Opsoclonus is a rare disorder of the saccadic system, in which fixation is continuously interrupted by multivectorial, back-to-back saccades that at times can be seen only with an ophthalmoscope. To diagnose it reliably, eye movement recording is required. Opsoclonus may be a harbinger of an occult malignancy, though many cases are postinfectious, toxic-metabolic or idiopathic. The underlying malignancy is usually neural crest tumors in children and lung, breast, or gynecologic cancer in adults. Opsoclonus can be accompanied by myoclonus and ataxia. Concurrent appearance of oscillations affect

Is there an effect on binocular coordination during reading of oculomotor imbalance (heterophoria, strabismus and inadequate convergence) and of functional lateral characteristics (eye preference and perceptually privileged visual laterality)? Recordings of the binocular eye-movements of ten-year-old children show that oculomotor imbalances occur most often among children whose left visual perceptual channel is privileged, and that these subjects can present optomotor dissociation and manifest lack of motor coordination. Close binocular motor coordination is far from being the norm in reading.

A 32 year old patient with unexplained cough developed an opsoclonus-ataxic-myoclonic syndrome. Chest X-ray film were normal at this onset but became pathologic, and the diagnosis of multisystemic sarcoidosis was made. The possibility of sarcoidosis being the cause of the opsoclonus is discussed.

We report a patient who developed a delayed onset of oculopalatal myoclonus, characterized by synchronous oscillatory movements of the eyeballs, soft palate and the orbicularis oculi muscle, 5 months after a pontine hemorrhage. The involuntary oscillations did not affect the speech or swallowing but continued in sleep with decreased amplitudes. These oscillations also failed to respond to clonazepam therapy and the patient was unable to voluntarily suppress them for several minutes. A temporary association in synchronous titubation of the head was also observed. A characteristic vertical ocula

BACKGROUND: Fragile X-associated tremor/ataxia syndrome (FXTAS) is a neurodegenerative disorder characterized by ataxia, tremor, and parkinsonism. Eye motility abnormalities on the clinical examination of FXTAS patients have not been formally studied.
METHODS: A case-control study with fragile X gene mutation carriers with and without FXTAS and normal controls was conducted and included a videotaping of ocular items of the International Cooperative Ataxia Rating Scale (ICARS). A neuro-ophthalmologist blinded to gene status rated nystagmus, ocular pursuit, and saccades.
RESULTS: Forty-four ca

Myasthenia gravis is an autoimmune disorder affecting the neuromuscular junction. Ocular myasthenia gravis presents as ptosis with extraocular motility restriction and is prone to be misdiagnosed as third nerve palsy or congenital or aponeurotic ptosis. Juvenile ocular myasthenia gravis in very young children is difficult to diagnose and can be easily labeled as a case of congenital ptosis, the more so when the condition is bilateral. We present a case of a two-year-old child who presented with bilateral ptosis and was diagnosed as a case of simple congenital ptosis elsewhere with the advice t

The clinical signs of the rarely encountered ocular neuromyotonia consist of transient involuntary tonic contraction and delayed relaxation of single or multiple extraocular muscles, resulting in episodic diplopia. With a mean time delay of 3.5 years, this motility disorder frequently follows tumor excision or adjuvant radiation near the skull base. Ocular neuromyotonia may reflect inappropriate discharge from oculomotor neurons with unstable cell membranes because of segmental demyelinization by tumor compression and radiation-induced microangiopathy. In the present paper, the authors present

BACKGROUND: Dyslexia is defined as a reading and/or writing disability persisting after exclusion of organic causes. Studies show that ocular disorders, especially small refraction errors, hypoaccommodation and symptomatic heterophoria, are often not detected or treated in cases of reading and/or writing problems which were otherwise diagnosed as dyslexia.
METHODS: We evaluated the data of patients referred to our department from December 1997 to March 2000 with the diagnosis of dyslexia.
RESULTS: We found ocular disturbances in 28 (84.8%) out of 33 children, 26 (78.8%) showed improved readi

