan for a newborn with diagnosed congenital defect gastroschisis in the postoperative

Suppression of milk-derived miR-148a caused by stress plays a role in the decrease in intestinal ZO-1 expression in infants.

BACKGROUND & AIMS: Milk-derived miR-148a-3p (miR-148a), which is abundant in breast milk, has been shown to be associated with the development of infants' intestines. Although it is well known that stress during lactation changes milk constituents in terms of lipid and protein, no studies have examined the influence of stress on miR-148a expression in breast milk. The objective of this study is to investigate the relationship between stress and miR-148a expression in milk, and to evaluate whether the changes in milk-derived miR-148a expression-caused by the mother's exposure to stress-influenc

Separation of conjoined twins: a case study in critical care.

Conjoined twinning, one of the rarest and perhaps most fascinating congenital malformations in children, presents a major challenge to the healthcare team. This article describes a case study of the course of female omphalopagus twins. Favorable anatomy and the combined expertise of nurses, physicians, and supporting hospital staff during perinatal, neonatal, surgical, and postoperative care and management led to the successful separation of these conjoined twins.

Management of neonatal jaundice.

Jaundice is the most commonly encountered neonatal clinical problem; 80 percent of neonates become clinically jaundiced, while 5 percent develop serum bilirubin levels above currently recommended treatment standards. This article outlines theories about the pathophysiology of neonatal jaundice and presents a logical approach to its management. First, the health care provider must distinguish between physiologic, exaggerated and pathologic jaundice. The treatment modalities of exchange transfusion, phototherapy and cessation of nursing are discussed.

Parents and nurses balancing parent-infant closeness and separation: a qualitative study of NICU nurses' perceptions.

BACKGROUND: When a newborn requires neonatal intensive care unit (NICU) hospitalization, parent and infant experience an unusual often prolonged separation. This critical care environment poses challenges to parent-infant closeness. Parents desire physical contact and holding and touching are particularly important. Evidence shows that visitation, holding, talking, and skin to skin contact are associated with better outcomes for infants and parents during hospitalization and beyond. Thus, it would be important to understand closeness in this context. The purpose of this study was to explore fr

Neonatal diabetes mellitus.

Neonatal diabetes mellitus is an uncommon disorder of unknown cause that can be encountered in the first days to weeks of life. Two forms--transient and permanent--have been described. Signs and symptoms of both forms are identical with the ongoing need for insulin therapy past the neonatal period the only method to distinguish between them. This article describes the infant with transient neonatal diabetes and reviews the clinical characteristics of both the transient and permanent forms. The role of the nurse practitioner in the care and management of these infants is also addressed.

Background Healthcare-associated infections (HAIs) are a major cause of morbidity, mortality, and healthcare costs. Nurses in critical care units play a central role in infection prevention, but gaps in knowledge, attitudes, and practices (KAP) often compromise patient safety. Educational interventions provide a feasible strategy to strengthen infection control competencies. Aim This study evaluated the effectiveness of a structured instructional module in improving nurses' KAP regarding HAI prevention in tertiary care hospitals of Gautam Budh Nagar district, Uttar Pradesh, India. Materials an

Infants of diabetic mothers: the effects of hyperglycemia on the fetus and neonate.

News that a woman with diabetes is about to deliver brings up images of a macrosomic infant. This infant may experience birth injuries, asphyxia, respiratory distress, hypoglycemia, hypocalcemia, hyperbilirubinemia, polycvthemia/hyperviscosity syndrome, asymmetric sepral hypertrophy, and other congenital malformations. Uncontrolled diabetes has profound effects on embryogenesis, organogenesis, and fetal and neonatal growth, and evidence increasingly indicates that some of these effects are lifelong and may contribute to adult obesity. Preconception control of diabetes and monitoring throughout

"Caution! Contents should be cold": developing a whole-body hypothermia program.

Hypoxic-ischemic encephalopathy (HIE) is characterized as brain injury that results from lack of oxygen or blood flow to the brain in the perinatal period. Neonatal whole-body hypothermia and selective head cooling are becoming increasingly common care practices across the U.S. and Canada for infants with moderate-to-severe HIE because of the demonstrated ability of these approaches to reduce reperfusion injury to the brain. Health care professionals must develop a clinical care path for these fragile infants. For best results, induced hypothermia should be initiated within six hours of birth;

The relationship between feeding and necrotizing enterocolitis in very low birth weight infants.

Necrotizing enterocolitis (NEC) is the most common gastrointestinal emergency in the NICU, with often devastating consequences. The etiology of NEC is probably multifactorial, with preterm infants at the highest risk. The relationship between feeding and NEC was identified in the 1970s, leading to delayed feeding becoming standard treatment in NICUs. More recent research suggests that early feedings not only are safe, but reduce other morbidities associated with prematurity. Standardized feeding guidelines seem to confer some benefits in decreasing NEC, despite a wide variability in feeding pr

Treatment of physiological and pathological neonatal jaundice.

Neonatal jaundice (hyperbilirubinaemia) is a common condition and usually a benign transitional event that resolves without treatment. However, in some infants it can be a symptom of an underlying pathological condition, which is important to identify. If bilirubin levels necessitate it, treatment for jaundice involves phototherapy and/or exchange transfusion of donor blood. In cases of pathological jaundice the underlying cause must also be treated. Parental involvement is important to minimise the trauma of having a sick baby and its effect on bonding.