BACKGROUND: The ocular signs of internuclear ophthalmoplegia (INO) with slow, restricted adduction of one eye and abduction nystagmus of the contralateral eye are easily recognized and have a high localizing/lateralizing value. However, subtle INO is difficult to diagnose. Recent reports identified novel vestibular abnormalities in unilateral and bilateral INO. Frequent findings include decreased horizontal and posterior canal gains, and generally relative sparing of both anterior canals. We studied one patient with a subtle bilateral INO, performed serial quantitative saccade (QS) and video h

Anecdotal oculomotor disturbances have been described in spastic paraplegia type 7 (SPG7). We investigated oculomotor and vestibular dysfunction in five patients with genetically verified SPG7. All five patients exhibited significantly slower velocities of vertical saccades compared to controls, but significantly faster than in progressive supranuclear palsy, with upward saccades being particularly affected. Horizontal saccades, cerebellar oculomotor markers, and vestibuloocular reflex seem to be variably affected. Thus, albeit subclinical in some cases, slowing of the vertical saccades may be

PURPOSE OF REVIEW: The components of the efferent ocular motor network include supranuclear, nuclear, internuclear, infranuclear, neuromuscular junction, and muscle. Within this schema, clinicians are often least acquainted with the supranuclear components; however, derangement of this system is a common cause of ocular dysmotility and diplopia. This article will provide the neurologist with an overview of the anatomy and clinical aspects of the supranuclear ocular motor control systems.
RECENT FINDINGS: Continued research moves us toward a more complete understanding of the anatomy and physi

Abnormal eye movements in the infant or voting child can be congenital or acquired. They may be a result of abnormal early visual development or a sign of underlying neurologic or neuromuscular disease. It is important to be able to detect these abnormalities and to distinguish them from normal but immature eye movements. The spectrum of disease in children differs from that in adults. Serious, potentially fatal but treatable disorders can be acquired in infancy, and abnormal eye movements in a sometimes apparently well child should never be labeled as congenital or benign without careful inve

INTRODUCTION: Parkinson's disease is associated with multiple abnormalities of both the afferent and efferent visual systems. Blepharospasm, paucity of blinking, apraxia of lid opening, visual neglect, reduced vergence, reduced upgaze, and blurred vision are reported findings in these patients. The association of these findings with the disease, and their duration, severity, and treatment have not been systematically investigated.
PATIENTS AND METHODS: Patients with Parkinson's disease were prospectively examined. An age-matched control group was recruited from accompanying family members and

A 19-year-old female was seen at the emergency department following an auto-intoxication. An oculogyric crisis (ogc) was observed, in the absence of other extrapyramidal symptoms (eps). In a second anamnesis, patient indicated that she had taken risperidone 3 mg (an atypical antipsychotic). This particular case description of an isolated ogc shows that care providers should be attentive to the occurrence of ogc, even if the most frequent eps are absent. This case also emphasizes the importance of a complete history in order to efficiently and timely guide the care provider to the correct diagn

Inborn errors of metabolism in adults are still largely unexplored. Despite the fact that adult-onset phenotypes have been known for many years, little attention is given to these disorders in neurological practice. The adult-onset presentation differs from childhood-onset phenotypes, often leading to considerable diagnostic delay. The identification of these patients at the earliest stage of disease is important, given that early treatment may prevent or lessen further brain damage. Neurological and psychiatric symptoms occur more frequently in adult forms. Abnormalities of eye movements are

PURPOSE: This study aims to evaluate the efficacy of a new method for measuring the near point of convergence (NPC) using a Nonius polarized target stimulus, comparing its sensitivity in detecting convergence ability to that of traditional approaches.
METHODS: In this prospective study, 65 participants underwent near point of convergence (NPC) measurements using three methods: an accommodative target (NPC1), a penlight with a red filter (NPC2), and a Nonius polarized target (NPC3). Participants also completed the Convergence Insufficiency Symptom Survey (CISS). Statistical analyses, including
